3 EXECUTIVE SUMMARY As the healthcare industry transitions from a volume-based to a value-based patient care model, seamless workflows, access to data, and collaboration across a healthcare provider organization are more important than ever. Meanwhile massive consolidation and creation of new entities in the industry require providers to minimize TCO and complexity of healthcare IT infrastructure. A key part of this infrastructure is for managing medical images and other unstructured data. Healthcare providers are increasingly adopting Vendor Neutral Archive (VNA) or similar methods to break down departmental silos and centralize archive, management and delivery of medical images and other unstructured data in an enterprise repository, which allows more efficient use of IT assets and lower TCO, facilitates easier access and collaboration, and enables better patient care. efficiency, resilience and simplified management. The Conserus suite of Image Repository, Enterprise Viewer, and applications provides this type of enterprise repository or VNA, along with additional features that enhance medical imaging workflows. Dell EMC Conserus Solution is a Hyper- Converged Infrastructure (HCI) solution that is designed using the Dell EMC converged hardware platform and VMware Software Defined Storage (SDS) technology for Conserus workload requirements, and validated by a joint McKesson-Dell EMC-VMware solution integration team. The solution provides healthcare customers a flexible and scalable architecture for their implementation of Conserus, with additional benefits in 3

4 HEALTHCARE DATA CHALLENGES The healthcare industry is moving from a volume-based to a value-based patient care model, creating a shift towards consolidation of healthcare providers and payers, and creation of new entities. The transition from paper records to electronic medical records (EMR) has been completed for most healthcare providers contributing to the doubling of healthcare data every two years. All of these factors present an acute data management challenge, requiring providers to minimize TCO and complexity of their healthcare IT infrastructure. Figure 1. Healthcare Data Growth Trend Exabytes (EB) 2,500 2,000 1,500 1, Healthcare Data VENDOR NEUTRAL ARCHIVE OVERVIEW To address healthcare data management challenges and facilitate easier access and collaboration across an entire healthcare enterprise, providers are increasingly adopting solutions that centralize the archiving, management and delivery of medical images and other unstructured data into an enterprise repository. This enterprise repository is referred to as a vendor neutral archive (VNA) because it can use standard formats to store and manage medical images and unstructured data independently from the systems that generated the data, without locking providers into specific vendor formats or clinical systems such as a picture archiving and communication system (PACS). VNA is a solution that centralizes the archiving and management of images and unstructured data generated by disparate systems, and enables access and sharing across a healthcare enterprise. Older data needs to be archived for reference and compliance. A healthcare organization needs to store and manage petabytes of data, causing mounting IT challenges and costs. In addition, departmental silos of imaging data impede enterprise-wide access and collaboration. Labs / Opthalmology / Pathology Other Specialty Digital Images Individual Departmental Archives Multiple Clinical Viewers For a healthcare provider, deployment of a VNA solution can start with a single department and then expand into an enterprise-wide system over time, achieving one of the three levels with increasing degrees of image consolidation and scope managed by the VNA. Namely: Level 1 Department-Specific DICOM: Data for single PACS stored neutrally to support future choice and provide continuity, often used for a PACS in a single department. Level 2 Enterprise-wide DICOM: Support for storing data from multiple PACS in one or more departments, includes DICOM viewing. Level 3 Enterprise-wide, All Image Formats: Support for managing non-dicom clinical data using industry standards. 4

5 CONSERUS OVERVIEW The Conserus suite of flexible, comprehensive and vendor-neutral solutions helps orchestrate patient care across a healthcare provider s entire enterprise, putting the right imaging data in the right hands at the right time. Conserus helps customers to achieve Level 3 enterprise-wide VNA deployment. Conserus Image Repository Conserus Image Repository (CIR) is a VNA solution that provides an efficient way to archive, manage and deliver medical imaging data. CIR brings together scattered images from siloed systems for radiology, cardiology and other ologies within an organization, and provides a single view of a patient s medical imaging history, accessible from anywhere across the enterprise. CIR features include: Enterprise image archive, independent from any PACS Ability to share image data between disparate healthcare enterprises or systems, when permitted Single view of patient image data across the enterprise Ability to manage compliance with retention requirements for image data established by the provider CIR also helps reduce image management-related costs through: Economies of scale by investing in a single larger storage platform for all image data Centralized management to help reduce the number of individual systems that need maintenance and disaster recovery plans Savings from never needing to migrate PACS data again Conserus Enterprise Viewer Conserus Enterprise Viewer (CEV) is a US FDA cleared image viewer (Class II) solution fit for diagnosis of all medical images, with the exception of mammography. CEV is also called an enterprise universal viewer because it can integrate seamlessly with PACS, VNA, EMR, and other clinical systems, enabling clinicians to access a patient s images and reports across the entire enterprise, perform medical diagnosis, and collaborate on care of the