1 Individuals with Managing Control Individuals with Managing Control The following will explain how a user fills out the Individuals with Managing Control topic of the Medicare enrollment on PECOS. This topic is typically found when filling out a group enrollment. However, if a provider is a Sole Proprietor or Sole Owner they may be required to complete this topic depending on how they answered the questions in the application questionnaire. 1. The "Individuals with Ownership Interest and/or Managing Control" topic asks if the applicant has any individuals having managing control to report. If the user has anyone to report, they will answer "Yes" and select "Add Information." Every application is required to have an Owner, Authorized Official and W-2 Managing Employee. If they do not have an Owner, AO and a W-2 employee listed, the user will not be able to submit the enrollment.

2 2. The user will enter the "Personal Information for the Individual with Ownership Interest and/or Managing Control" of the organization and select "Next Page." Since the topic is asking for information about an individual person, the user will select SSN from the "TIN Type" and then enter the individual's SSN under "Tax Identification Number (TIN)". If the user selects TIN from the "TIN Type" drop-down menu and enters an ITIN they will not be able to complete electronic signatures.

4 3. The user will pick the individual's "Country of Birth" from the drop-down menu and then "Select". The user will select the "State/Territory" where the individual was born from the dropdown menu and "Next Page". If the user selects a country that is not in the United States, the State/Territory field will become a type in field. 4. On the Individual s Relationship to the Applicant, the user will need to check all options that apply to them, enter the effective date and select "Next Page."

6 5. The user selects "Neither an Authorized nor a Delegated Official", "Authorized Official", or "Delegated Official". The user will enter the individual's phone number, if it is required, and select "Next Page. If the phone number is grayed out, then a response is not required. Every application requires at least one Authorized Official. This person will sign the application during the submission process.

7 6. The user will indicate if the individual has ever had a Final Adverse Action.

8 7. If the individual has had a Final Adverse Action, the user will enter the information and select "Save." 8. The user will be taken to the Topic Summary and be able to view all the information that was added. If something needs to be corrected or deleted, the user will select "Edit" or "Delete".The user can select "Go to Error Check" or "Next Topic" to continue.

10 9. The user will see the tabs for "Topic View," "Fast Track View," or "Error/Warning Check." If the Individual Control topic is still in the "Error/Warning Check," the user must read the error information shown, go back to the "Individual Control" topic by selecting the name of the topic, and correct the information.

11 10. The user will see all the information previously entered. To add a different person as the W-2 Employee, the user will select "Add Information". To edit previously entered information the user will select Edit under the Individual's Relationship to the Applicant, if the user chooses to edit previously entered information skip to step 13.

12 11. The user will enter the "Personal Information for the Individual with Ownership Interest and/or Managing Control" of the organization and select "Next Page." Since the topic is asking for information about an individual person, the user will select SSN from the "TIN Type" and then enter the individual's SSN under "Tax Identification Number (TIN)".

13 12. The user will pick the individual's "Country of Birth" from the drop-down menu and then "Select". If the user selects a Country that is not the United States the State/Territory field will become a field that the user can type in. 13. On the Individual s Relationship to the Applicant, the user will check "Managing Employee (W-2)" and enter the effective date then select "Next Page."

15 14. The user will choose "Neither Authorized nor a Delegated Official" for the Managing Employee and select "Next Page." The Telephone field will be grayed out. The user is not required to enter phone number.

16 15. The user will indicate if the individual has ever had a Final Adverse Action.

17 16. If the user has had a Final Adverse Action, they will enter the information and select "Save." 17. The user will then go back to the Error/Warning Check. The Individual with Managing Control topic error should be removed. If the Individuals with Managing Control topic is listed as an error in the Error/Warning Check, be sure to note the exact error message and troubleshoot using the section where the user is receiving the error message.
