5 Page iii Scope and Field of Application 1 INTRODUCTION This supplement describes ten new Transfer Syntaxes to embed High Efficiency Video Coding (HEVC) / H.5 Main Profile / Level 5.1, Main Profile / Level 5.1, Scalable Monochrome Profile / Level 5.1, Scalable Monochrome 1 Profile / Level 5.1, Scalable Monochrome 1 Profile / Level 5.1 and Scalable Main :: Profile / Level.1 encoded pixel data in DICOM. It does not introduce any new SOP Classes or IODs. HEVC allows two new major enhancements of the DICOM standards: 1) The improvement of ::0 content compression efficiency ) The improvement of the lossless content management IMPROVEMENT OF THE ::0 CONTENT COMPRESSION EFFICIENCY The use of video and still image data in the medical industry has increased and new technologies providing better colors or higher precision are available on the market. Meanwhile, the needs for reduced storage and media exchange cost remains important. To answer the related demand for higher ::0 compression efficiency, this supplement proposes to add the following profiles. Transfer Syntax HEVC/H.5 Main Profile /Level 5.1 will perform consistent with the ISO/IEC 0-:01 HEVC Main Profile at Level 5.1. This will enable the storage of video files with a resolution of 09x at 50Hz/0Hz. Transfer Syntax HEVC/H.5 Main Profile /Level 5.1 will perform consistent with the ISO/IEC 0- :01 HEVC Main Profile at Level 5.1. This will notably enable the storage of video files with a higher dynamic range and a wider gamut space and a resolution of 09x at 50Hz/0Hz. 3 IMPROVEMENT OF THE LOSSLESS CONTENT MANAGEMENT In this supplement, it is also proposed to include new profiles in order to offer a more efficient way to store lossless images. The targeted applications include using HEVC streams to store and exchange lossless images. The proposed profiles aim at the compression of Monochrome and :: images as these are the ones which usually require processing and/or storage in a lossless format. The proposed HEVC profiles for monochrome and :: lossless support are based on scalability to bring a new efficient way to share and manage lossless images within PACS, and between PACS and other facilities or other devices inside a facility. The proposed new format of lossless stream would be composed of a lossy light base (with possibly a high quality) and a lossless heavier complementary stream. By using the profiles introduced below, it is possible to manipulate the lossy and the lossless versions of images without any additional compression on the storage side. The proposed syntax would allow for a software encoding roughly times slower than JPEG and decoding of roughly the same speed as JPEG (please note that these numbers are given as indications and may vary depending on the implementation). The expected compression gains which can be expected when using the transfer syntaxes below compared to using transfer syntaxes with JPEG000 are of about 1% to % depending on the content. The reader should be reminded that these syntaxes are proposed for the extra features they bring rather than the raw compression efficiency they bring.

This will enable the storage of monochrome sequences of still pictures with a resolution of 09x at 50Hz/0Hz in a lossless or lossy format comprising a detachable lossy and lighter version. Such content can typically be produced from CT scanners. Transfer Syntaxes Lossless HEVC/H.5 Scalable Monochrome 1 Profile /Level 5.1 and Lossy HEVC/H.5 Scalable Monochrome 1 Profile /Level 5.1 will perform consistent with the ISO/IEC 0- :01 HEVC Scalable Monochrome 1 Profile at Level 5.1 except for additional constraints on the enhancement layers of the coded bitstreams. Such content can typically be produced from CT scanners. Transfer Syntaxes Lossless HEVC/H.5 Scalable Main :: Profile /Level.1 and Lossy HEVC/H.5 Scalable Main :: Profile /Level.1 will perform consistent with the ISO/IEC 0-:01 HEVC Scalable Main :: Profile at Level.1 except for additional constraints on the enhancement layers of the coded bitstreams. This will enable the storage of RGB video files and sequences of still pictures with a resolution of 190x0 at 50Hz/0Hz in a lossless or lossy format comprising a detachable lossy and lighter version. Such content can typically be produced from CT scanners. The levels defined in this section are meant to constrain the maximum picture size but not the decoder buffer size contrary to what is written in the HEVC specifications. This follows the simple fact that when encoding a video in lossless mode, the compression rate cannot be guaranteed. It is important to note that media file format and media applications standards are usually not designed for monochrome or :: images. Planning for a future compatibility with such standard is therefore deemed unnecessary. SCOPE OF THE SUPPLEMENT This proposed supplement includes Addenda to existing Parts of DICOM: PS 3.5 Addendum: Data Structures and Encoding PS 3. Addendum: Data Dictionary PS 3.11 Addendum Media Storage Application Profiles 5 Licensing issues Fujitsu does not have any patent pertaining to the support of HEVC bitstreams inside DICOM. HEVC has now two pools of patents which define royalties to pay for when buying or using HEVC codecs: MPEG-LA patent pool ( HEVC Advance ( While MPEG-LA requires a royalty based on the purchase of equipment, HEVC Advance requires companies to pay royalties on the basis of HEVC streams they produce which they sell to customers. Interested readers are encouraged to look for further information with the help of specialized professionals.

