2 This is the link to the Clearinghouse: *You have to use Mozilla Firefox or Google Chrome. It doesn t work with Internet Explorer. *The Representative is considered the User. The Consumer is considered the Provider. This doesn t make sense but when AHCA created this clearinghouse, they were not thinking about CDC+. *Write down and save the User Name and Password you create. Neither DCF, AHCA nor APD has access to that information. If you forget these you have to create a new User Name and Password and start over. *Keep your updated in the Clearinghouse. You will be ed alerts if an employee gets arrested, is due for five year re-screening or if there are new results to go in and view.

3 *When you send in the User Agreement, the Rep signs both lines at the bottom- they sign as User and Review and Approval by Provider. *Make sure to copy your government issued ID (Driver s License) and send that in with the User Agreement to DCF. The fastest way to get the approval back is to scan and the signed User Agreement and ID to DCF at the address listed at the top center of the User Agreement. *After DCF reviews your information they will you to tell you that you have been approved to use the Clearinghouse. Once you receive that , sign back into the Clearinghouse using the user name and password you created and you can search for people, initiate a screening, locate a Live Scan vendor, etc.

4 Background Screening Clearinghouse In response to the requirements passed during the 2012 Legislative session, the Agency for Health Care Administration (AHCA) created the Care Provider Background Screening Clearinghouse, or Clearinghouse Website. The legislation specified the agencies to be included in the Clearinghouse.

5 The following agencies are included in the Clearinghouse: Agency for Health Care Administration Agency for Persons with Disabilities Department of Children and Families Department of Elder Affairs Department of Health Department of Juvenile Justice Division of Vocational Rehabilitation

6 Allows each agency to make a determination on screening results using criteria established for the programs they oversee. Creates a status report and a completed screening listing report of screenings requested by the user eliminating the need to search for each screening result individually. Maintain an employee roster by entering hire and separation dates for each employee. This facilitates a notification if the eligibility status of an employee changes. This must be done within 10 business days of action.

7 Providers will no longer utilize the DCF Background Screening Web Portal. Providers will no longer receive the DCF Clearance Letter but will have access to the FDLE Public Record with their screening. The provider will print the screen for documentation. Providers will be required to use the Clearinghouse to request screenings and maintain employee roster. There is one ORI number for APD and each CDC+ Consumer receives their own OCA number.

17 Provider Type is APDCDC+. Provider Name is the CDC+ Consumer s name. Type their first name, then a space, then their last name. Then Click Add Provider.

18 Don t do anything on this page except click the Submit Request and Generate User Agreement button.

19 Print this page. The Representative signs their name on both lines at the bottom of the page. Send signed User Agreement and copy of Drivers License to the address in the box below.

20 After requesting access, a link will appear under Program Access Within a few days you will receive an from DCF saying that you are approved to use the Clearinghouse. Sign back in using the user name and password you created. Then click the blue Background Screening Clearinghouse link above.

21 After approval, you will be able to access the Clearinghouse. Click the blue Background Screening Clearinghouse link.

22 The Blue Bar below lets you choose what you want to do.

23 To Search: You must enter the Social Security Number of the employee AND Last Name OR Date of Birth Then Select Search If the person does not come up then you will need to Initiate a Screening.

24 If the individual has not previously been screened through the Clearinghouse, select the Initiate Screening Buttonat the bottom right of the form. If the individual is in the Clearinghouse, click the Initiate Agency Review Button.

25 Request a screening Enter all required information, as designated by the red asterisks (*). Ensure all information is accurate and select the Next button.

26 Select the position type that individual is applying for from the drop down list. For CDC+ the position will be Employee or Staff Person.

27 Select the Privacy Policy link to view and print the privacy policy. Have the employee sign and date the form. Check the affirmation box to confirm that the applicant has signed and agreed to the Privacy Policy. If you do not do this, the results will not be sent to you. The results will say, Awaiting Privacy Policy. Also, you need to save a copy for your Delmarva reviews. After you click that box you should be able to print a copy of the referral to give to your employee to take to the Live Scan vendor. This form has all the information the Live Scan vendor will need, including the ORI and OCA numbers. You do not have to schedule an appointment in the Clearinghouse for your employee unless you would like to. You can search for approved Live Scan vendors in the Clearinghouse. Check around for prices. They all charge different prices BUT the cheapest it will be is around $65. We have heard that some have charged up to $110. A photograph must be taken at the same time as the fingerprints. Since the Florida Department of Law Enforcement made this a requirement, not all of the old Live Scan vendors are equipped to take a photo. If they cannot take a photo- do not have your fingerprints done there.

28 Allows providers to search for approved Live Scan Service Providers.

29 Allows the user to select from a search of Live Scan approved operators, or skip if they have other access to a Live Scan machine. **You do not have to search for a Live Scan vendor here. You may just click the Submit button under the Search button to get the form you need to give to your employee.

30 Select a Live Scan Provider. Set an appointment for fingerprinting. Setting an appointment isn t required. It is better to call local Live Scan vendors and make sure they take APD CDC employees and that they take a picture at the same time they do the fingerprints.

31 The system will confirm when the screening request has been submitted. You can print the screening appointment form.

32 This is what the form looks like that you will give the employee to take to the Live Scan vendor. The applicant will provide this form to the Live Scan Service Provider when being fingerprinted.

33 Contains the screening eligibility status. View the public version of the criminal history report (if you initiated the screening). Connect to a screening that is already in process Receive notifications when the screening is complete. Provides an employment history for the individual as reported by any provider regulated by an Agency in the Clearinghouse.

34 Providers must register with the clearinghouse and maintain the employment status of all employees within the clearinghouse. Initial employment status and any changes in status must be reported within 10 business days.

35 For those employees hired prior to the completion of screening for orientation or training purposes, the provider may add a provisional hire date for an applicant with a current Screening in Process. You will use this screen to record any change in employment.

36 Provides a listing of all screening requests initiated or connected to and the current status. Requests will remain on the list for 7 days once a determination is made. View an individual s profile page by selecting the last name of the individual. Filter the list by using the search options and selecting Apply. Sort the list by selecting a column header.

37 Provides a listing of all screening requests initiated or connected to and the final determination.

38 Provides a listing of employees as entered through the Employment History section of the individual s profile page.
