Typographical conventions: Bold Italic Monospace Identifies options, keywords, and items in a description. Indicates variables, new terms and concepts, foreign words, or emphasis. Identifies examples of specific data values, and messages from the system, or information that you should actually type. Icons are used to highlight new features and indicate enhanced features and item keys: Icon Indicates an item key. Description Identifies new features, suggested by clients, from the ecwideas website: Identifies new features. Indicates an enhanced feature. Points out helpful tips or additional information. Indicates feature meets a Meaningful Use requirement. 6 The Meaningful Use, Adoption, Quality (MAQ) Dashboard is a tool to measure provider and practice performance on meaningful adoption of EHR, population health trends, and chronic disease/preventive care management. The MAQ Dashboard is designed to help practices report on Meaningful Use Objectives and Measures. The benefits of using the MAQ Dashboard are: Identify provider and practice pattern around EMR usage Track changes in clinical charting patterns over time Side-by-side provider and practice comparison of MU (Meaningful Use) measures Compare provider and practice performance against community, state, and national standards Build automation to proactively inform the providers if they do not meet the thresholds set for Meaningful Use or at a community level Experience efficiency gains and improved quality of care Dashboards help physicians see improved quality through data Benchmark against other providers/practices regional and national comparisons based on eclinicalworks providers Can use in payer fee schedule negotiations by showing improvement in quality of care based on actual/real data Population health trends Copyright eclinicalworks, July MAQ Dashboard Guide 6
7 REFERENCE DOCUMENTS FOR MAQ DASHBOARD This level of data cannot be analyzed when using paper records; it can only be achieved through true EHR adoption and meaningful usage This method will be the only way to view MU data for reporting to CMS Patient-level drill-down capability will be available from the Dashboard Reference Documents for MAQ Dashboard This document provides information on how to use the MAQ Dashboard. Additional documents on Meaningful Use and the MAQ Dashboard are present in the Customer Portal and some of them can also be accessed through the MAQ Dashboard Help options. Please refer to the additional documentation to understand Meaningful Use Objectives, Measures, and reporting criteria on the MAQ Dashboard: A detailed description of Core Set Objectives (including Clinical Quality Measures) and Menu Set Objectives can be found in the Meaningful Use Training Scenarios Guide. Calculations used by eclinicalworks software for Meaningful Use Reporting on the MAQ Dashboard can be found in the eclinicalworks Meaningful Use Calculations Guide. Common questions concerning the eclinicalworks MAQ Dashboard can be found in the MAQ Dashboard Frequently Asked Questions. A list of available resources and the most common questions concerning the Meaningful Use incentive program and eclinicalworks can be found in Meaningful Use Resources And Frequently Asked Questions. For additional documentation on Meaningful Use, refer to the Knowledge > Meaningful Use tab in Customer Portal. Access MAQ Dashboard The MAQ Dashboard can be accessed by providers and ecw practice administrators. Staff members who are not ecw practice administrators cannot access the MAQ Dashboard. To access the MAQ Dashboard: 1. 8 2. Read the terms and conditions carefully and then click I Agree to proceed to MAQ Dashboard. The MAQ Dashboard displays in Provider View or Practice View depending on whether the logged-in user is a provider or ecw practice administrator. Note: Once the user is logged into ecw, they do not need to log in again to access the MAQ Dashboard. Note: Microsoft Silverlight must be downloaded on each machine that will be used to access the MAQ Dashboard. Practice View When an ecw practice administrator logs into eclinicalworks and clicks the Meaningful Use link in the main toolbar, the MAQ Dashboard window is launched in Practice View, displaying the Welcome page with an overview of Meaningful Use for 2014: Copyright eclinicalworks, July MAQ Dashboard Guide 8
9 The Select Provider option is available only if the logged-in user is an administrator of the software. The Meaningful Use and the Quality tabs under Stage 1 and Stage 2 are available for the ecw practice administrator and provider for viewing. To display Meaningful Use Summary for measures in Practice View: 1. Configure a Meaningful Use stage and Clinical Quality Measures for extraction. For more information, refer to MAQ Measure Configuration. 2. Select providers. For more information, refer to Selecting Providers. 3. Select a view. For more information, refer to Selecting the Viewing Period. 