11 12 CRM in action Risk: Work Area: MEM Workshop, Pilbara Scenario Description: First Field Verification Coaching session with Supervisor on a Haul Truck Rebuild in MEM Workshop CRM Question: Is installed edge protection adequate to prevent falls? Note: Includes railings, horizontal life lines, rat lines etc. What they found: What they found: Inadequate edge protection while working at heights. What was in place: There was a site approved ladder in place for module removal however this was not suitable to gain access to maintainers work area. 4 meter fall from height Action: Job was stopped and maintainer guided to step down into a secure area. Installed scaffolding to the exposed area and work recommenced.

15 16 Contractor Management 1. Scope of Work/ Risk Matrix Complete a comprehensive Scope Of Work, taking into consideration the results of the Risk Matrix. 2. Planning and Qualification Choose the appropriate category of Contractor and complete the necessary prequalification process based on the category 3. Contract preparation and tender Considering contractors HSEC Plan and past safety record. 4. Evaluation and award Consider Contractors Safety plan and ability to carry out the work. 5. Mobilisation Before the contractor comes to site carry out a pre-mobilisation meeting. Ensure that contractors have appropriate qualifications and inductions. 6. Monitor progress and audit Carry out pre-start meetings. Carry out HSE monitoring. Conduct safety audits. Ensure continuous communication with Contractors. 7. Review and close out Conduct a Contractor Safety performance review.
