Or alternatively print your name, postal address and phone number in the spaces provided on the booklist. Place a tick in the TICK BOX for items that ARE required. Return this booklist to school or submit online by the date shown. If you are unable to return your booklist to school by the due date, or lodge online, booklists can be submitted directly to Ballarat Books. An advice card showing the total cost of your booklist will be sent prior to distribution. Payment can be made by Cash, EFTPOS, Credit Card or Money Order. Pre-payment can be arranged through Ballarat Books prior to the parcel collection date. Lists received after 20th December 2016 will incur a $4.5 late fee.