patient, anytime, anywhere, using supported web browsers and validated desktop and mobile devices. CEV benefits and features include: Access to medical images with a secure, zero-footprint viewer through HTML5-supported web browsers Launch in context from EMR Advanced software tools for diagnosis from any location Collaboration in real-time with other clinicians Support for multiple PACS and multi-vendor PACS environments Support for multiple device platforms, browsers, and screen sizes Secure architecture and server-side rendering to protect patient data Figure 4. Conserus Enterprise Viewer is a FDA cleared universal image viewer that is fit for diagnosis of most medical images and allows secure access to medical images and reports across a healthcare enterprise from any location. HYPER-CONVERGED INFRASTRUCTURE OVERVIEW Traditional 3-tier infrastructure design uses physical or virtual servers, shared storage such as Storage Area Network (SAN) and network fabrics. HCI design differs by combining compute, storage and virtualization in a node or an appliance that is built on standard-based server hardware. Multiple nodes or appliances form a cluster that is managed by virtualization and SDS software such as VMware vsan (vsan). HCI design allows a customer to save capital expense by buying infrastructure to meet exact needs, while ensuring the ability to scale out the infrastructure for future growth. The skillset required for implementing and managing a HCI solution already exists within many customers IT departments because of familiarity with virtualization technology and management tools. SDS features enable IT to shorten implementation windows, reduces management complexity and costs, and mitigates risks for business continuity. Figure 6. 7 DELL EMC CONSERUS SOLUTION Dell EMC HCI solutions are part of a larger SDS portfolio; HCI options include ready nodes (e.g. vsan ReadyNode in PowerEdge FX2) and appliances (e.g. PowerEdge VxRail and XC appliances). Dell EMC Conserus Solution is a HCI solution designed using PowerEdge FX2 converged hardware platform and VMware vsan SDS technology for specific requirements of Conserus workloads. The solution provides healthcare customers a flexible, scalable, efficient, manageable and reliable architecture for their implementation of Conserus. The solution is sold by McKesson, with hardware and software integrated at Dell EMC, then shipped to customer sites for Conserus implementation by a McKesson professional services team. The deployed solution is fully controlled by McKesson, integrated into the customers environments, and supported through a collaborative support model with commitment to SLAs. Dell EMC PowerEdge FX2 Overview PowerEdge FX2 converged hardware platform combines compute, storage, and networking into a 2U rack-based module, providing density and efficiency of blades with simplicity and cost advantage of rack-based systems. Its flexible and modular architecture allows customers to create solution configurations that provide required performance and capacity without overprovisioning resources. FX2 is: Flexible: Tailor infrastructure precisely to workloads with a variety of resource blocks. Various types of FX2 compute and storage nodes that can combine into architectures optimized for performance, density, scalability, or capacity; compute nodes can access direct attach storage (DAS) FX2 storage nodes, a SAN, or Just a Bunch of Disks (JBODs) Scalable: Grow and deploy as needed while avoiding excessive investment from over-provisioning. Compute resource can start with 1 4 compute nodes in 1 FX2 chassis and scale out to 64 nodes (using FC430) in 16 chasses in a VMware vsan cluster. Storage capacity can start with 1 2 FD332 storage nodes and grow to 32 storage nodes in the cluster Efficient: FX2 provides shared power and networking within the chassis itself. FX2 supports passthrough IO modules or IO aggregator modules. Powerful IO aggregators such as FN410 provide plug-and-play network switch layer 2 functions, simplify cable management, and enable optimized east/west server-to-server traffic within the chassis Manageable: FX2 also provides system management capabilities of Dell EMC OpenManage integrated with VMware vcenter, giving IT professionals a powerful set of tools to manage workloads and reduce complexity FX2 Architecture for Conserus The FX2 architecture for Conserus consists of four FC430 server nodes (Intel Xeon E v4 10 cores) and two FD332 storage nodes (each with 16 all-flash or hybrid drives) in one FX2 chassis. There are several configurations based on this architecture, which we detail in the Recommended Configurations section. Figure 7. FX2 within a rack Rack front view Rack rear view ToR FX2:2U required VMware vsan Overview vsan is VMware s SDS solution and a radically simple HCI solution. vsan is built from the ground up for vsphere VMs, it abstracts and aggregates locally attached disks in a vsphere cluster to create a storage solution that can be provisioned and managed from vcenter and the vsphere Web Client. vsan is delivered on the same x86 server platform running the hypervisor. Industry-standard components are used with direct-attached storage devices, from spinning disks to SSDs to latest NVMe technology. vsan delivers enterprise-class features, scale and performance, making it the ideal storage platform for VMs. vsan integrates with the entire VMware stack, including features like vmotion, HA, DRS etc. VM storage provisioning and day-to-day management of storage SLAs can be all be controlled through VM-level policies that can be set and modified on-the-fly. vsan delivers significant values over traditional IT infrastructure by enabling rapid deployment of validated technology stacks, reducing IT management complexity and costs, enhancing applications performance, and allowing scalability for future growth. vsan Architecture for Conserus The vsan architecture for Conserus consists of four VMware ESX hosts on four FC430 server nodes, with each host assigned two disk groups, a total