1 INSTRUCTION MANUAL / GUIDELINES : HOW TO FILL UP ONLINE APPLICATION FOR THE POST OF JR/SR/TUTOR Candidates having eligible qualifications (MBBS with completion of 1 year rotating horsemanship) as on the Date of Application / Counseling (as the case may be) can apply for the Post of Junior Residents (Non Academic). In the Online Application process, the Candidates have to follow the following steps. 1) First to give payment of Application Fee (either through SBI e-collect / Online mode) or through off line / by means of Bank Draft drawn in favour of Convener, selection Committee, DMET, Odisha payable at Bhubaneswar 2) Keep scan copy of your Passport Colour Photo in.jpeg /.jpg format within 30 KB 3) Keep scan copy of your full Signature in.jpeg /.jpg format within 30 KB 4) Keep ready your mark sheets of 10 th, Intermediate (10+2), MBBS (All), Attempt Certificate (MBBS), MD/MS/PG Certificate, if any, DM/MCH Certificate (if any), Category Certificate, Medical Registration Certificate Step-1 : PAYMENT OF APPLICATION FEE ONLINE / THROUGH BANK DRAFT If you are giving payment through SBI e-pay, please follow the following steps. Open website and then click on SB Collect You will be redirected to the State Bank Collect Webpage Like below. Or you can directly open the following link

2 Then select the Terms and Conditions (Accept/ Tick) and click on Proceed and the following page will be displayed. Now select the State Odisha and type of Corporate / Institution- Govt. Department, then Click Go and the following webpage will be displayed

3 Now Select DMET Odisha, CONVENER SELECTION COMMITTEE and the following webpage will be displayed

4 Now select APPLICATION FEE From the Drop down option and then write your Name, Mobile No, ID in respective Textboxes. Then select the Name of Fee (JUNIOR RES / SENIOR RES/ TUTOR) for which you are applying. Then select the APPLICATION FEE (Amount) from the drop down. If you are applying for One Post, then the amount should be chosen as Rs.500/-. But if you are applying for more than one post / discipline like for the Post of Tutor / SR in more than one discipline, then you have to pay individually multiplication of Rs.500/- x No of disciplines applied separately. Then re-enter again your name, date of Birth, Mobile Number and captcha as shown in the form and click Submit button. You will be redirected to a confirmation page as shown below. Click Confirm.

5 Then you will be redirected to the SBI Payment page where you can see Net Banking / Credit Card / Other Bank Net Banking like different options and Click the link which is applicable to you to give online Payment. After giving the payment successfully, you will be redirected to a page which indicates that payment has done successfully. And Take the print out of e-receipt (a pdf copy below shown) which need to be submitted during the time of verification. Or you can make bank Draft by going to any Nationalized Bank as mentioned before.

6 Step-2 : PRELIMINARY REGISTRATION : 1) Please click on the Corresponding Post (Junior Resident / Senior Resident / Tutor) button in the linked website 2) You will be directed to the respective online Preliminary Registration Page like shown below. 3) Please fill up your valid ID, own created Password, Your name, Select a security question and write your own password in respective text box. 4) Please write down your ID which will be your Log in ID and your given Password in safe position so that it can be remembered easily during the time of Choice locking and Allotment letter download etc. Only one ID can be registered against one name. In no case no duplicity will be accepted by the system. Then Click CLICK HERE TO PROCEED ONLINE APPLICATION.

7 Step-3 : ONLINE APPLICATION : 1) In this Online Application please choose category Applied For :- whether You are a Direct Candidate or You are an In-service Candidate. If you are presently working under Odisha State Govt. (after selection by OPSC / Regular Job) you select In-service, else rest candidates can select Direct Candidates option. 2) In second point, your Name as filled up during preliminary Registration page will be displayed automatically and no corrections can be made. Pls do not make any entry in the Name field. 3) Please enter your Father s Name as mentioned in your Matriculation Certificate 4) Don t enter your ID again as it will be displayed automatically as filled uop during the preliminary registration time 5) Please enter / select from the Calender Control your date of birth as per the matriculation certificate. Automatically after few seconds, your Date of Birth in words and Age as on 01/01/2019 will be calculated and will be shown in respective boxes in below sections. No need to enter again. 6) Please select the Gender / Sex and Category that you belongs to. If you belong to more than one category like SC, Ex-S and Women, please Press Ctrl Key and Select all the three categories and if you want to strike off any category from the selected list, please reslect it again and it will be deselected. 7) Then please enter your permanent Address in the left side and if your Present Address is same as of your permanent Address, then select the Chk box present in the middle and automatically the address in left side will be copied to the Right side (Present address), but if bot the address are different, then you have to fill up both the address and in this case, please do not select the Chk box present in the middle. 8) Then please fill up your Educational Details and while filling the educational details, after entering the Full Marks and Marks Secured, automatically, the percentage of Marks will be calculated and displayed in next section, which will be automatically by system and no need to be entered manually. Please select the No of Extra attempt you have taken to pas the examination. If you have passed out in a single chance, then select Zero from the drop down.

8 9) Then enter your Medical Registration No, Registration Date and the Name of the Registered Council and the date of Validity of the Registration (if given) 10) Then fill up your Application fee details like Bank Draft Number in case of Bank Draft and SBI e- Pay Challan No in case SBI e-pay, bank Name as the case may be, Bank Draft or date of SBI e- Pay Challan and bank Draft Number or SBI e-pay Payment Transaction Number in respective boxes. 11) Then please fill up your present Place of Posting, Your type of Job and the CDMO Office or District address in respective section. It will be helpful in case of In-service Candidates to get relieved from their present Head quarters and Direct Candidates may leave the section unfilled / blank.

9 12) Please upload your scanned copy of passport Photo and Signature and Click on Save. 13) You will be now redirected to the PRINT Page and you are advised to Cross chk the information details as entered by yiu. If all the information found to be correct, then you can take the print out of the Application (2 Copies) and one set to be submitted during the verification time and the other can be kept with you for future reference purpose only. 14) If any information found to be entered wrong, then in spite of taking the printouts, please click HOME and you can see the EDIT option, after clicking on the EDIT option, you can edit / change the information and take the Printouts of the Final Application. 15) After successfully taking the printout of the application, please attach the following documents being self signed and submit one set of documents along with the application during the time of Verification. During the verification, it is mandatory to verify with Orginal documents failing which your application will be rejected. DOCUMENTS TO BE ATTACHED: (Self-attested photocopies) a. Original Bank Draft / SBI E-Pay Challan of Rs 500/- or online deposit proof of bank. If you are applying for more than one post or discipline in case SR/Tutor, the number of posts / disciplines that you are applying, different drafts to be deposited against each post / desciplines. b. Copy of online application form. c. H.S.C./ Equivalent Certificate in support of age; d. +2/ Equivalent Examination Certificate; e. MBBS/ BDS/M Sc. Pass Certificate; f. MD/ MS/ MDS/ DNB Pass Certificate; g. Mark sheets in support of all the aforesaid examinations (i.e. from H.S.C. to MBBS/BDS) passed including fail marks and chance certificate if any h. Certificate of Medical/Dental Registration (Both up to date UG & PG); i. Certificate from competent authority in support of Reservation; j. Valid Photo ID issued by the Govt. authorities i.e. Passport/PAN Card/Voter I card/aadhar card
