2 This Construction Management Plan has been prepared by Valenti Builders and outlines the construction procedures that will be used during the North Shore Residence Addition project located at 1619 Chicago Avenue in Evanston, Illinois. Page 5 Staging and Logistics The attached Exhibit A is the construction site logistics/staging plan that will be used during the project. An eight foot fence with mesh screening will be installed around the perimeter of the project site to protect the public from the construction work. Jersey barriers will be installed at Chicago Avenue and at the alley to separate traffic from the construction staging area and site. Construction staging areas will be maintained within the project barricades. The field office for the site will be located in the parking lot north east of the project site per the attached Exhibit A. Pedestrian traffic along the East side of Chicago Avenue will be redirected to the closed traffic lane on Chicago Avenue adjacent to the enclosed truck loading/unloading area. Pedestrians will be protected from traffic on Chicago Avenue by jersey barriers on the west side of the temporary pedestrian walkway and by the 8 foot screened construction fence on the east side. Signage directing pedestrians to the relocated walkway will be in place and maintained throughout construction. Traffic control flagmen will be provided on the street when trucks are entering or leaving the project site for the safety of pedestrians. Five Chicago Avenue curb lane parking meters will be removed during the construction period. Scheduled closures for portions of Chicago Ave and the Alley will be required for the connection of underground utilities. This work will be scheduled with the City and at least one drive lane at Chicago Ave. will be maintained during the work. The 20 wide North-South public alley east of the project site will be available for access to the construction site for immediate just in time deliveries and access to the jobsite. All vehicles will be required to stage in the parking lot north of the site and not block alley access for residence and deliveries to adjacent businesses. 3 Project Schedule Project Milestone dates for the project: Earth Retention/Caissons/Foundation October 2014 February 2015 Building Structure March 2015 June 2015 Façade/Windows/Roof May 2015 August 2015 Interiors August 2015 December 2015 Construction Work Hours Work hours for the project will be per the City of Evanston requirements: Monday - Friday: 7am - 7pm Saturday: 8am - 5pm Sunday: Work not allowed unless special permission is granted from the City. Contractor Parking There will be no onsite parking for contractors. Contractors will be required to park in one of the Public Parking garages in the surrounding area the closest of which is at the corner of Church Street and Chicago Ave. (shown as lot #5 on the attached public parking map Exhibit B). Subcontractors will not be allowed to park on residential city streets. Any contractors who park on residential city streets will be removed from the jobsite. Parking restrictions will be incorporated into all subcontract agreements and communicated weekly to all on site personnel. Delivery Routes and Staging We have reviewed the various streets entering and exiting Evanston and have developed a site access plan that is in strict accordance with the City of Evanston Truck Routes as published on the City of Evanston website as well as attached to this plan as Exhibit C. The following traffic controls will be in place during the project: Traffic entering the site will only use the approved truck routes from the North, South and West to access Chicago Avenue and enter the site keeping in mind the posted height restrictions as noted in Exhibit C. Traffic exiting the site will only exit on Chicago Avenue to the approved truck routes per Exhibit C that will take them to the North, South or West to exit the City of Evanston. Contractors and Subcontractors will be directed to use just in time deliveries. Staging of trucks on local residential streets will not be permitted. Any Contractor or Subcontractor whose personnel do not adhere to the foregoing contractual terms will be required to utilize other drivers for their subsequent deliveries. 4 Alley Work New underground conduit will be installed under the alley east of the project site so ComEd and AT&T can remove overhead facilities and 2 poles adjacent to the project site. This underground conduit installation work will be completed in phases to minimize the impact to traffic in the alley. The attached Exhibit D shows the 3 sections of work and schedule for each section as well as the notification to the neighbors of this work. Alternate parking at the parking garage at the corner of Church Street and Chicago Ave. will be provided for residents at 1616 Hinman and the First United Methodist Church who are unable to access parking lots while the work is being completed. A storm/sanitary line, gas service and AT&T services will be installed under the alley. Future alley closure dates will be coordinated with the City and proper neighbor notifications will be provided for this work. Worker safety will be in accordance with OSHA requirements and as required by the Contractor s safety program. Vibration Monitoring and Surveys Vibration Monitoring Prior to commencement of construction activities, seismographs to monitor vibrations will be installed/implemented to monitor the ground surrounding the project site to ensure that proper construction procedures are employed to mitigate vibration transmission to, settlement of, and damage to nearby utilities/structures/ground/etc. Seismographs will be used and installed in accordance with manufacturers and consultant s recommendations. We anticipate [3] units will be installed. One unit located in the basement of the south existing structure, one unit located on the North existing structure, and one unit located east of the project site. A diagram will be prepared indicating the locations of the [3] units prior to installation. All units will be confirmed in the on status prior to commencement of work. Monitoring shall be continuous during the all deep foundation and vibration producing operations. Our consultants will define and communicate the acceptable range of measurement results and will work with Valenti to implement a mitigation plan immediately if levels are greater than acceptable levels. Written documentation of the results from the monitoring equipment will be available 7 days after completion of the subgrade construction operations for review and documentation. 5 Building/Foundation Survey Prior to commencement of construction activities, a building and foundation survey (with Photos) will be performed to document the existing conditions at the following properties: 1600/1608 Hinman 1616 Hinman 1611 Chicago Ave Chicago Ave. Settlement Monitoring Survey points will be placed on site for monitoring and documentation as follows: 1. Settlement/Lateral survey points on all surface utility structures within 40 of site property lines on Chicago Ave and in the Public Alley 2. Settlement/lateral survey points on 20 foot intervals along east curb line of Chicago Ave; 3. Settlement/lateral survey points on 20 foot intervals at the center line of the public alley east of the project site. 4. Settlement/lateral survey points on existing buildings north and south of the project site. Work Site Communication with Residences The project team will update the property owners or management companies of buildings on the same block as the project and are located to the west of the project on Chicago Avenue from Davis Street to Church Street via newsletter and project website monthly beginning no later than November of 2014 and continuing through issuance of the certificate of occupancy. Newsletters will be delivered to referenced properties by US Mail, , and/or other means supported by property owners or their management companies. A website will also be set up and maintained for weekly updates chicagoavenue.blogspot.com. All public way closure notifications will be provided (1) week prior to commencement. Emergency phone numbers of contractor s key employees will be posted on the construction gates and on the project website chicagoavenue.blogspot.com.
