1 1 Client Intake Form ICANETWORK MOBILE APP FORM Hello and congratulations! We are excited that you have made the decision to improve your mobile presence. We really look forward to working with you and making your business as successful as it can be. In order for us to set up your Mobile APP properly we need to know some details about your business. We suggest you to be as thorough and detailed as possible by taking a little extra time to include as many details about your business as you can. This ensures efficiency, which helps us to complete your project and makes sure we get things done in a timely fashion. If you have any additional questions, please feel free to give us a call or us. Contact Information Your Full Name: Position: Your address: Phone #: Fax #: App Title: 1 ST CHOICE 2 nd CHOICE: The maximum is 15 characters. Keep it SHORT! Your App Title will be used as the text under the icon for your app on your subscribers smartphones. Because there is a small amount of space under the icon, we recommend limiting the number of characters to 7 for all caps, or 9 for upper and lowercase characters. (Examples: "John S", "Joe's Pizza", "Plumbing", etc.) App Address: 1 ST CHOICE 2 nd CHOICE: This will be the mobile website/ app address you give your customers to visit and then download your app. (Examples: johns, joespizza, plumbing). It can be the same as your app title or different. Do not use spaces or any characters except letters and numbers. Select Phone App SMS Keyword: 1 ST CHOICE 2 nd CHOICE: Your mobile app includes unlimited text messaging to all your text subscribers (U.S. only) Keep your keyword short & simple! This will be used as the unique word your customers will use to subscribe to your texting service. (example "pizza")

ICON: This image is what your customers will install on their smart phone, identifying your phone app. Please submit the image you would like us to use in JPG or PNG format and name the file icon.jpg or icon.png depending on the format you use. HOME PAGE Your custom mobile app includes your Home page, Contact page, Alert page, Join Us page, Three (3) open-format pages, plus a link to share your app via or text message. The Home page of your app / mobile website also serves as its Main Menu, listing all the key pages available to the user. This page shows a custom image that will serve as the Main Banner of your App and Mobile Website. The image is also used in both the Contact Us and the Join Us pages. Please submit the image you would like us to use in JPG or PNG format and name the file homepage.jpg or homepage.png depending on the format you use.

4 4 Client Intake Form Alert Page The Alert Page is the very first page listed in the Main Menu. Use this space for a special Alert Message to all your app users and SMS subscribers. IDEAS: Use the Alerts Page to announce your promotional offers, or to feature your "Today's Specials" or "Deal-of-the-Day" marketing campaigns. This message can be changed at any time thru your unique app backoffice. You can use up to 250 characters for your Alert Page message. This page also gives your app users instructions to sign up for your text messaging. Give your customers a reason to sign up to receive text messages from you right to their smart phone. This page also shows a custom image of your choosing. Please submit the image you would like us to use in GIF, JPG or PNG format and name the file AlertPage depending on the format you use. Please include the text for your Alert Page here (250 characters or less):
5 5 Client Intake Form Pages One, Two, & Three Pages 1, 2, & 3: These are "open-format" pages. Each include an image, headline, and up to 1,400 characters and spaces. Each page can use the same image or different images. Please submit the image you would like us to use in GIF, JPG or PNG format and specify/ name the image files Page1, Page2, & Page3. For example, 1. A restaurant may use 'Page 1' to introduce their cuisine; 'Page 2' to highlight a popular menu item; 'Page 3' to list hours, talk about their head chef, or list customer comments. For the images, a restaurant may want to include photos of their restaurant, staff, signature dish, head chef, or awards. 2. A real estate agent may use 'Page 1' to introduce himself; 'Page 2' to highlight a particular area; 'Page 3' to highlight a particular property. A real estate agent may want to include a photo of his or herself, a highlighted property, a bird's eye view of their area, or their "for sale" sign or office. 3. A business may decide to use 'Page 1' for highlighting products or services; 'Page 2' for client testimonials; 'Page 3' to talk about their business history or background.

An effective App does not try to sell anything to your customers. An App is not intended to have all of the content that is on your website. Try to stick close to what your business does. Add value and solutions. A good App is user-friendly. It must be add value to for the customer to connect with you on their mobile device. Most just want to know what you do and how to contact or find you. What would you like the pages to be on your APP? Change the headings as you see fit then fill in the content/text where indicated. Try to be concise. On an actual smartphone this page will have a registration form tab for the user to enter their name, address, and phone number.