4. Select a stage and a measure. For more information, refer to Selecting the Measure Type, Stage 1 Tab, and Stage 2 Tab. MAQ Measure Configuration Configure Meaningful Use stage and select Clinical Quality Measures for extraction using the MAQ Measure Configuration tool. The MAQ Measure Configuration tool is available under Score Card View and Historic View for Clinical Quality Measures. The view is same for both providers and ecw practice administrators. Currently there are no security settings that can restrict users from accessing this tool. 10 Configuring the Meaningful Use Stage To enable automatic extractions for 2014, configure Meaningful Use stage preferences from the Configuration tool on MAQ Dashboard. Select the stage that you will be attesting for this year. If you are a practice administrator, set up the preferences for all providers. This will enable viewing of the appropriate Meaningful Use Stage 1 or 2 data on the MAQ Dashboard based on your participation. To select a Meaningful Use stage: 1. From the Select View drop-down list, click Score Card View or Historic View. 2. Click the Settings icon: The MAQ Provider Measure Configuration window displays. The Practice Settings tab displays the following measure configuration - Include Outgoing Referrals sent to providers within the same eclinicalworks database for the denominator calculations of the Summary of Care measure (Stage 2: Core-15) and the Transition of Care Summary measure (Stage 1: OBJ-304I). This measure configuration is disabled by default. To enable the measure configuration, click Yes. 3. Click the Provider Settings tab. 4. Enter the name, facility, speciality, stage, or program in the Search field, and then click Go. Providers that match the search criteria display. For example, if the user enters a facility name in the Search field, all providers that have the same facility display: 5. Select one or more providers, and click Edit Settings. The Select Stage window displays. 6. Click from one of the following options: Stage 1 ( ) - To select Meaningful Use Stage 1 data from Stage 1 (2014) - To select Meaningful Use Stage 1 data from 2014 Copyright eclinicalworks, July MAQ Dashboard Guide 10
11 Stage 2 (2014) - To select Meaningful Use Stage 2 data from Select a program the providers are attesting for - Medicare or Medicaid. Note: Selecting the program is for ecw reference purposes only. 8. Click Save. A confirmation message displays. The stage name displays in the Stage column for providers that have been configured for that stage. Note: Automatic extractions cannot be performed for providers that have not configured a Meaningful Use stage preference. Configuring Clinical Quality Measures for Extraction The MAQ Measure Configuration Setting is also used to request specific Clinical Quality Measures for one or more providers. Once the specific measures are selected, users can wait for the monthly automatic extractions or use the On-Demand Data Request Tool to extract results for the selected measures. To set measure preferences for the 64 clinical quality measures for 2014, practices can use the Configuration Tool, and select ecqm 2014 option. 20 measures can be selected per provider for extraction. Each provider has to attest for only 9 quality measures. Note: This functionality is available only through a Product Hub patch. For more information on getting this patch, contact eclinicalworks Support. For more information on the On-Demand Data Request tool, refer to MAQ On-Demand Data Request Tool. To select specific Clinical Quality Measures for extraction: 1. From the Select View drop-down list, click Score Card View or Historic View. 2. Select one or more provider(s), and then click CQM ( ) or ecqm (2014): CQM ( ) - Users that are still attesting for 2013 can use this option: The three Core Clinical Quality Measures and the three Alternative Core Clinical Quality Measures are selected by default. The Measure Dictionary field displays CQM Select up to six Additional Clinical Quality Measures, and then click Save. The Clinical Quality Measures configuration is saved. ecqm (2014) - There are 64 clinical quality measures for 2014 that are divided into six different categories, i.e., National Quality Strategy Domains. IMPORTANT! A Meaningful Use stage must be configured for the provider(s) before selecting the measures. If the stage is not configured, the Measure Selection window does not open for the provider. 13 Users must select at least nine and up to a maximum of 20 of the approved 64 measures. The measures that are selected must fall under at least three of the six measure categories (six National Quality Strategy Domain): Note: A set of nine measures will be extracted by default if users have not selected the measures. The nine default measures are - CMS 138, CMS 90, CMS 69, CMS 68, CMS 156, CMS 166, CMS 165, CMS 2, CMS 50. The Measure Dictionary field displays ecqm Select the nine measures, and then click Save. If a minimum of nine measures is not selected, and the user clicks Save, a message displays - A minimum of 9 measures must be selected. Note: If the logged-in user is an ecw practice administrator, they can request extraction for one or all providers in the list. If the logged-in user is a provider, they will see only their name in the provider list and can request extraction for themselves. To view the measures saved for each provider, click the More (...) button next to a provider. The Measure Configuration Detail for the provider displays. Selecting Providers Select providers to view Meaningful Use data comparison summaries between providers and the entire practice. The number of providers that can be selected for a measure varies by the view: In Score Card view, up to 225 providers can be selected. In Historic view, only one provider can be selected. In Monthly, Calendar Year, 90-Day, YTD view, Quarter, QTD, and Yearly View, up to five providers can be selected. Copyright eclinicalworks, July MAQ Dashboard Guide 13
