1 Process for downloading of Health Identity E-Card (Health Identity Card will help employees in availing Cashless Facilities in the United India TPA Network Hospitals) Sum Insured : Hospitalisation & Domiciliary Treatment Coverage (Officer: Rs.4,00,000/ : Award Staff: Rs.3,00,000/-) Every employee is requested to take a print-out of the Health Identity E-Card of him / her and also for their dependants who are under cover of Bank s Medical Insurance Scheme w.e.f Step : 1 : Please go to United India TPA website - Click on IBA BANK STAFF 1 P a g e

2 Step : 2 : On clicking IBA BANK STAFF, System will show User the below mentioned screen. Click on E-CARD 2 P a g e

3 Step : 3 : On clicking E-CARD, System will show User the below mentioned screen. Select Bank from the drop down box UNITED BANK OF INDIA and Put SPF Number in EmpID column. (Card Number is not necessary for generation of E-Card) 3 P a g e

4 Step : 4 : After selecting United Bank of India (FOR EXISTING EMPLOYEE) and United Bank of India-Retired (for retired employees) from drop down box and on putting SPF No. in EmpID column and On Clicking to Search button (just below EmpID field) the System will show User the below mentioned screen Take a print out of all the Health Identity E-Card of you and your dependants. 6 Process for downloading of Health Identity E-Card (for TOP-UP) (Health Identity Card will help employees in availing Cashless Facilities in the National Insurance (Family Health Plan) TPA Network Hospitals) Sum Insured : Hospitalisation & Domiciliary Treatment Coverage Rs.6,00,000/- for all category of existing & retired employees (Domicile is not admissible in case of retired employees) This facility is applicable over and above Rs.4,00,000/- (Officer) and Rs. 3,00,000/-(Award Staff) Every employee is requested to take a print-out of the Health Identity E-Card (for TOP-UP) of him / her and also for their dependants who are under cover of Bank s TOP-UP Medical Insurance Scheme w.e.f to Step : 1 : Please go to National Insurance Family Health Plan TPA website - Select E-CARD in LOGINS 1 P a g e

7 Step : 2 : On clicking E-CARD, System will show User the below mentioned screen. Click on E-CARD Put Corporate ID : 3880 (for existing employees) and 3851 (for retired employees) and Put your 5-digit SPF Number (fill first field with 0 if your SPF Number is of four digit) in both the fields User Name and Password On filling the information please Click on Login 2 P a g e

8 Step : 3 : On clicking LOGIN, System will show User the below mentioned screen. Click on Member Detail for detail 3 P a g e

9 Step : 4 : On clicking the Member Detail, System will show User the below mentioned screen. To get the E-card, click on CLICK HERE System will download all E-Card. Take a print out of all the Health Identity E-Card (FHPL) of you and your dependants. Try to carry the same with you for any unforeseen incident. 4 P a g e
