3 1076 Municipality Module Register for Industry Access First go to the LDI website at: Click the Industry Access button at the top of the page. 2

4 The Industry Access Login screen will open. The Login screen allows new users to register for Industry Access and returning users to log into the Industry Access Portal. Before you can log into the Industry Access Portal, you must first register your address. Click the Register button within the New to Industry Access section to open the Sign Up screen. 3

5 Sign Up The Sign Up screen contains fields for contact, municipality, and password information. To register for the Industry Access portal, you must fill out all fields that are marked with a red asterisk. Note: The address and password entered on this screen will later be used to log into the portal. First fill out the fields for your Full Name, Mailing Address, and Contact Information. 4

6 Once you have filled out the Full Name, Mailing Address, and Contact Information sections of the form, enter the PIN number in the Municipality Access section. Your PIN number is the number or numbers listed in the letter at the beginning of this document. Each PIN number represents a municipality. Enter the PIN number into the Add PIN field and click the Add button. Please note that multiple PIN numbers must be added one at a time. Note: If you enter a PIN number in error, click to highlight it and then click the Remove button. 5

7 Once you have entered all applicable PIN numbers, enter the password you would like to log into the portal with, and choose a security question and answer. Click the Register button to submit your request. Your request must still be verified before you can log into the Industry Access portal for the first time. 6

8 If your sign up was successful, the Sign Up Submitted screen will display. If your sign up does not succeed, please review the form to ensure that you did not leave any required fields blank and try again. You should receive a verification within minutes to the address which you used to register. This will contain a web link. Click on the link or copy and paste the link into your internet browser. This will verify your registration. You may now click the log in link to access the Login screen. 7

9 Log into Industry Access You will be returned to the Login screen of Industry Access. Enter your address in the field and the password you registered with. Then click the Log in button.. 8

10 Access the 1076 Municipality Module The 1076 Municipality Module will open. The module contains two tabs: Premium Info and Contact Info. Premium Info In the premium Info tab, select a year and a municipality from the dropdowns. Once both are selected, the grid will load all premium data for each company within that year and municipality in the grid below. To export this data to an Excel spreadsheet, click the Export to Excel button. 9

11 Contact Info Click the Contact Info tab. To view all current contact information for a municipality, select the municipality from the dropdown. The contact information for the selected municipality will load in the grid below. You can edit a contact s information by clicking the Edit/View button for the contact. You can also add a new contact by clicking the Add new record button on the grid. 10