14 To select providers: 1. Click Select Provider to open the Provider Lookup window. The Provider Lookup window displays: The Facility and Specialty columns display the billing facility and the specialty associated with the provider respectively, in the Provider Setup. The Provider Type column displays whether the provider is a Supervising Provider. The Supervising Provider information is captured from the Claim Providers section in the Appointment window. The NPI column displays the provider s NPI number. To sort the results by facility name, provider name, specialty name, NPI, or provider type, click the respective column headers. 2. Check the box(es) next to provider name(s) to select the provider(s), and then click OK. 3. (Optional) To filter by provider, enter the name or the first few letters of the provider name in the Search box. Alternatively, to filter by facility, provider, specialty, NPI, or provider type, click the funnel icon column header to open an additional filter window for that column: in the Copyright eclinicalworks, July MAQ Dashboard Guide 14
15 Check the boxes to select only those values. OR a. Select a value from the Is equal to drop-down list. b. Enter a corresponding value in the following field. c. Select a value in the And drop-down list, and the second Is equal to drop-down list, to set an additional filter. d. Enter a corresponding value in the following field. To clear values in the filter, click Clear Filter. e. Click Filter. The results display. 4. Click OK. Note: When switching views, such as from Monthly to 90-Day, the provider selection is retained. Note: The Select Provider option is available only if the logged-in user is an administrator of the software. Selecting the Measure Type Select between Core, Menu, or Clinical Quality Measures from the Meaningful Use tab or the Clinical Quality Measure tab respectively. Stage 1 Tab Stage 1 tab displays the Meaningful Use Stage 1 measures, that are divided under the Meaningful Use tab and the Quality tab. For more information on different measures available for viewing under the Stage 1 tab, refer to: Stage 1 Meaningful Use Tab Stage 1 Quality Tab Note: Automatic extractions cannot be performed for providers that have not configured a Meaningful Use stage preference. Stage 1 Meaningful Use Tab The Meaningful Use tab contains two options - Core Measures and Menu Measures. To select a measure: 1. Click the arrow in the Meaningful Use tab to display the options: 2. Click on Core or Menu to view the summary. Stage 1 Core Measures The Core Measures tab displays the comparison between the provider and the practice s Meaningful Use of Core Objective Measures in graphs. The comparison summaries can be viewed in eight different formats: Monthly, Calendar Year, 90-Day, Quarter, QTD (Quarter to Date), YTD (Year to Date), Yearly, Score Card, and Historic. 17 The Core Objective Measures for which the summaries are displayed are: Computerized Provider Order Entry (CPOE) Maintain an up-to-date problem list of current and active diagnosis Generate and transmit permissible prescriptions electronically (erx) Maintain active medication list Maintain active medication allergy list Record of demographics Record and chart changes in the vital signs Record smoking status for patients 13 years old or older Patient Electronic Access (Timely) Provide clinical summaries for patients for each visit Note: Three asterisks (***) displayed next to a measure indicates that the measure is not eligible for MU attestation. Stage 1 Menu Measures The Menu Measures tab displays the comparison between the provider(s) and the practice s Meaningful Use of Menu Objective Measures in graphs. The comparison summaries can be viewed in eight different formats: Monthly, Calendar Year, 90-Day, QTD (Quarter to Date), Quarter, YTD (Year to Date), Yearly, Score Card, and Historic. Only five Menu Objective Measures are available for viewing because CMS has defined thresholds for only these five Menu Objective Measures. The five Menu Objective Measures for which the summaries are displayed are: Clinical lab test result Patient reminders Patient-specific education resources Medication reconciliation Summary care record Note: Three asterisks (***) displayed next to a measure indicates that the measure is not eligible for MU attestation. Stage 1 Quality Tab The Quality tab displays the comparison between the provider(s) and the practice s Meaningful Use of Clinical Quality Measures. 