Extension stations will be provided in connection with all classes of main station service, excluding public telephone. Extension service provides the capability of originating or receiving calls from locations equipped with instruments in addition to the location of the main station. B. Extension stations must be located on the same premises of the subscriber on which the main station is located (for definition of same premises see Explanation of Terms) and are restricted to the use of the subscriber, his representatives and associates or to members of the subscriber s immediate family or domestic establishment, except that in the case of individual line subscriber s service, extensions may be located on other premises of the subscriber or premises of someone other than the subscriber under the following conditions provided facilities are available and technical equipment limitations in each specific case permit and provided that extensions from residence service are not located on business premises. 1. Where two or more premises of the same subscriber are used in the conduct of one establishment or business; or 2. Where the extension is located on other than the subscriber s premises for the purpose of answering calls at such times as the subscriber is not available at the main station, provided that separate exchange service is also provided on these other premises. C. The provision of circuits required to connect main stations is subject to the regulations and charges shown in applicable sections of this. D. The number of extension stations which may be permitted with main station is listed below according to class of service: Class of Service Number of Extensions* Individual lines E. It is not contemplated that conversations will be held between main and extension stations. F. Separate telephone numbers or other distinctive designations are not assigned to extension stations nor is selective or code ringing permitted. Extension stations are not listed, and no additional listings without charge are allowed in connection with the main station on account of extension stations EXTENSION LINE MILEAGE A. An extension line mileage charge of $5.00 per month per quarter mile or fraction thereof, route measurement, is made for extension stations located in another building on the same or different premises of the same subscriber or in the same building off premises and also for other circuit extensions of like character. (I) NOTE: For definition of same building and same premises, see Section 1 of this Price list. B. All mileage charges are in addition to the base rate charges applicable to the particular station against which mileage charges are assessed. C. Mileage charges are computed on route measurements form the point where the extension line leaves the building in which is located the main station or private branch exchange switchboard to the place of location of the extension station or private branch exchange station. Mileage charges are computed separately for each extension line or cable pair required. *On or after by bill cycle.

When the expected life of all or part of the outside circuit extension is shorter than the normal life of the plant or the cost of providing the plant is such as to render inadequate the mileage charges quoted herein, the plant required to furnish such service will be provided on an installation and a reasonable and proper monthly carrying charge basis in lieu of mileage, or on an installation and mileage charge basis GENERAL RATES A. Touch-calling service provides for the origination of telephone calls through the use of push buttons in lieu of a rotary dial. B. Touch-calling service is for use with individual lines and PBX trunks. C. Touch-calling service requires special central-office equipment for lines and trunks and will be provided only from central offices where facilities are available. D. No non-recurring charge will be applicable to changes in telephone numbers required by the Company in those instances when a subscriber must have his number changed for the provision of touch-calling service. The following charges are to be applied in addition to the monthly rates applicable to the service with which it is associated. The regular service connection, move and change charges apply. Individual Service Monthly Rate (1) Residence Service Touch-Calling Line, per Line $-- (2) Business Service Touch-Calling Line, per Line -- Touch-Calling PBX Trunk, per Trunk CUSTOM CALLING FEATURES PROVISION OF SERVICE A. Custom Calling Features are furnished only from central offices which have been arranged to provide these services. The services are provided subject to the availability of the facilities. B. Except as provided in Section C. following, Custom Calling Features are furnished only in connection with individual line residence and business main service and are not available in connection with Meridian Digital Centrex Service, PBX, and PTAS access line or trunk. C. The Call Forwarding feature is offered for use with PBX trunk service subject to the following limitations: (1) May be provided when compatible with the equipment configuration at the customer s premises. (2) Not available with Direct Inward Dial type trunks.

