1 Wellness Website Registration Process *New Hires have access to the site on the Friday of the week of their hire date. 1. Go to 2. If you are accessing the site while logged into the Baylor Network, you will be directed to the page below. You will be asked to enter your BHCS username and password. This is your mybaylor username and password associated with your BHCS network access. a. On a Windows 7 workstation the SSO page will appear as below i. If the BHCS domain is selected, the User Name will be the employee s network login (Ex: e12345 or SmithJ) ii. If the BHCS domain is not selected or not available, the User Name must be bhcs\ and the employee s network login (Ex: bhcs\e12345 or bhcs\smithj iii. The password would be the user s network password For User name enter your BHCS / MyBaylor Username Ex: e12345 or SmithJ For Password enter your BHCS / MyBaylor password Contact the BIS HELP DESK at if you need assistance with your MyBaylor username / password

2 b. On a Windows XP workstation the SSO page will appear as below i. The User Name will be bhcs\ and the employee s network login (Ex: bhcs\e12345 or bhcs\smithj ) ii. The password would be the user s network password For User name enter your BHCS / MyBaylor Username Ex: bhcs\e12345 or bhcs\smithj For Password enter your BHCS / MyBaylor password Contact the BIS HELP DESK at if you need assistance with your MyBaylor username / password You will then be taken to the Thrive for Wellness Registration page shown in Step 3. If you are accessing the site while NOT logged into the Baylor network, you will taken directly to the Thrive for Wellness Registration page shown in Step 3 below. 3. All Baylor employees & spouses are New Users and must setup an account for the Thrive for Wellness site Click on Start Registration

Enter your unique ID, group number and date of birth Unique ID: Employees enter Baylor ID (no zeros or e ex: 12345) Spouses enter full 9 digit SS# (ex: ) Group Number: H75160 for all Baylor employees and spouses Date of Birth: Enter with MM/DD/YYYY format 5. Answer 2 security questions, which will help identify you should you need to reset or unlock a password in the future 6. Create a unique Username and Password for your Thrive for Wellness account Username: Must be 6-32 characters and must contain at least 2 numbers and 1 letter. Spaces and special characters, including punctuation and symbols are not allowed. Password: Must be 6-32 characters and must contain at least 2 numbers and 1 letter. Spaces and special characters, including punctuation and symbols are allowed. Your password cannot be the same as your username and is case-sensitive.

4 7. Sign in as a Returning User to start using your Wellness Website 8. Accept Terms of Use this illustrates each party s responsibility for use of the website. Hit I Agree to accept the terms. Need Help? Call , Monday Friday, 7 am to 7 pm CT

5 9. Provide your preferred address. You may opt out of providing an . If you opt out, an additional acceptance screen will display. Click I Understand to begin using the portal. NOTE: If you opt out of s, you will NOT receive confirmation of completion of activities notifications. 10. 6 11. Click the Take my Wellness Assessment now! link, located on the right-hand side of the page. 12. Once you click Submit / Finish, you will see a confirmation notice stating Thank you. Your Wellness Assessment has been submitted and is complete. You will also receive a confirmation to your preferred within the Thrive system within 1 hour of completion.

7 13. After you have completed the Wellness Assessment and your 2015 Screening Form has been received, you will be able to view your Personal Wellness Report. Click on Review Report The most current results will be on the top of the page and the previous years will be below that.

8 14. AFTER your initial registration within the Thrive for Wellness website you will then be able to use the Single-Sign On feature. When logged into a Baylor network, Baylor Employees may access the Thrive Website directly from MyBaylor! Click on About You, Benefits, Thrive Wellness Program links to access. Remember to update any of your MyLinks on the MyBaylor Home Page and any of your saved Favorites so you may access the updated Thrive site You will then see your personal Thrive for Wellness account Home Page.

9 To select Health Related Topics to receive additional information: Click on My Profile Click on Health Information

10 Click on the topics you wish to receive information about and hit Save to capture any changes:
