4 Online Reports Overview Available Online Reports Report Type M = Management Report Reports that help you manage & monitor your every day work process S = Summary Report Reports that aggregate data and provide high level view with NSQIP comparison L = List Report Reports that provide site specific case details in table like format D = Data Report Reports that generate data files for manipulation. Results are not displayed in web browser. G = Graphical Report Reports that produce a graphical representation of data

6 Available Online Reports Workflow Reports 30 Day Patient Letter o Generates 30 Day Follow Up Letters and Mailing Labels o Has customizable letter templates for English and Spanish Based on preferred language in Demographics Request for Medical Records o Produces a list of cases that can be used when requesting medical records for case review. o Usually generated for specific 8 day cycle 30 Day Call List o Creates a list of cases for patients who haven t been contacted o When the patient contact information is updated, the case no longer appears on this list

8 Available Online Reports Site Level Reports Condition Versus Non-Condition Report o Summary report comparing patients with a selected condition vs. those without the condition (ex. smoker vs. non smoker) o This report also allows you to select categories for condition such as Any Wound Occurrence for Post operative Occurrences condition or Any Renal for Pre operative Risk Factors condition. Pre-Operative Risk Factor Summary o Summary report providing overview of pre operative risk factors 30 Day Post-Operative Occurrences* o Summary report providing overview for occurrences o Takes into consideration pre existing conditions (PATOS) o A few occurrences which combine intra operative occurrences with post operative occurrences. (i.e. Myocardial Infarction) o Useful report to identify potential issues related to morbidity

10 Available Online Reports Site Level Reports Select Post-Operative Occurrences Summary* o Summary report that compare patients with an occurrence to those who do not the occurrence. o Can be used to investigate potential problematic occurrences. Select Post-Operative Occurrences - Patient List* o List report of cases that had selected occurrence o Provides way to dig deeper into details of cases with occurrence. o The Case Number column in this report allows you to click on any of the case numbers listed and open that case assessment. Select Post-Operative Occurrences - CPT Occurrence Rate* o List report of cases that had selected occurrence by CPT o Provides total # of cases with occurrence and total # of cases that had that CPT code. o Can be used see what CPT groups are associated with occurrence.

11 Available Online Reports Site Level Reports 30 Day Post-Operative Occurrences Stratified* o Summary report giving overview of occurrence stratified by Inpatient vs. Outpatient patient populations o For Inpatient, further distribution of whether the post operative occurrence happened in hospital or post discharge Mortality Summary* o Summary report for cases with a death within 30 days of the date of operation vs. those alive 30 days post surgery. o Includes intra operative Death During Operation, as well as, cases where the Date of Death was marked as Unknown and Postoperative Death within 30 Days of Procedure as Yes on the DISCHARGE panel. Mortality - Patient List* o List of patients with a death within 30 days of the date of operation o Ability to drill down on case number to open case assessment

13 Available Online Reports Site Level Reports Wound Occurrences Stratified by Wound Class - Summary o Summary report providing overview wound occurrences distributed by wound classification o Clean, Clean/Contaminated, Contaminated, Dirty/Infected Wound Occurrences Stratified by Wound Class - Patient List o List report reflecting cases with specified wound class and wound occurrence. Wound Occurrences Stratified by Wound Class - CPT Occurrence Rate o List report reflecting cases with specified wound occurrence and wound class grouped by CPT.

14 Available Online Reports Site Level Reports Patient Variable Statistics Report o Summary report that compares patient variables recent timeframe to same timeframe in previous year (calendar year 2011 vs. 2010) Case Details Report o Exports all case data per row for the date range and criteria selected. o One to many fields (i.e. occurrences) are aggregated to a number. o Has a limitation of 2500 rows in export o Easiest way for site to begin to analyze their own data Custom Fields Report o Exports demographics and custom field data o Easiest way for a site to review custom field data they created Print Cases by Cycle o Allows printing of selected cycle cases to PDF format

16 Available Online Reports Database Statistics Ageing Case Report o Consists of cases that do not have 30 day follow up data and are within 120 days of the date of surgery o Designed to assist with 120 day case lock process. Weekly Accrual Report - Summary o Summary of cases entered and transmitted for all cycles 8-Day Cycle Report - Summary o Summary of cases entered and transmitted per each 8 day cycle with day distribution of when case was entered o Has print feature that work like the Print Cases by Cycle report

17 Available Online Reports Database Statistics List Complete Assessments o List report with completed cases within specified date range List Incomplete Assessments o List report with incomplete cases within a specified date range o Also helpful for identifying cases to work on before they lock. List Unable To Follow 30 Day Cases o List report containing cases that could not be followed up o This is indicated by information in Follow Up panel in workstation

18 Available Online Reports Database Statistics CPT Volume Analysis - Site o Summary report showing the CPT distribution of cases entered o Good for showing site case mix CPT Volume Analysis - Comparison o Summary report showing CPT distribution for comparison o Good for showing comparison case mix o Both reports can be filtered by surgical subspecialty, CPT codes, and CPT Code Volume percentage (i.e. top 10%).

20 Available Online Reports Data Analysis Measures o Graphical report that allows you to analyze your data using pre defined indicators o A graph with appear as a visual representation of your report o All reports will also contain a table with numerators and denominators for each time period. Data Download o Data generated as downloadable text files for local analysis. o Files are in CSV format bundled in one zip file which can easily imported using Microsoft Excel. o Although a little more difficult to initially work with, it does provides all case details, including information related to the one to many data (i.e. CPT code for Other Procedure). o Best option for sites to use for investigating their data.

Data Collection Case Volume o Summary report of case volumes by Bariatric procedure group with NSQIP comparison. Data Collection Follow-up o Summary report of case follow up rates along with comorbidity data. o Shows cases eligible vs. non eligible for follow up. o Non eligible cases may be patient died, patient care transferred, etc.

42 Online Reports Overview Obtaining Data Not Available Online Adhoc Requests o If you are interested in data that is not available online, you can make an ad hoc report request online. o When making a request, please be as specific as possible (date range, CPT codes, case details or summary, etc.) o Ad hoc reports reflect raw data. Risk adjusted reports will not be processed. Participant Use File (PUF) o Contains aggregate, de identified case data. o Provided on a calendar year basis. o Data available in delimited text, SAS, and SPSS file types.

43 Online Reports Overview Online Reports Summary Use different reports to help identify potential issues to investigate at your site to help improve quality, and in turn drill down into the case details for comprehensive review. Use the postoperative occurrence and mortality reports to investigate areas that Need Improvement in the semiannual report. The Case Details Report, Custom Fields Report, and the Data Download tool allow sites to explore all aspects of their own data. Share your reports and results at your site on an ongoing basis.
