The following items are issued to add to, modify, and clarify the Contract Documents. These items shall have full force and effect as the Contract Documents, and the cost involved shall be included in the bid prices. Acknowledge receipt of the addendum by inserting its number and date on the Bid Form of the Bid Documents. Failure to do so may subject the bidder to disqualifications. This Addendum No. 1 consists of 5 pages. Page 1 of 2

2 GENERAL 1. The Engineer s Opinion of Probable Cost of is $1,275, The pre-bid conference was held on January 6, 2016 at 10:00. The sign-in sheet (1 page) is attached and hereby made part of this addendum. 3. The Plan Holders list (2 pages) as of January 13, 2016 is attached and hereby made part of this addendum. 4. As clarification, Liquidated Damages shall be determined per Table (Calendar Day), Page 79 of the Louisiana Standard Specifications for Roads and Bridges, 2006 Edition, as amended (Blue Book). CONTRACTOR QUESTIONS Q-1) A-1) There is one conflict box shown on the plans and there are two conflict box details in the plans. One details shows a split casing and one just show ductile iron. Which is correct? Use the detail for ductile iron pipe only. The split casing will not be used. Q-2) A-2) Special Provision SP.48 Site Survey indicates Prior to mobilization, the Contractor shall individually photograph/video the interior and video the exterior of all buildings and structures within 300 feet of the project limits 300 feet would include the houses on the next streets to the north and south (William Mary & Miami). Is that the intent? No, only photograph/video the houses on Millsaps Place. Q-3) A-3) Will the Contractor be allowed to alter the Phasing Plan, i.e. reduce the phasing from thirds to less than thirds (half or whole)? No, the project must be completed per the Phasing Plan as described on Sheet 2 of the plans. Q-4) A-4) The Specification for Bedding Material states Bedding Material (#57 Limestone) shall be compacted to 95% maximum dry density, according to TR 418, Method E What is the Relative Density? The Contractor shall achieve Relative Density by mechanical methods in lifts not exceeding 8 inches. END OF ADDENDUM Page 2 of 2
