2 Since 2016, the data collection of health care expenditure and financing statistics is mandatory for all EU MSs and EEA countries under the Commission Regulation 2015/359 on healthcare expenditure and financing statistics. This document outlines the practical arrangements of the data collection, validation by the three international organisations (Eurostat, OECD and WHO) and data publication. The primary goal of the collaboration between OECD, Eurostat and WHO is to reduce the burden of data collection for the national authorities responsible for the provision of statistical information to the international organisations. Therefore, the same means (i.e. templates for data and metadata, infrastructure for the submission of the data/metadata) will be used for the collection of the Commission Regulation variables and for conducting the JHAQ 2018 data collection exercise. 1. DATA TO BE SUBMITTED Article 5(2) of the Commission Regulation 2015/359 stipulates the set of data that have to be transmitted. This set of data, as specified in Annex II to the Commission Regulation, is a subset of the variables of the ICHA classifications as provided in the SHA 2011 manual. Therefore, even if the focus is on the Commission Regulation variables, countries are strongly invited to continue reporting data at the most detailed level possible, completing all variables and tables of the annual Joint Health Account Questionnaire (JHAQ). Preliminary data for t-1 for a more limited set of spending aggregates, as well as other policy relevant additional items and tables, such as HCR.1 Long-term (social) and total Pharmaceutical Expenditure, as well as the breakdowns of HFxFS, HKxHP, HPxFP, are part of the JHAQ. 2. THE JOINT HEALTH ACCOUNTS QUESTIONNAIRE The 2018 JHAQ consists of three elements: SHA data tables; methodological questionnaire (questions and tables requesting metadata), and explanatory notes/guidelines, including the description of the practical working arrangements of the Joint data collection. Template for submitting the data Within the framework for a joint health accounts data collection, Eurostat, OECD and WHO agreed on an annual Joint Health Account Questionnaire (JHAQ) for their respective Member States, which has been in use with updates and adjustments since December The SHA 2011 JHAQ will be the template used for filling in data and submitting them to Eurostat. An empty default file will be sent to countries for filling in their data for the reference year

3 This empty default file should also be used for revised SHA data tables and for the preliminary t-1 aggregate estimates, if applicable. The automation of the validation and dissemination process will be facilitated by the use of standard format and procedures. Countries are therefore asked to comply with the following requirements: Use the latest version of the JHAQ template for the submission of all years (data for 2016, preliminary aggregates for 2017 and revisions for previous years) The format of the Questionnaire is not to be modified by countries in any way (i.e. no rows/columns are either deleted or added, no change in the names of the excel worksheets ) All filled data are in numerical format (e.g. the use of dashes, white spaces or the change from Number to Text format will result in an error) All data and metadata are to be submitted through edamis with the correct Dataset Identifier as mentioned under point 3 All revisions of data and metadata are also to submitted through edamis Template for submitting the metadata The requirements for the metadata to be submitted are described in Article 5(2) of the Commission Regulation. Member States shall provide the necessary reference metadata, where any deviation from standard definitions, information on data sources and their coverage and compilation methods used should be explained. In 2018 the countries are encouraged to fill in the metadata template with detailed information on definitions, data sources and methods used for the compilation of the 2018 JHAQ. A single metadata file should cover all submitted data under SHA2011, contain information on all years and also be updated during the validation process. Calendar The 2018 JHAQ will be sent to designated focal points in the countries concerned by 12 January Article 4(2) of the Commission Regulation stipulates that the data and reference metadata for the reference year 2016 shall be transmitted by countries on 30 April However, as in the previous years, countries are encouraged to submit data by the deadline of the end of March. 3. ELECTRONIC SUBMISSION OF DATA AND METADATA EDAMIS Article 4(2) of the Commission Regulation stipulates that: "Data and reference metadata shall be provided to the Commission (Eurostat) using the single entry point services or should be made available for retrieval by the Commission (Eurostat) by electronic means on an annual basis." 3

4 Therefore, EU Members States and EEA countries shall submit the completed questionnaire to EUROSTAT Single Entry Point, via the edamis transmission tool. Any submission that is not realised through Eurostat's IT infrastructure will not be treated as an official submission by the countries. It has to be noted that edamis is programmed to automatically forward the submitted data to OECD and WHO. Guidelines on the correct use of edamis will be distributed together with the Joint Questionnaire. To enable a smooth validation and publication process of submitted files, countries are asked to enter correct edamis Dataset Identifiers for the SHA 2011 Joint Questionnaire: HCSHA_2011NAT_A (for the data) HCSHA_2011MET_A (for methodological information) The year to be selected in edamis should be the reference year of the data submitted or metadata. EU Members States and EEA countries shall use edamis also for submitting revised data. 4. VALIDATION AND OTHER CLARIFICATIONS Since 2006, the International Health Accounts Team, set up by OECD, WHO and Eurostat, has continued to develop a methodology and a co-ordinated way of correspondence with national focal points in order to check the data submissions of the participating countries. Data validation will be carried out in a similar way for the 2018 exercise. The aim is to finalise the data validation process within two months after data submission. To meet such a target requires a suitable commitment of resources from both the three organisations and the national reporting authorities. For 2018, because of the increased policy relevance and the importance in overall health spending estimates, it is proposed to work to improve information on: Expenditure on household out-of-pocket payment (HF3) The objective is to further improve international health spending estimates for out-of-pocket payment (OOP). To this end, data submitted during the 2018 JHAQ will be analysed with the information provided in the metadata file which should clearly detail the administrative and statistical data sources and the methods of estimation of this item (see item 2 on the agenda of the Technical Group Health Care expenditure statistics on 23 November 2017). 4

5 Long-term care (HC3 and HCR1) Particular attention will be given during the validation process to the quality of data provided for Long-term care (health) (HC3) and Longterm care (social) (HCR1). In this respect, information in the metadata file will be used. If as a result of the validation, countries need to submit a revised version of the joint questionnaire and/or metadata file, they should be transmitted via edamis. All correspondence concerning the SHA data collection should be addressed to all three organisations. 5. DISSEMINATION OF RESULTS Data dissemination is subject to completion of data validation. Therefore, data will be uploaded on Eurostat's online database (Eurobase) as soon as they are completely validated. In 2018, metadata will be systematically published at the time of the publication of the data. The Working Group is hereby invited to take note and give their opinion on the practical arrangements of the next 2018 Joint Health Accounts data collection. 5