(Cont d) (3) Available with non-hunting arrangements, multiline and series completion hunting arrangements only, and subject to the limitations of these hunting arrangements. D. Three-Way Calling is available on either a flat monthly rate or on a usage sensitive basis where facilities are available DESCRIPTION Custom Calling services are auxiliary features provided in addition to basic telephone service. Custom Calling services consist of the following features: A. Call Forwarding Variable This provides an arrangement for transferring incoming calls to another local service telephone number by dialing a code and the number of the service to which calls are to be transferred. In addition, calls may be transferred to a long distance message telecommunications point subject to the availability of the necessary facilities in the central office from which the calls are to be transferred. Call Forwarding shall not be used to extend calls on a planned and continuing basis to intentionally avoid the payment in whole or in part, of message toll charges that would regularly be applicable between the station originating the call and the station to which the call is transferred. B. Three-Way Calling This permits an existing call to be held, and, by dialing, a second telephone call can be established and added to the connection. This service contemplates that normal transmission performance quality cannot be guaranteed on all calls. The customer subscribing to this service will be responsible for all network usage charges both local and toll appropriate for the line with which this service is associated. Three-Way Calling is available on a flat monthly rate or on a usage sensitive basis. With the usage sensitive option and the use of the switchhook flash and an activation code, the customer will incur a per use activation charge for each occasion the feature is successfully used. The per use activation charge is applied only when a second call is completed and bridged to the first call. Completed calls include, but are not limited to, those calls terminated to telephones, voice messaging systems, answering machines, facsimile machines, modems, etc. The provision of this service on a usage sensitive basis is subject to technical limitations and is available where facilities permit. Customers may request blocking of this feature at no charge, as specified in Section H. of this Price list. C. Call Waiting By means of a tone signal a customer who is using his telephone is alerted when another caller is trying to reach that station. Permits putting first call on hold so that second call can be answered. Cancel Call Waiting provides the ability to deactivate the Call Waiting where available. In Central Offices where the capability exists and has been implemented, subscribers to Call Waiting may dial activate a Control Call Waiting feature. Before a call is initiated, the subscriber may activate the Control Call Waiting feature and Call Waiting is then made inoperative on the first call initiated by the subscriber immediately following activation of the cancel feature. The feature may also be activated after a call is established, if the customer subscribes to a service that allows flash-hook privileges such as Three-Way Calling. Call Waiting is restored automatically on termination of such a call. During the time the Control Call Waiting feature is activated, incoming callers receive a busy tone.

Call Waiting ID Enables a subscriber to view on an Analog Display Services Integration (ASDI) compatible CPE display device the calling party s name and telephone number associated with an incoming Call Waiting call, unless the name and telephone number of the calling party is suppressed, either via Per Call or Per Line Blocking. This gives customers the opportunity to identify incoming callers without interrupting their current call, which helps the customer decide whether or not to answer the new call. The customer must subscribe to a Calling Identification Delivery feature, such as Caller ID (Number Delivery) or Caller ID (Number and Name Delivery) as specified in Section 13.9 for the calling identification data of the waiting call to be provided following the Call Waiting ID alerting tone.

