(Note: tied to C ). Process: Order; Jurisdiction: West; System: Primary Area: All; LSOG Version: All Initiator/Date: Joyce Perry, AT&T, 07/22/03 3 C Title: T5 Outside Moves # Description: Use elements of the migration process to facilitate outside moves. The CLEC would first perform loop qualification to assure the service is viable to the new address and then issue a change order to move the service to the new address. Process: Order, Provisioning; Jurisdiction: West; System: WISE GUI, EDI; Primary Area: All; LSOG Version: All Initiator/Date: Michael Clancy, Covad, 10/04/02 CTSI = 5 ChoiceOne = 5 MCI = 5 VarTec = 5 Overall = 4.9 MCI = 5 ChoiceO = 4.5 CTSI = 5 VarTec = 5 Overall = 4.8 SBCT = P Allegiance = P Eschalon = P VarTec = 5 09/12/03 CR escalated into Feb /05/03 Business Application Design (BAD) session held and Business Requirements Document (BRD) completed. 08/19/03 Scheduled as a candidate for the June /15/03 New to List. STATUS: PENDING 08/19/03 Verizon will re-contact Bell South for further clarification. 07/29/03 Conference Call with Bell South concluded that Bell South has the ability to have 2 pending orders at the same time, currently using the RPON field on the LSR to force a specific code in, causing the order to drop out to a manual process to be worked by a rep. The order comes in the door electronically, but drops out to a manual process if that specific code has been populated in the field. However, the end user has to alert Bell South and the interval must be 2 days or more. Verizon will revisit the feasibility of implementing this request. 07/01/03 Request has been sent to Bell South to discuss manual processes they are currently supporting. 06/17/03 Verizon restated their position that, in order to accept a move order, we have to have established service at that address. CLECs pointed out that Bell South can do this now and they can also take an order on a pending order. This is manual at this time but will be automated in 3 rd Qtr. 03/18/03 Initiator has asked for a more detailed explanation of what type of activity this includes. Page 1 of 18

2 03/10/03 CR has been denied. The CLEC can perform this activity themselves by using a new CFA, and coordinate that cut over date themselves. Discussion has been held with initiator regarding this same issue in the East, and the same answer would hold true for the West. CR on hold pending initiator s re-evaluation. 12/17/02 Put on hold per initiator pending further review. CR moved from Oct 03 to. 11/19/02 Internal discussion taking place. 10/15/02 New to List. Allegiance has requested the following specific work order status indicators to be developed for: 1) An indicator on the jeopardy notification that states there will be a work order issued to provide additional facilities and an estimated date of completion. 2) That there is not a planned work order to relieve the no facility condition. 7/15/03 It was noted that the 65 tabled entries for the jeopardy remarks field going in with C were not sufficient for satisfying this CR. Initiator wants to see the remarks on the form. CR was, therefore, rated during the July Meeting. 6/17/03 CR should be satisfied with existing CR C Before withdrawing this CR, initiator would like to first determine whether or not the 65 tabled entries for the jeopardy remarks field will be sufficient. Challenge underway by Covad. 10/15/02 All Bulk Loop Qual Extract CRs scheduled for implementation Nov 26 are under review. A complication has been raised and so the implementation of these CRs is on hold until the questions can be resolved. All Service identifiers are on-hold pending development of generic notifiers. 09/17/02 CR has been removed from the November 26 implementation. John Rudden spoke to the possible revision or cancellation of this CR due to being challenged by Legal for Carrier Proprietary Rules. The issue deals with the level of specificity. John will discuss with Legal a modification of this CR to make the indicators generic. Change Mgmt will report back to the CLECs. 06/10/02 - CR will be implemented with a Flash Cut. All regions will go the 2 nd week in November. 06/10/02 CR has been escalated for implementation in the Oct /21/02 - Included in the discussion of Bulk Loop CRs for a Phased Approach beginning in Oct, or All at Once in February. 05/08/02 During an interim CMP Conference Call, it was agreed to modify wording of the CR to remove ISDN in request, since ADVSVC Service Type Code indicates ISDN. With ISDN removed, CR will be considered as a candidate for the Oct 02 release. 03/19/02 Conf Call to be set up with VADI to walk thru the report. 03/06/02 CR has been rescheduled as candidate for Feb 03 release. 03/05/02 Per Network Operations: this info is already contained in the Page 13 of 18

