Tariff No. 4 - Telephone, for incorporation into the Company s tariff. With this submission, Granite complies with Tariff Filing Requirements, Section H.34, of its Enterprise Infrastructure Services contract with the U.S. General Services Administration. 5 Original Sheet No. 86 SECTION 6 - FEDERAL GOVERNMENT SERVICE AGREEMENTS 6.1. General The Company may enter into contracts for the use of all authorized Federal agencies; authorized federal contractors; agency-sponsored universities and laboratories; and when authorized by law or regulation, state, local and tribal governments, and other organizations. All organizations listed in General Service Administration (GSA) Order ADM E (as updated) are eligible Enterprise Infrastructure Services (EIS) Services In addition to services covered elsewhere in this Tariff, the Company may offer services pursuant to the EIS contractual agreement. A. Terms and Conditions 1. EIS Services are only available pursuant to the EIS contractual agreement. 2. The Company reserves the right to limit the locations where services will be offered. B. Rates and Charges 1. The nonrecurring charge below applies for the installing, connecting or moving of EIS Services. 2. If the Company continues to provide service after the expiration of the customer s term without a further agreement, the customer s monthly recurring term rate shall continue to apply until the customer enters into a new service agreement or the service is disconnected. 3. The following maximum rates and charges apply.