Speed Dialing This provides for the calling of a 7-or 10-digit telephone number by dialing an abbreviated code. The two arrangements available are an eight-number capacity (8-code) and a thirty-number capacity (30-code). F. Call Forwarding Busy Line This feature provides for calls terminating to subscriber s busy directory number to be forwarded to either a different subscriber s telephone number or a different telephone number for the same subscriber located on a different premises from the Call Forwarding Busy Line provisioned premises. The customer selected forward-to-telephone number is preprogrammed at the time service is established and can only be changed via service order. Call Forwarding Busy Line shall not be used to extend calls on a planned and continuing basis to intentionally avoid charges associated with Rotary Line Service. G. Call Forwarding Don t Answer This feature provides for calls terminating to a subscriber s idle directory number to be forwarded, after a customer preselected interval, to another telephone number. The customer selected forward-to telephone number and specified interval are preprogrammed at the time service is established and can only be changed via service order. H. Call Forwarding Busy Line Multiple Simultaneous Calls This feature provides a customer who has Call Forwarding Busy Line the option to specify the number of calls that will be forwarded simultaneously to the forward-to telephone number. The number of intraoffice Call Forwarding paths provided will be equal to the interoffice Call Forwarding paths provided and are preprogrammed via service order. Only one calling path will be provided for a single (non-rotary) exchange line/trunk. The number of calling paths provided on exchange lines/trunks equipped with Rotary (Grouping) arrangement cannot exceed the number of exchange lines/trunks so equipped. I. Call Forwarding Don t Answer Multiple Simultaneous Calls This feature provides a customer who has Call Forwarding Don t Answer the capability to specify the number of calls that will be forwarded simultaneously to the forward-to telephone number. The number of intraoffice Call Forwarding paths provided will be equal to the intraoffice Call Forwarding paths provided and are preprogrammed via service order. Only one calling path will be provided for a single (non-rotary) exchange line/trunk. The number of calling paths provided on exchange lines/trunks equipped with Rotary (Grouping) arrangements cannot exceed the number of exchange lines/trunks so equipped. J. Call Forwarding Variable Multiple Simultaneous Calls This feature provides a customer who has Call Forwarding Variable, Call Forwarding Variable Without Call Completion the capability to specify the number of calls that will be forwarded simultaneously to the forward-to telephone number. The number of intraoffice Call Forwarding paths provided will be equal to the interoffice Call Forwarding paths provided and are preprogrammed via service order. Only one calling path will be provided for a single (non-rotary) exchange line/trunk. The number of calling paths provided on exchange line/trunks equipped with Rotary (Grouping) arrangements cannot exceed the number of exchange lines/trunks so equipped. K. Hot/Warm Line a. Hot Line The line is programmed in the central office by the Company to automatically route a call to a predetermined number as soon as the telephone is placed in an off hook condition. The telephone cannot be used to make dialed calls. b. Warm-Line When the telephone is placed in an off hook condition and after a set time interval, the call is automatically routed to a predetermined number. During the time interval, the telephone can be used to make regular dialed calls if desired.

Call Transfer Allows the subscriber to transfer calls to another telephone number. The transfer is activated by flashing the switchhook followed by the transfer-to telephone number. The telephone where the call was first received is then placed on-hook, freeing the line for incoming and outgoing calls. All calls, whether originating or terminating, can be transferred. M. RingSmart Service Enables a subscriber to have up to four telephone numbers associated with a single line. Customers subscribing to this service will be able to receive calls dialed to two, three, or four separate telephone numbers without having a second, third, or fourth line. A distinctive ringing pattern will be provided for each of the additional telephone numbers to facilitate identification of incoming calls. A distinctive Call Waiting tone for each additional telephone number will be provided, where facilities permit, to customers subscribing to Call Waiting service. Regulations & Limitations: 1. This service is available to individual line residence and business customers. 2. The service is not compatible with Meridian Digital Centrex Service, PBX trunk service, Advantage Communication Service, Company or customer provided public telephone service, lines equipped with hunting arrangements, foreign exchange service, or with access lines terminating in customer premises switching or key equipment. RingSmart Service may not be compatible with all types of customer provided telephone equipment. 3. RingSmart Service is provided subject to the availability of facilities. 4. RingSmart Service subscribers will be entitled to one listing with each RingSmart Service number. Listings for RingSmart Service are subject to regulations specified in Section 6 for directory listings. Other listings will also be provided under the terms and conditions described in Section 6 of this Price list. 5. All telephone numbers associated with a line equipped with RingSmart Service must originate from the same central office switching machine. 6. When establishing RingSmart Service, Call Forwarding service subscribers must choose one of the following options: a. All telephone numbers associated with one line will be forwarded to a single number when Call Forwarding service is activated, or, b. The main telephone number only will be forwarded when Call Forwarding service is activated. The additional RingSmart service numbers will continue to ring and may be answered at the subscriber s premises. 7. A Secondary Service Charge, as specified in Section 4 of this Price list, will apply when changing from one option to the other subsequent to the establishment of RingSmart Service. N. Call Forwarding Remote Access This feature provides a customer the capability to activate and deactivate the Call Forwarding Variable service and change the forwarded-to number remotely from any line/equipment capable of Touch-Tone signaling rather than only from the base station line. This feature does not require that a courtesy call be completed to the forward-to-telephone number. This feature requires that you order Call Forwarding Variable service separately.