18 CQMs - This tab displays Clinical Quality Measures from under the Core tab. Users that are still attesting for 2013 can use this option. The Clinical Quality Measures consists of 44 measures defined by CMS. Of these, only the measures requested by the provider/practice is extracted and displayed. CMS has categorized these measures as Core Measures, Alternative Core Measures, and Additional Measures. No thresholds have been defined for Clinical Quality Measures. Currently, only the Core tab is viewable ecqms - There are 64 clinical quality measures for 2014 that are divided into six different categories, i.e., six National Quality Strategy Domain - Patient and Family Engagement, Patient Safety, Care Coordination, Population/Public Health, Efficient Use of Healthcare Resources, and Clinical Process/ Effectiveness. The measures that are selected for the providers through MAQ Configuration tool will display across these six categories. There are 17 Core Objective measures, of which 13 are displayed for viewing. The rest of the measures do not require calculations and can be self-attested by users. The Core Objective Measures for which the summaries are displayed are: Using CPOE for Medication, Laboratory, and Radiology Orders Generating and Transmitting e-prescriptions Recording Demographics Recording Vital Signs Recording Smoking Status Patient Electronic Access Providing Clinical Summaries Incorporating Lab Test Results as Structured Data Preventive Care Patient-Specific Education Resources Medication Reconciliation Summary of Care Use Secure Electronic Messaging Stage 2 Menu Measures The Menu Measures tab displays the comparison between the provider(s) and the practice s Meaningful Use of Menu Objective Measures in graphs. The comparison summaries can be viewed in eight different formats: Monthly, Calendar Year, 90-Day, Quarter, QTD (Quarter to Date), YTD (Year to Date), Yearly, Score Card, and Historic. Three of the six Menu Objective measures that need calculations are displayed for viewing. Rest of the measures do not need calculations and can be self-attested by users. 21 Monthly View The graphs displayed in this view are side-by-side comparisons of the providers and the entire practice s Meaningful Use of the selected measure type for the current month, against the threshold set by CMS. No thresholds have been defined for Clinical Quality Measures. Note: Practice data includes an average of all results for all providers in your database. To display data in Monthly View: 1. Click a measure from the Meaningful Use tab or the Quality tab for the stage that has been configured for extraction. 2. Click Select View in the upper-right corner of the window and then click Monthly View: The Meaningful Use comparison summary displays for the current month. 23 Clinical Quality Measures - Monthly View: The entire practice s Meaningful Use of the selected measure type is displayed in red on the graph, and the providers Meaningful Use is displayed in various contrasting colors. Note: In any view, click the small black icon located on the upper-right corner of a graph box to view the graph in a larger window. Use the scroll bar to move down through each of the measures to view comparison summary for that section. The percentage for the Meaningful Use of the selected measure type achieved by the practice and the provider(s) is displayed on top of their corresponding bars. The percentages can also be viewed by hovering the mouse over each individual bar. Note: If the denominators for some of the measures are zero (0), then the graphs for those measures do not display. 37 Provider Summary Breakdown - To view the performance, numerator, and denominator summaries for the provider(s), click the Plus (+) icon next to a measure in the table. Note: Graphs have been disabled for the Score Card View. The provider names display along with their individual numerator, denominator, threshold, and performance percentage breakdown for the selected period: The Plus (+) icon does not display if the logged-in user is not the ecw practice administrator. In this case, providers can view their individual numerator and denominator information directly on the window. Provider Exclusion Column - The Provider Exclusion column displays patients that meet certain exclusion criteria and have been manually marked as excluded from the Progress Notes Chart Panel to prevent their inclusion from the measure calculation. The exclusions are displayed for both the selected periods. Note: This column only applies to Clinical Quality Measures Patient-Level Breakdown - To view patient-level breakdown of a provider s performance summary, click the Plus (+) icon next to a measure. Then, double-click in the Provider Period A or Provider Period B Threshold fields. Note: In a Provider View, simply double-click in the Provider Period A or Provider Period B Threshold fields to view the patient-level breakdown. The patient list for the selected provider s performance summary opens, displaying the account number for the patients and if the patients satisfy the numerator and denominator for the measure: If a patient satisfies the denominator and numerator of the measure, they are captured in the list and are denoted by Yes in the Denominator and Numerator columns respectively. 