1 SECTION 4 - Other Operator Services Original Sheet 1 BUSY LINE VERIFICATION AND INTERRUPTION OF CONVERSATION Upon request, where the type of customer facilities will permit, the operator will verify and provide the status of a subscriber line, subject to the charge below. This charge applies to requests from coin and non-coin telephones. The charge does not apply when verification indicates a trouble condition. When the caller requests that the operator interrupt a conversation, an additional charge applies. These charges do not apply when the interruption request is for an emergency call to or from Official Law Enforcement Agencies or Fire Departments..

3 SECTION 4 - Other Operator Services Original Sheet 3 TEMPORARY INTERCEPTION OF SERVICE General Temporary Interception of Service is available at the request of the subscriber to the extent that equipment and number assignments permit. Arrangements for the service are made with the understanding that the subscriber assumes all risk in connection therewith, and that no liability attaches to the Telephone Company by reason of failure to complete any call. The service is furnished subject to receipt of the subscriber's request in sufficient time to permit the Telephone Company to effect the necessary arrangements. Temporary Interception of Service is offered in the following arrangements. Temporary Number Referral provides for business or residence service to be intercepted temporarily without termination of service. Calling persons will be informed that the service is being temporarily intercepted at the request of the subscriber and, if desired by the subscriber, the telephone number at which he may be reached or the telephone number at which his calls will be received during the period of interception. No outward or inward service is contemplated during the period of interception. Multiple Number Referral provides for multiple intercept messages in the event a business or residence telephone service is disconnected and the subscriber requests that the calls be referred to more than one telephone number. Persons calling the disconnected service will reach an operator who will determine who the caller is trying to reach and will then route the call to the appropriate intercept phrase via the Automatic Intercept System. Direct Inward Dial (DID)/Centrex/Advanced Centrex Number Referral provides for a disconnected or vacant DID, Centrex or Advanced Centrex station, that is not listed in our directory, to route via the Automatic Intercept System to the appropriate intercept message. Regulations Temporary Number Referral is provided for a one month minimum service period. Multiple Number Referral is provided for period of three months for Residence customers and for a period of twelve months for Business customers. No extensions of the service period are permitted.
4 SECTION 4 - Other Operator Services Original Sheet 4 TEMPORARY INTERCEPTION OF SERVICE (cont d) Regulations (cont'd) DID/Centrex/Advanced Centrex Number Referral is provided for a period of twelve months. No extensions of the service period are permitted. There will be no charge for Temporary Interception of Service if necessary for company reasons. The charge to establish temporary interception of service and the monthly interception charge applies for each line or trunk intercepted. In the case of rotary service, the main listed number and any auxiliary numbers listed as an additional listing will be intercepted. In addition, at the customer's request, other numbers in rotary service but not listed may be intercepted.
5 SECTION 4 - Other Operator Services Original Sheet 5 TEMPORARY INTERCEPTION OF SERVICE (cont'd) Prices Temporary Number Referral Nonrecurring Price To establish Temporary Number Referral Service Request Charge-Initial (See Section 3) For each charge in announcement No Charge In addition, an interception charge of $12.50 /TYS/ per month for business service and $7.95 /TYS/ per month for residence service applies for each period of interception. A new period of interception will begin with each change in announcement requested by the subscriber. The minimum interception charge for each period of interception shall be $12.50 for business service and $7.95 for residence service. Multiple Number Referral Nonrecurring Price Residence Business Multiple Number Referral $ /1/ $ /1/ Changes to an existing Multiple Number Referral service will not cause an extension to the length of the original service period. DID/Centrex/Advanced Centrex Number Referral Nonrecurring Price DID/Centrex/Advanced Centrex Number Referral $41.00 /1/ For each additional station Changes to an existing DID/Centrex/Advanced Centrex Number Referral service will not cause an extension to the length of the original service period. /1/ A Service Request Charge, as found in Part 3, is applicable.
6 SECTION 4 - Other Operator Services Original Sheet 6 EXTENDED INTERCEPT SERVICE A. Description Extended Intercept Service (EIS) provides an additional announcement intercept other than nonworking station announcement and reference to listed directory number. EIS provides an intercept message for Direct Inward Dialing (DID) customers who disconnect or change telephone numbers. EIS is designed to give a separate and discrete intercept message per affected DID number. Calls to any number having EIS on the DID service station line, shall, upon disconnection of these lines, be referred to individual intercept announcements which will provide the calling party with the called party's new individual line number. B. Terms and Conditions EIS will be provided subject to the availability of telecommunication facilities. Arrangements for the service are made with the understanding that the customer assumes all risk in connection therewith and that no liability attaches to the Telephone Company by reason of failure to complete any call. EIS is furnished subject to receipt of a customer s request in sufficient time to permit the Telephone Company to effect the necessary arrangements. The following rates and charges do not apply to the attendant interception of calls to non-working stations of a system, or to the mechanical interception of calls to non-working stations of a system, when used in lieu of the attendant intercept.

8 SECTION 4 - Other Operator Services Original Sheet 8 INTERCEPT REFERRAL EXTENSION SERVICE General Intercept Referral Extension Service is provided to business customers for the purpose of notifying calling parties about changes in the status of a business customer s telephone number when the number has been called. Intercept Referral Extension Service is available for a maximum of nine months following the initial free, three month period of regular intercept. Subsequent to the initial three months, Intercept Referral Extension Service will be charged on a per month basis. Upon transfer, telephone number change or disconnection of the business access line, the customer will specify the number of months which the Intercept Referral Extension Service is required. Intercept Referral Extension Service is provided as facilities permit at the discretion of the Company. Intercept Referral Extension Service is available only for business access line customers and not available for Centrex service in conjunction with PBX.
9 SECTION 4 - Other Operator Services Original Sheet 9 INTERCEPT REFERRAL EXTENSION SERVICE (cont d) Prices No service charge will be applied for Intercept Referral Extension Service when requested on the same order for a disconnection of service, telephone number change or a transfer to another address. If a subsequent request is made to add or change the duration of the Intercept Referral Extension Service, a subsequent service order charge is applicable. Nonrecurring Price Monthly Price Intercept Referral Extension Service - per account intercepted /a/ $6.00 /a/ A service order charge as specified in Part 3 is applicable to add or change the length of months requested for the Intercept Referral Extension Service. A change request is not available for services which have been terminated.
