1 Registration Information & FAQ Thank you for your interest in our popular Math Olympiads course. In order to ensure your registration is valid and processed correctly it is imperative that you read carefully and follow the instructions below. Any registration form that is incomplete will not be processed. Please call with any registration questions. Note that the City of Cupertino reserves the right to change any of the information below at any point during the registration process. Registration may be submitted in three ways: Registration Process 1. Drop-off your COMPLETE registration form, including payment, in person to the Quinlan Community Center: N. Stelling Road, Cupertino CA Our office is open during the following times: Monday-Thursday- 8:00am-10:00pm Friday- 8:00am- 5:00pm Saturday- 8:30am- 4:30pm 2. Fax your COMPLETE registration form to our secure line at Mail the COMPLETE registration form to the Quinlan Community Center: N. Stelling Road, Cupertino, CA You may pay with cash, check, Visa, MasterCard, American Express, or Discover Card. Checks must be made payable to CITY OF CUPERTINO. There will be a $27.85 service charge for all returned checks. 1 st Day of Cupertino Resident Registration: July 24 th 1 st Day of Non-Resident registration: July 31 st Registration forms dropped off BEFORE the registration dates will NOT be accepted. Ranking your class choices: Rank the choices of each section for which you would be willing and available to attend. Do not mark any days that you cannot attend. (Mark 1 for first choice, 2 for second, etc.). Families who wish to change their section after securing a spot will be moved to the bottom of the waitlist. When registration is processed, you will be placed based on your ranking selection & space. If your first choice is full, you will be placed in the next class available based on your ranking. If no spaces are available in any of your marked choices, you will be placed on the waiting list. Payment will not be processed, unless you have a spot. Rank your choices carefully. If you want to switch after a class has been assigned, you will be removed from the class and be placed at the bottom of the waitlist. Math Olympiad Registration Information & FAQ 1

2 Registration Information & FAQ Random Processing Procedure All registrations received between July 24 th and August 7 th will be entered into daily random processing. For example, forms received between 12am and 11:59pm on the July 24 th date will be randomly processed during our regular business hours on July 25 th. Registrations submitted after August 7 th will be placed in classes on a first-come, first-serve basis and are subject to availability. Due to the large volume of applicants, confirmation notifications will be mailed home between 1-2 weeks following the August 7 th date. Refund Policy Withdrawals must be requested at least 8 calendar days prior to the start of the class for a full refund. Withdrawals requested 7 calendar days or less prior to the first class meeting will incur a 10% processing fee. Refunds will not be issued 3 calendar days or less before the start of the class. No refunds will be issued once the class has begun. Frequently Asked Questions Q: I will be out of the country during the Info Night and/or registration days! How can I register in time? A: You may still register in time by doing one of the following: 1. Have a family member or friend turn in the completed registration form for you. Parent must sign the liability waiver. 2. Fax in the completed form to We will not accept faxes prior to registration date. In order to get a copy of the registration form, please visit our website at the following page: The registration form will be available AFTER the Math Olympiad Info Night. 3 Registration Information & FAQ When submitting registration forms, please be sure ALL FIELDS are filled out properly. Forms must have the following information to be considered complete: Student name and information Parent contact information (WRITE LEGIBLY - USED FOR MR. G s PARENT S!) Ranking of class choices Liability waiver signed & dated by parent Payment type If paying by card, complete credit card information CC #, CVC #, expiration date, and signature INCOMPLETE OR EARLY SUBMITTED FORMS WILL NOT BE PROCESSED. Q: Can I in my registration forms? A: In order to be financially compliant, we CANNOT accept ed registration forms. Any ed forms will NOT be processed. Q: What is Mr. G s ? How can I contact him? A: Cupertino Recreation staff will be able to assist with questions regarding registration and other Cupertino Recreation courses. For all questions regarding the Math Olympiads class, curriculum, class make-ups, exam results, progress reports, and events, please contact Mr. G at matholympiads123@gmail.com. Q: If my child attends a CUSD school or I work in Cupertino, but we live in another city, am I considered a Cupertino resident? A: At this time, Cupertino resident registration and residents fees are only for families who currently live and hold an address within the Cupertino city limits. We will be using our internal systems to verify residency. Forms with discrepancies will experience a delay in processing and additional proof of residency may be required before final processing. Q: How can I ensure my child gets in the class? A: Being open with your scheduling will increase the chances of getting a spot. However, you must guarantee that you can attend the sections you have selected as options. Q: My schedule has changed, may I transfer to another day/time? I hereby acknowledge that I am voluntarily participating in this event or class and I agree to assume such risks, including risk of personal injury, death, or damage to property.