of eight disk groups across two FD332 storage nodes. For each disk group, the vsan cache tier uses one SSD drive, and the capacity tier can use either two SSD or two hard drives. A detailed specification is provided in the Recommended Configurations section. Virtual Network Architecture for Conserus Virtual Machines PER HOST CIR Virtual Application Server: The application server performs the functions needed by the web server to display content to the end users. The application server can be deployed as a single VM to connect to the database VM, or as a group under a load balancing virtual appliance. Microsoft Windows IIS server is required as the application is written in.net. It is deployed for redundancy while the load balancing is performed across only the CIR Virtual Application servers. CEV Virtual Server: The universal image viewer CEV is deployed on one or more virtual servers. The CEV servers run on Linux and provide server-side rendering of diagnostic-grade images to web and mobile user clients. Teamed Figure 12. The virtual servers are separated out and located behind a load balancing virtual appliance to limit attack surface. OS of the virtual servers is Windows 2012 R2, except CEV servers. Below are the highlevel descriptions of key Conserus virtual servers: CIR Database Server CIR NoSQL Database Servers CIR Application Servers CEV Servers CIR Storage Platform CIR Virtual Database Server: A dedicated Oracle database server to store CIR configuration, metadata, document pointers, content status, and security information including rights, privileges, and authorization to various areas of CIR. CIR Virtual NoSQL Servers: Dedicated Couchbase NoSQL database servers to store Non-DICOM metadata, and HIPAA audit log. 9

10 Virtual Server Requirements The following tables summarize the compute and storage requirements for Conserus and Infrastructure virtual servers in our test environment. In customers production environments, the numbers of virtual servers and their requirements may differ. McKesson and Dell EMC Sales Engineers will analyze a customers requirements and provide sizing recommendations. Table 1. 11 RECOMMENDED CONFIGURATIONS To take full advantage of the modularity and scalability of Dell EMC HCI Solution for Conserus, we designed the below recommended configurations (with detailed specifications) that can be used as building blocks to create an infrastructure solution to implement and manage Conserus for every customer s unique requirement and environment. The configurations vary in terms of the number of FC430 server nodes, corresponding VMware licenses, and the number and capacity of FD332 storage nodes. Each FC430 server node has 2 Intel Xeon E v4 processors, which provides ample compute performance while allows optimization of configuration costs. 4-Node All Flash Configuration The 4-Node All Flash Configuration is the primary building block for the Dell EMC Conserus Solution. Customers can start with one or more of this type of configuration to implement Conserus based on sizing recommendations from McKesson and Dell EMC teams. The configuration provides 320 vcpus (physical to virtual ratio 4:1), 1 TB memory, and 15 TB usable capacity (RAID 1). 14 3-Node All Flash Configuration The 3-Node All Flash Configuration can be used as the initial building block for customers to implement Conserus if the required compute and storage resources can be adequately provided with this configuration. This configuration can also be used as an additional building block to an existing Conserus environment. The configuration provides 240 vcpus (physical to virtual ratio 4:1), 768GB memory, and 11.5 TB usable capacity (RAID 1). Specifications for FX2 chassis and networking IO modules are the same as in the 4-Node All Flash Configuration so that information will not be repeated. 17 SAMPLE CUSTOMER SOLUTION DESIGN This section illustrates how the above configurations can be used to create a Conserus Solution design for a sample customer. The following design is for a deployment of Conserus 3.0 with workload requirement that supports an annual volume of 1 million studies of medical images. The design leaves sufficient extra capacity for additional requirement. The storage and long-term archive of the medical images is external to this design and can be implemented on Dell EMC storage platforms such as Isilon or Unity. Sample Customer Requirement Below table illustrates the Conserus 3.0 workload requirement that supports an annual volume of 1 million studies of medical images. Sample Customer Solution Design Based on the requirement outlined in Table 14, the 3-Node All Flash Configuration (specification shown in Table 8, 9 and 10) will be selected as the initial solution design. This configuration has sufficient extra capacity to support the future release of Conserus which may require additional resources. If the customer s annual volume is expected to increase over time to beyond 2 million studies, the Add-On All Flash Configuration (specification shown in Table 11, 12, and 13) can be added to the existing 3-Node configuration. An updated workload requirement will be needed to confirm the final design. 18 TEST SUMMARY Conserus Functional Test Test Scope: Includes the majority of the test cases as specified in Conserus Image Repository and Enterprise Viewer Tests Validation Guide; test cases that require interface with external EMR and PACS systems using HL7 messages are excluded. Process & Method: 1. Establish a test environment consistng of a 4-Node All Flash Configuration (as shown in Figures 8 & 9 and specified in Table 3 through Table 6) 2. Install VMware vsphere and vsan 6.0, vcenter 6.0, create infrastructure management VMs per Table 2, configure vsan and disk groups per Figure 11 and virtual network per Figure Create CIR and CEV VMs per Table 1, create and apply storage policy (RAID 1) 4. Import OVA files for CIR and CEV VMs, configure CIR and CEV settings and network IP addresses 5. Execute the test cases in the Scope and Document results 6.