15 Not in DB Title: No Facilities Jeopardy Update Description: The No Facility Process requires the CLEC to resubmit an order within 30 days if the order is Jep d back with no facilities. Allegiance would like to see a more proactive response from Verizon, telling when the relief date will be for facilities if known, and provide updates if the date for relief is extended beyond the original date, or if facilities become available earlier. Process: Order; Jurisdiction: West; System: WISE, EDI, CORBA; Primary Area: All; LSOG Version: All Initiator/Date: Bob Buerrosse, Allegiance Telecom, 08/09/01 Overall = 3.1 AT&T = 1 WorldCom = 3 SBCT = 1 Sprint = 4 VADI = 4.5 Eschalon = 3 BTI = P ICG = P STATUS: HOLD 08/19/03 Initiator stated that this CR could be closed if there were two conditions met in C : 1) If there is an indicator on the jeopardy notification that states there will be/has been a work order issued to provide additional facilities and an estimated date of completion. 2) If there is not a planned work order to relieve the no facility condition, that this information be stated on the jeopardy notification. 06/17/03 Initiator stated they understand VZ will not build facilities in order to accommodate a CLEC order. In some cases in the past, however, agents have been able to inform the CLEC of pending facilities. This CR was related to the recent CR introduced by Covad, C Jeopardy Remarks, which should be satisfied with existing CR C Allegiance will be meeting with Tom McGuire to pursue this issue. 01/21/03 Will be included in Conference Call between Process Group and Allegiance to discuss CR C /17/02 Referred to Process Group 11/19/02 It was noted that this issued was not discussed during the Operations Meeting, and is still an issue with Allegiance. CLECs have asked for clarification of No Build Policy and requested that it be placed on the web-site. 09/17/02 Added to agenda for the Operations Meeting to be held in October. 04/16/02 Allegiance working with VZ Account Team to satisfy their request with a different approach. 03/19/02 Bob Buerrosse reported that Allegiance will be initiating escalation process. 11/15/01 No change in status. 10/16/01 Rated by CLECs. VZW s consistent policy has been to not provide this info to CLECs. VZ not required to build facilities to accommodate CLEC svc requests and providing planning info would be considered proprietary. IP Support does not support changing the current process, since OSP will not pursue or launch wo s or provide follow-ups and updates on facility relief. VZ does not provide relief for NO FAC conditions for UNE Wholesale activity. Note: VZE does not provide the requested info or follow-up. 08/21/01 Will not be rated until Sept Mtg Page 15 of 18

16 C # Title: T5 ANI Test at DD-2 (Due Date Minus 2) Description: Allegiance is requesting that Verizon perform an Automatic Number Identification (ANI) test at DD-2 on the CLEC collocation pin & lug, to ensure the dial tone and correct telephone numbers are received on the Verizon side of the co-location. If there is no dial tone or the ANI doesn't match, VZW will notify the CLEC of jeopardy. The jeopardy notification will be an electronic notification based on the method used to send the LSR. Process: Order; Jurisdiction: West; System: WISE, EDI, CORBA; Primary Area: All; LSOG Version: All Initiator/Date: Bob Buerrosse, Allegiance Telecom, 07/05/01 Overall = 3.0 AT&T = 2 WorldCom = 3 SBCT = 2 Sprint = 3 VADI = 3 Time Wrnr = P Cox = 3 Eschalon = 3 STATUS: HOLD 08/19/03 Meeting has been rescheduled. 06/17/03 Allegiance has a meeting next month with Tom McGuire and the matter will be pursued with him. 12/17/02 Referred to Process Group. 10/16/02 Status provided at the Operations Mtg was VZ West was seriously considering ANI Test at DD-2. No commitment. The ANI Test is an East process. 09/17/02 Added to agenda for October Operations Meeting. 08/08/02 Deployment of the process cannot be confirmed in the West at this time. 06/24/02 Scheduled as candidate for Jun /08/02 During Interim CMP CC: VZ stated that the ANI Test at DD-2 does not replace the CLEC s obligation to maintain accurate connecting facility assignments and to provision their dial tone services w/ the due date in mind. VZ understands the CLEC request to expand the DD-2 ANI test process across the footprint, however VZ cannot commit to DD-2 ANI deployment in the West at this time. The DD-2 process is another example of a successful test of a provisioning process being gradually expanded across territories served by like operational systems. Because VZ E&W do not share common operational support systems, expansion of the process will require extensive procedural, system enhancement and possibly creation of new systems. VZ is currently planning and evaluating options. Additional information should be available in 4Q02. AT&T and Allegiance noted the above status as unacceptable. Escalation of the issues will continue. Allegiance wanted access to internal system used by NMC to obtain status. AT&T wants EDI notification of status, not look up of status as currently performed. 03/19/02 Bob Buerrosse reported that Allegiance has initiated escalation process. 11/15/01 No change in status. 10/16/01 There is currently no procedure for the CO to know 2 days in advance what is due, nor is there a procedure to capture the CO time required to perform this function. A procedure would be required between the NCPC and the CO to provide this requirement on a source document. This additional step would require a new procedure and significant additional resources in both the NCPC and the CO. Page 16 of 18