13 COMPANY Original Sheet 9 High Point, North Carolina Effective: December 31, TIE LINE MILEAGE GENERAL A. Tie lines are circuits connecting subscriber s private branch exchange switchboards. Tie lines are intended as means of communications, between stations directly connected with and in the immediate vicinity of the switchboards in which the tie lines terminate. The Telephone Company does not undertake to assure commercial transmission where tie lines are used for communications between stations located at a distance from the switchboard or where connections involve the use of trunking circuits between central offices. B. For tie lines connecting switchboards contracted for by the same or different subscribers and located in the same exchange area, a charge of 61 cents per month per quarter-mile or fraction thereof is made. The minimum charge for each tie line is $2.43 per month. C. Mileage charges are computed on route measurements between switchboards and separately for each tie line. D. Tie lines are not furnished to connect a flat rate private branch exchange switchboard with a message rate private branch exchange switchboard. E. Customized Code Restrictions (CCR) will provide a choice of code restriction options for selected outgoing calls. Options 1-5 will permit local calls except 976 local calls. Non-chargeable calls to Company numbers such as repair service and emergency numbers (911) are permitted for each option. (C) B. Customized Code Restrictions will be available to exchange customers with Individual Line Residence Service, Business Service, PTAS Service, PBX trunk service, and Key trunk service. C. Customized Code Restrictions are furnished only from central offices which have been arranged to provide these services. The services are provided subject to the availability of facilities. D. The Company shall not be liable to any person for damages of any nature or kind arising out of, resulting from, or in connection with the provision of Customized Code Restrictions offered herein, including, without limitation the inability of the station user to access the operator for any purpose and any of the other restricted codes specified in the dialing plan options listed hereunder. E. Subscribers dialing restricted codes in the CCR Dialing Plan will be sent an appropriate recorded announcement. F. It shall be the responsibility of the subscriber to notify all authorized users of this service that it is impossible to reach the operator using all restricted telephones. G. Remote Call Forwarding (RCF) is a service whereby a call placed from a station (the originating station) to a customer s (the RCF customer) telephone number in one exchange (the call forwarding location) is automatically forwarded by Telephone Company central office equipment to another station designated by the RCF customer (the terminating station) LIMITATIONS A. RCF service is offered subject to availability of suitable facilities. B. RCF service is not offered when the terminating station is a coin telephone. C. The Telephone Company will not provide identification of the originating telephone number to the remote call forwarding customer. D. RCF is provided on the condition that the customer subscribe to sufficient RCF features and facilities to adequately handle calls to the RCF customer without interfering with or impairing any services offered by the Company. If, in the opinion of the Company, additional RCF features at the call forwarding location or facilities at the terminating station line are needed, the customer will be required to subscribe to such additional RCF features and facilities. With the exception of Calling Number Delivery Blocking, these features cannot be offered on Toll Terminals or in association with PTAS service. Due to call blocking, the Caller ID feature is not suitable for the provision of 911 or E-911 and is, therefore, not available to 911 or E-911 providers. E. Call Return and Repeat Dialing are available on either a flat monthly rate or on a usage sensitive basis. The remaining Star*Plus Service features are available on a flat monthly rate basis only. Call Return allows a customer to automatically return the last incoming call whether it is answered or not. If the called line is found to be busy, a 30-minute queuing process begins. The customer is given an indication that the network will attempt to set up the call when the called line is idle. The network periodically tests the busy/idle status of both parties until both lines are found idle, the customer cancels the request, or the queuing process expires. If both lines are found to be idle, the calling party is alerted with a distinctive ring signifying that the call may now be completed. Where facilities permit, upon activation of the Call Return feature, the customer will receive a voice announcement stating that Call Return has been accessed. In addition, the announcement will provide the Directory Number of the last incoming call, unless the telephone number of the last incoming call was suppressed either via per call or per line blocking, or unless the last call was from outside the serving area. If the call was from a number within the serving area, the Call Return user will be prompted to enter an additional digit to continue with the feature activation, or to hang up to abort the activation. The Call Return feature cannot be used to return some operator-assisted or credit card calls. An announcement of the number of the last incoming call associated with all other type calls will be provided, subject to technical limitations and certain exceptions as stated below (including telephone numbers associated with Non-Published and Non-Listed Number Service described in Section 6.4 of this Price list).