Times are in seconds, relative to the value of the Content Time (000,0033) in the parent object. The range shall include all frames between the specified times including any frames at the specified times. The range may be expanded as a consequence of the format in which the information is stored. Where such expansion occurs, any embedded audio data shall be similarly selected. Under all circumstances, the returned Composite SOP Instance shall retain the relationship between image and audio data. DICOM provides a mechanism for supporting the use of HEVC/H.5 Image Compression through the Encapsulated Format (see PS 3.3). Annex A defines a Transfer Syntax that references the HEVC/H.5 Standard. The use of the DICOM Encapsulated Format to support HEVC/H.5 compressed pixel data requires that the Data Elements that are related to the Pixel Data encoding (e.g., Photometric Interpretation, Samples per Pixel, Planar Configuration, Bits Allocated, Bits Stored, High Bit, Pixel Representation, Rows, Columns, etc.) shall contain values that are consistent with the characteristics of the compressed data stream, with some specific exceptions noted here. The Pixel Data characteristics included in the HEVC/H.5 bit stream shall be used to decode the compressed data stream Notes: 1. These requirements are specified in terms of consistency with what is encapsulated, rather than in terms of the uncompressed pixel data from which the compressed data stream may have been derived.. When decompressing, should the characteristics explicitly specified in the compressed data stream be inconsistent with those specified in the DICOM Data Elements, those explicitly specified in the compressed data stream should be used to control the decompression. Source pictures that have a lower value should be re-formatted by scaling and/or pixel padding prior to HEVC/H.5 encoding.

16 Page 3. The Frame Time (001,3) may be calculated from the frame rate of the acquiring camera. A frame rate of 9.97 frames per second corresponds to a frame time of ms.. The value of chroma_format_idc for this profile and level is equal to 1, indicating the usage of ::0 content. One fragment shall contain the whole HEVC/H.5 bit stream Note: If a video stream exceeds the maximum length of one fragment (approximately GB), it may be sent as multiple SOP Instances, but each SOP Instance will contain an independent and playable bit stream, and not depend on the encoded bit stream in other (previous) instances. The manner in which such separate instances are related is not specified in the standard, but mechanisms such as grouping into the same Series, and references to earlier instances using Referenced Image Sequence may be used. The container format for the video bitstream shall be MPEG- Transport Stream, a.k.a. MPEG-TS (see [ISO/IEC 131-1]) or MPEG-, a.k.a. MP container (see [ISO/IEC 19-1] and [ISO/IEC 19-1]). The PTS/DTS of the transport stream shall be used in the MPEG coding. When decompressing, should the characteristics explicitly specified in the compressed data stream be inconsistent with those specified in the DICOM Data Elements, those explicitly specified in the compressed data stream should be used to control the decompression. The DICOM data elements, if inconsistent, can be regarded as suggestions as to the form in which an uncompressed Data Set might be encoded The requirements are: Planar Configuration (00,000) shall be 0 Samples per Pixel (00,000) shall be 3 Photometric Interpretation (00,000) shall be RGB Bits Allocated (00,00) shall be Bits Stored (00,01) shall be High Bit (00,0) shall be 7 Pixel Representation (00,03) shall be 0 The value of HEVC/H.5 sample aspect_ratio_idc shall be 1 in the encapsulated HEVC/H.5 bit stream if aspect_ratio_info_present_flag is 1. Pixel Aspect Ratio (00,003) shall be absent. This corresponds to a 'Sampling Aspect Ratio' (SAR) of 1:1. The values for Rows (00,00), Columns (00,0011), Cine Rate (001,000), and Frame Time (001,3) or Frame Time Vector (001,5) shall be compliant with the Scalable Main :: Profile / Level 5.1 of the HEVC/H.5 standard ([ISO/IEC 0-:01]) and restricted to a square pixel aspect ratio. The value of HEVC/H.5 vps_max_layers_minus1 shall be 1 for all VPS (Video Parameter Set) data present in the encapsulated HEVC/H.5 bit stream. The value of HEVC/H.5 cu_transquant_bypass_flag shall be 1 for all coding units associated to a picture with nuh_layer_id equal to the value of vps_max_layers_minus1 of its associated VPS in the encapsulated HEVC/H.5 bit stream. The value of HEVC/H.5 slice_type shall not be equal to for all slices associated to a picture with nuh_layer_id equal to the value of vps_max_layers_minus1 of its associated VPS in the encapsulated HEVC/H.5 bit stream.
