1 MAXIMUS Provider Billing Application v.3 Sign In and Forgot Password The following training documentation contains detailed step-by-step instructions and may be used as reference in understanding the Sign In and Forgot Password features and are broken down into the following sections: Sign In and Forgot Password Process and Exception Handling Sign In Step-by-Step Instructions Forgot Password Step-by-Step Instructions Sign In and Forgot Password Additional Reference Materials Sign In and Forgot Password Process and Exception Handling Sign In will be used to gain access to the MAXIMUS Provider Billing Application v.3 authenticated user features such as Certificates, Attendance Entry, and Remittance Statements. Sign In requires that the Account Activation process has been completed prior to Sign In. The setting of the Password (for Sign In) and the Security Questions (for Forgot Password) are important parts of the Account Activation process that are user-specific login criteria necessary to Sign In. The process for Sign In and Forgot Password consists of the following components: User gets 5 tries to enter their password successfully, after the 5 th they become locked out of the system. Locked out users must contact a MAXIMUS Customer Service Representative for assistance with resetting the password. For sign in failed attempts 1-4, the user is encouraged (with link provided) to perform the Forgot Password function. Notifications occur for the 3 rd failed attempt (MAXIMUS CSR) and on for the user account lock out situation (both user and MAXIMUS CSR). Similar to the Account Activation process, checks also occur here to ensure a user Password is set (if a PW reset situation), Security Questions are set, and a Location is selected (if applicable) before landing on the user-specific Dashboard. Sign In Step-by-Step Instructions 1. Navigate to the CAPS Provider Billing Application v.3 Home Page by typing the following URL in your web browser:

2 2. Click on the SIGN IN link in the upper right hand corner of the page 3. Enter Username and Password and Click Sign In. The Username will always be an Address and will be the one that is activated per the Account Activation process. If this has not been completed, refer to the Account Activation Training Guide to complete that process.

3 4. Beware of CAPS lock. If the password provided matches the password that has been set for the user, they will next be presented with the PBA User Dashboard page. If the user is associated with multiple Providers, a Location Selection page will appear next to allow the user to select which Provider account they wish to manage using the application. If presented, the user must click the GO button for the account they wish to manage.

4 5. Upon successful user activation and location selection (if applicable), the user is then presented with their location and role-specific User Dashboard.

5 6. If the password provided DOES NOT match the password that has been set for the user, an error message appears with a link to the Forgot your password? page. The Forgot your password? feature is also accessible at the bottom of the Sign In portion of the page as shown with the arrows below.

6 a. After the 4 th failed login attempt, the Error message changes to indicate that they will be locked out of the application after the next failed login attempt: b. After the 5 th failed login attempt, the Error message changes to indicate that they are locked out of the application and to contact MAXIMUS for assistance:

7 Forgot Password Step-by-Step Instructions 1. Click on the Forgot password? link either in the red error message or the Forgot your password? link in the bottom of the Sign In portion of the page. See arrows for the screenshot provided above. Enter your address and Click the Next button.

8 2. Enter the answer to the Security Question and click the Next button.

9 3. The user is then brought to the Enter new password page to set a new password. This password must conform to the criteria specified for the input box. As the criteria is met, a green check will appear next to the satisfied criteria. The user must also confirm this password by retyping it. The user then selects Next button. 4. A Password Changed Successfully! message appears to indicate that the password change was successful. From this page, the user can then click on SIGN IN in the upper right corner and proceed with the Sign In process following the steps outlined earlier.