Call Selector allows a customer to program up to twelve (12) telephone numbers of selected callers enabling the customer to distinguish certain incoming calls from all others by a distinctive ring tone. If a customer also subscribes to Call Waiting, calls from the selected numbers will be distinguished by a special Call Waiting tone. D. Preferred Call Forwarding is an arrangement which permits a customer to specify telephone numbers (maximum of 12) from which incoming calls are to be forwarded. During the period that Special Call Forwarding is activated, only calls from the specified number will be forwarded. E. Selective Call Rejection allows a customer to block incoming calls from a maximum of 12 telephone numbers. The customer also can block incoming calls placed from the last calling number even if the number is unknown to the customer. Blocked calls are routed to a recorded announcement which specifies that the called party is not accepting calls. F. Selective Call Acceptance allows a customer to select up to 12 customer telephone numbers from which calls are to be received. All other calls are intercepted and routed to a recorded announcement that informs the caller the customer is not accepting calls. Calls from numbers outside the specified exchanges will ring normally. G. Anonymous Call Rejection (ACR) an arrangement that allows subscribers to reject receipt of calls from callers who utilize Caller ID Blocking. The calling party who has chosen to block delivery of his name and number will hear a recorded announcement stating that the called party will not accept anonymous calls. The announcement will also state that the caller should hang up and call back with caller identification unblocked to complete the call. Anonymous Call Rejection subscribers may deactivate the feature by dialing the deactivation code (*87 or 1187) and may receive calls from callers utilizing Caller ID Blocking. Subscribers must dial the reactivation code (*77 or 1177) to reactivate Anonymous Call Rejection. Confirmation announcements will confirm activation and deactivation of the feature. Local, expanded local, and toll calls routed to the Anonymous Call Rejection recorded announcement are not considered complete calls and will not be billed by the Company. Anonymous Call Rejection is provided at no additional charge to Caller ID (Number Delivery and Name and Number Delivery) subscribers, subject to the availability of facilities. The feature is available to non-caller ID subscribers for a flat monthly rate. H. Caller ID an arrangement that allows the subscriber to view the telephone number and name of an incoming call before answering. If the calling party has designated their telephone number and name as private, or if the incoming call is handled by an operator or originates from outside a Caller ID equipped calling area, the calling number and name will not be displayed. Customers will have the option of choosing between two Caller ID packages telephone number delivery only or telephone number and name delivery. The Caller ID display feature requires a telephone set, an adjunct unit, or key/pbx system capable of recognizing and displaying the calling telephone number and name (dependent upon which Caller ID package is selected) sent from the central office.

Caller ID is not available on operator handled calls. If the incoming call is from a caller who subscribes to RingSmart service, the number and name will always be those associated with the main number rather than the RingSmart numbers. Where facilities are available, Anonymous Call Rejection is provided with Caller ID at no additional charge. I. Caller ID Blocking Per call Blocking will be automatically provided to single-line residential, single-line business, Centrex, Key, PBX and Public, Semipublic, and PTAS Payphone customers. When a customer utilizing Per Call Blocking calls an Anonymous Call Rejection subscriber, the call will not be completed. The calling party will receive a recorded announcement stating that the called party will not accept anonymous calls and instructing the caller to hang up and call back with caller identification unblocked. Per Line Blocking, when requested, will be provided to single-line residential, single-line business, Centrex, Key, PBX, and PTAS customers.
