1 The CHECKBOX Quick Start Guide This guide will provide step-by-step directions in order to help you get started faster with Checkbox. First, Some Basic Concepts The CHECKBOX Survey Lifecycle Create Edit your new survey, adding items and logic; stylizing the survey with images and colors. Publish Test the survey internally, then activate the survey, and invite survey respondents and users. Analyze Viewing the raw results live, viewing Checkbox reports, and exporting to other formats. An Overview of Management Tools available in Checkbox When you first login to the system, you will see some of the following options depending on which type of trial you are using (software, private demo, and public demo). Survey Manager Style Manager Library Manager Report Manager User Manager Invitation Manager Application Settings Online Help Logout Allows you to create, edit, and save surveys from an XML file, or from a blank survey. Lets you change the look and feel of your surveys. Change the background color; add a logo, header or footer, and more. Then apply the style to surveys. Allows you to save and borrow from surveys and questions that you use frequently. Lets you view responses, create reports and filter results, and export data out of the system. Manage user groups and lists (new user creation disabled in public demo) Allows you to create invitations to individuals and groups (disabled in public demo) Allows you to make changes to how the survey platform works at an overall level. Help system lets you get started faster by walking you through basic and advanced tasks. Logs you out of your current session. The next pages of this document will walk you through the creation, publishing, and analysis of your first public survey.

2 Getting Started - How to Create a Simple Survey with Checkbox Here you will start a new survey, name it, and then add some basic survey items. 1. Login to Checkbox. 2. Select the Surveys button from the application toolbar. 3. Select the New Survey button. 4. Enter a name and description for your survey. Hit Submit. 5. You should see a screen similar to the picture at right. 6. Click Add Item. 7. You should see the Add Item to Survey screen. 8. Using the Item Preview feature, view some of the options. 9. Choose a Open-Ended Single-Line Text question. 10. Fill out using the following fields: Question Text / Subtext - Question Text: What is your name? - Question Sub Text: Please provide your full name - Alias: leave default Options - Default Text: leave default - Answer Format: None - Max Length: 40 - Answered is required: Yes Appearance - Width: leave default - Item Position: leave default - Label Position: leave default 11. Hit Submit. It should look like the following: 12. Now add a few more Question and Display items by clicking Add Item again: Question Items CAPTCHA Checkboxes Drop Down List Matrix Open Ended Multi-Line Text Radio Buttons Rating Scale Open Ended Single-Line Text Description Uses a picture to verify spam or not A multiple choice question Choose Many A list of choices in a box A grid of answerable questions Large box for text comments A single choice question Choose One Radio buttons with numbered options Small open ended box for text comments Display Items Horizontal Line HTML Image Message Description Horizontal Line used to break up parts of a page. Displays HTML code. Display an image. Displays a message to the respondent (text and HTML)

3 Publishing a Simple Survey Now that you ve added questions to this survey, you want people to fill it out. But before sending it out, you want to first test and then activate your survey. 13. Test the survey when you have the survey you ve created open in the main window. Click the Test Survey Button. *The survey will look fairly plain, because a style has not been added. You may wish to navigate to the style menu and select a style and then click Submit at the bottom of the page to confirm your choice. 14. This will bring you to a page and a URL that can be clicked on so that you can test the functionality and look and feel of the survey. The URL (link) will look something like this: Before sending out a large survey to thousands of respondents, make sure you send a test to a group of internal employees so that spelling errors and grammatical mistakes are scrutinized by the whole team. 16. Activate the survey after testing. This will make your survey accessible to people who have the Survey URL. (Based on the permissions, which will be discussed later) Simply click the Activate button, and then either use our invitation manager or copy and paste the Survey URL to send it out on your own. 17. Now, you can invite respondents in one of many ways. - Send the actual link in an - Embedded in HTML on a webpage: Please Take Our Survey (use example code on next line) <a href=" Take Our Survey</a> - Invitation Management interface within Checkbox (not covered in this guide).

4 Analyzing a Simple Survey By now, you ve designed a survey and sent it out to respondents. Next, you want to be able see the results and be able to interpret the data. There are really three ways to view data: Look at the raw responses Create reports within Checkbox Export the data out of Checkbox and view data using a spreadsheet or other tools. Let s take a look at each option one by one. How to View Raw Responses 18. Click Reports 19. Click on the button which will bring you to the Responses section. 20. The responses page will allow you to view: Response ID User Name Completion Timestamp IP Address of Respondent View You can also edit the following: Edit Respondent Answers Edit Delete Selected Responses Delete All Responses *Deleting responses is good for when you rollout a survey in multiple phases (after you ve saved off the first round of results). Also it can be used to start fresh, after the testing phase of a survey.

5 How to Create Reports within Checkbox 21. Click Reports 22. Click on the Reports button 23. Click Autogenerate which will create a report for you. A new report will appear below. 24. To the right of the survey name, click on the button that says Run. There are many other things you can do with reports that we will not cover here, but you can: Edit the report and then add additional items to it. Edit and autogenerate filters Run cross tab analysis See the online help for more details. How to Export Data out of Checkbox 25. Click Reports 26. Click on the Export button 27. Choose your Options, experiment with the setting to get what you need: Output Options - CSV - SPSS CSV Options - Detailed Response Info - Detailed User Info - Merge Checkbox Results - Export Open-Ended Results - Export With Aliases - Export Hidden Items SPSS Options - View SPSS Key Output Encoding - ASCII - Unicode - UTF-8 - UTF-7

6 Some Things to Try Adding a Style to a Survey Now that you ve completed a survey, you can stylize your work so that the survey has a standard look and feel and save it as a style sheet which can be shared with other surveys in or beyond your organization. Click on the Styles button Click on the New Style button. Give the style a name. Hit Submit. This will bring you to the header/footer. You can start typing text, or start coding in HTML. What most people do is add a graphic to the top (Company Logo) and some descriptive text at the botton. To upload an image, click Upload Image. Then you can browse to your local computer and choose the image you d like to add into the survey header. Click the Add Image to Header option if you want to add the image to the top of all survey pages. Click the Add Image to Footer option if you want to add the image to the top of all survey pages. Things you can also modify using Style Sheets: o o o Base Styles - Base Font Type, Size, Color, - Title Font, Page Number, Error Message styles - Background of Page, Background Image Item Styles - Question/Subtext/Answer Font Type / Size / Color - Background Colors - Matrix Styles (Header/Subheading, Rows, Fonts, Borders) - Matrix Font Type / Size / Color - Matrix Border Width and Color Additional Styles (Optional) - Progress Bar Color, Size, Alignment - Button Styles Font Type, Color, Size - Item Spacing When you complete the style sheet, make sure that you select the style from the survey editor and choose the style from the drop down list.

7 Applying Survey Security Checkbox offers the ability to secure access to the surveys you are creating. Surveys will either be public or private (restricted in some way). By default, all surveys are public. To change these options you must first select a survey and then under the survey title, Permissions. Conclusion Public any respondent who can access the link will be able to take surveys Password Protected respondent must be a password before taking survey Access List a one-time list (CSV file) of addresses and sometimes first names All Registered Users only users that have been imported into the system can take We ve spent a long time building our application to allow survey designers to be able to use our application without requiring handholding or technical knowledge.
