1 Paylocity Web Benefits Enrollment Guide 2016

2 Access to Online Enrollment Please log into your Web Pay payroll portal. You can gain access to Web Benefits to elect your benefits by hovering your mouse over Web Pay (top left corner) in the Self-Service Portal and selecting Enterprise Web Benefits. You will have the ability to log straight into the Web Benefits site via Web Pay, using Single Sign On: Once you enter your Web Pay profile, you can access Web Benefits at the top of the screen by selecting: Web Pay >> Enterprise Web Benefits You can also go directly to our online enrollment site using the web address Please note: For successful navigation of the site, do NOT use the back button in your internet browser, as this will automatically log you out of the site. To navigate through the site, use the navigation bar located on the left hand side of the screen. 1 Page

3 From the Home Page, click on ENROLL NOW. The enrollment process consists of the following 4 steps/tabs. You will be taken through each tab to make changes or confirm your information on file and choose your benefits for the new plan year. 1. Employee (Personal Information) 2. Family (Family Information) 3. Enroll 4. Confirm 2 Page

4 Verify your Personal Information Before beginning your enrollment, please verify the accuracy of all of your personal information (e.g., address, DOB, etc.) If you need to make any changes, you can do so directly on the site. Verify that all information is accurate. When done, check I agree at the bottom of the page and click Continue." Please Note: Any field that has an asterisk is required. 3 Page

5 Verify your Family Information Please be sure to add all dependents that may be missing from the Family Information section before proceeding to the next section. To do this, click on Add Dependents. When all of your family information is accurate, check I agree and click Continue. 4 Page

6 Making Benefit Elections Medical, Dental, Vision Plans, etc. In this section, you can compare costs across all available plans and tiers, compare plan details side by side by clicking on the Compare Plans link above Plan Selection, and review details of a specific plan by clicking on the View plan details link. Upon making your plan selection, you will be able to waive or cover eligible dependents on file by selecting the Cover or Waive radio button next to each dependent s name. If you wish to add a new dependent at this time, click on the Add Dependent button to be taken directly to the family tab to add the dependent. If you are satisfied with your selection, click Next. 5 Page

7 Upon completing your benefit election, the box will turn green and it will say completed. Click Save and Continue Enrollment to be taken to the next benefit option. Please complete each benefit option that you would like to enroll in. Once you have completed all the elections, select Continue. Basic Life Plans You do not need to make an election for the employer provided plans. However, on the proceeding page you will be asked to designate your beneficiaries. Your dependents on file will automatically be listed as beneficiaries. Enter your assignments to total 100%. If you would like to add a beneficiary, click the Add Beneficiary button. When you are satisfied with your choices, please click Save and Continue. Voluntary Life To elect the Voluntary Employee Plan, select the button next to the plan and choose an amount from the dropdown. 6 Page

8 Short Term Disability & Long Term Disability To elect Short Term Disability and Long Term Disability, select the radio button next to the plan and choose an amount from the dropdown and then click Save & Continue. 7 Page

9 Beneficiaries Once all plans have been selected, you will be prompted to select beneficiaries. You can elect primary and secondary beneficiaries; each must total up to 100%. You can also use the Add Beneficiary link near the bottom of the page to add any beneficiaries who are not currently listed. 8 Page

10 Almost Finished! You will now be on the final review page. Review all of your benefit elections and covered dependents. Once you've completed your review, check the I agree, and I m finished with my enrollment box at the bottom of the page. Then click the "Save My Enrollment" button at the bottom of this page. Note: Once you select and finalize your enrollment options, the benefit elections must be approved by HR. 9 Page

11 Deadline & Confirmation You can send yourself an confirmation of your elections or print it for your records. Click the printer icon on the right hand side of the screen. Please Note: Although the online benefits enrollment site is a secure site, and your information is encrypted, it is important that you log off when you have completed your session. Click the Log Off icon in the upper right-hand corner of the enrollment site to log off. For security purposes, the system will automatically logout if you leave your system idle for more than 30 minutes. 10 Page