38 To export the entire patient list to Microsoft Excel and to save it to your computer, click Save (All) as CSV. To export the patient list displayed in the current window to Microsoft Excel and to save it to your computer, click Save (Page) as CSV. To filter the values in any columns, click the funnel icon in the column header to open an additional filter window for that column. For more information on using these filters, refer to Selecting Providers. Note: There are only 10 Core and 6 Menu Set measures because these are the only measures that require thresholds to be met and calculations to be made. The remaining measures are reported through self-attestation (Yes/No from the provider). Click the Disk icon and then click Save as CSV to save the report in a CSV format on your machine: For more information on the green folder icon with an arrow refer to MAQ On-Demand Data Request Tool. Note: When there is no data available for a measure, check the date range you are viewing. Some features required for Meaningful Use reporting (such as tracking printed and faxed prescriptions, sending reminders, viewing PHR, etc.) were added after the V10 upgrade, therefore date ranges prior to the upgrade will not display information. If you are viewing a date range after your upgrade, please attend our Meaningful Use webinars, which contain useful reporting tips. If the issue still exists after this point, please contact eclinicalworks Support. Historic View In Historic View, the user can select a Monthly, 90-Day, Quarterly, QTD, YTD, or Yearly viewing period from a past set of scheduled or manual extraction periods. Two different time periods, such as Monthly and YTD can be compared to one another, but two similar periods, such as Monthly and Monthly cannot be compared. When an extraction is requested or a scheduled extraction is performed, the period of extraction is added to the Historic View (Select Any One Period window). To display data in Historic View: 1. Click Select Provider button to open the Provider Lookup window. The Provider Lookup window displays. 2. Select only one provider. Note: The Select Provider option is available only if the logged-in user is an administrator of the software. In Historic View, only one provider can be selected at a time. 3. Click OK. 4. MAQ On-Demand Data Request Tool The On-Demand Data Request Tool is used to request a monthly, 90-Day, year-to-date, quarter-to-date, quarterly, and yearly data extraction for Meaningful Use of Objective and Clinical Quality Measures. Only past or current data can be extracted; future dates cannot be requested for extraction. This tool is available under Score Card View and Historic View for Core Objective, Menu Objective, and Clinical Quality Measures for Meaningful Use Stage 1 and Stage 2. The view is same for both providers and ecw practice administrators. Currently there are no security settings that can restrict users from accessing this tool. Generally, the extraction is performed overnight after submission of request, and the results can be viewed in the Score Card View. 42 MAQ ON-DEMAND DATA REQUEST TOOL Any dates that are requested for extraction, become part of the Historic View Dates (Select Any One Period window). Dates that are not in the current time period can also be viewed in the Historic View. If a user requested an extraction, everyone else in the practice with access to the MAQ Dashboard can view that extraction period in Historic View (Select Any One Period window). However, providers cannot view results for other providers; only ecw practice administrators can view results for other providers. For more information on MAQ On-Demand Data Request Tool, refer to the following section: Requesting Extraction of Data for Core, Menu Objective, and Clinical Quality Measures IMPORTANT! Only one extraction request can be submitted at a time; users must wait until an extraction is complete to request another extraction. For example, if Dr. Sam Willis has submitted a request for extraction, the ecw practice administrator, Jennifer Davis, cannot submit a request for Dr. Sam Willis or other providers at the same time. Jennifer Davis must wait until Dr. Sam Willis request has been processed before she can submit another request. Note: If the data extraction coincides with the latest date ranges on the regular view, then the data will display on the regular Monthly, 90-Day, Quarter-To-Date, Quarterly, YTD, or Yearly views (whichever was requested). But if the data is requested for a past time frame, then that data will be available in the Historic View. Requesting Extraction of Data for Core, Menu Objective, and Clinical Quality Measures Data extraction for all available Core, Menu Objective Measures, and Clinical Quality Measures can be requested at one time. To request data extraction by 90-Day, Monthly, YTD, QTD, Quarterly, or Yearly, only one extraction (Objective or Quality, or both) and one Meaningful Use stage can be submitted at one time. To request data extraction for Core, Menu Objective, and Clinical Quality Measures: 1. From the Select View drop-down list, click Score Card View or Historic View. 2. Click the black arrow button:. Click the Prev and Next buttons to browse to previous and next pages respectively. Click the Display number buttons to display a selected number of results per page. 3. Depending upon the number of providers for whom the extraction is requested, the extraction can take 24 hours or more to be completed. Users can view their results in Score Card View after the extraction is complete. Note: At this point, the name of the user that requested the extraction does not display. If an extraction request is submitted in error, users do not have to wait 24 hours to submit another request. They have the option to retire an existing request.
