1 ImagineCare Offering for D-H Employees Frequently Asked Questions What is the cost to sign-up for ImagineCare? ImagineCare is offered as an employee wellness benefit, however the IRS deems this a taxable benefit and employees are taxed based on the Fair Market Value of the band. Due to state tax differences, estimated tax for VT residents will be approximately $72 and estimated tax for NH residents is $64. What is ImagineCare? ImagineCare is an innovative digital health tool and service platform developed here at Dartmouth-Hitchcock. It offers personalized remote health monitoring and health coaching, with 24/7 access to a dedicated ImagineCare health care team. By connecting your health monitoring sensors provided through ImagineCare to your mobile device, a team of health navigators and nurses are able to receive data and provide support for activity, sleep, mood, hypertension, diabetes, congestive heart failure, or chronic obstructive pulmonary disease. You will have the insight and support you need to achieve whatever health goals you have. You only get the support on the things you choose. ImagineCare is an addition to our employee health and wellness programs. Dartmouth- Hitchcock offers two ImagineCare programs: The ImagineCare Wellness Program, which is for benefits-eligible employees to help track activity (such as exercise and sleep), and ImagineCare Plus Chronic Disease Management Program, which helps employees and their dependents on the DH health plan with at least one of four chronic conditions chronic obstructive pulmonary disease, congestive heart failure, diabetes, and hypertension manage those conditions in collaboration with ImagineCare staff, Care Managers at Dartmouth-Hitchcock Wellness Plus (a component of Dartmouth- Hitchcock s employer-sponsored health plan), and the employee s (or dependent s) designated health care provider. Is ImagineCare part of Live Well Work Well? ImagineCare works in close collaboration with the Live Well Work Well team of primary care providers and health coaches to help you manage your health and well-being and promote a healthy and active lifestyle. However, ImagineCare is not a part of Live Well Work Well. How does ImagineCare work? ImagineCare works by collecting personal data received from electronic devices you are sent and connect to the app. The data is then processed through evidence-based care pathways, created in conjunction with the D-H Knowledge Map team. These pathways combine personal data with other relevant data sets and provide feedback and alerts to both the patient/customer user and the care team in the ImagineCare command center, who monitor and respond to these alerts.
2 What is the ImagineCare command center? The ImagineCare command center is staffed 24/7 by specially trained Dartmouth- Hitchcock nurses and non-clinical staff called health navigators, who monitor personal data collected remotely, watch for "red flags" which may indicate a worsening of a chronic condition or an emergency, and respond as needed to the patient/customer. For example, ImagineCare nurse may reach out to a patient/customer, with a text reminder or a phone to discuss the need to seek care from the patient/customer s provider. What do I get as a part of ImagineCare? When you enroll in ImagineCare, you get an activity tracker and the ImagineCare app (currently available for the iphone, ipad or other ios device). Additionally, if you are enrolled in the ImagineCare Plus Chronic Disease Management Program, you may receive other remote sensors, such as a blood pressure cuff, weight scale, or inhaler use sensor, to help monitor your chronic condition. Who at Dartmouth-Hitchcock will have access to my ImagineCare information? A distinctive feature of ImagineCare is its chronic disease management pathways, which aim to help individuals with certain chronic diseases achieve better health outcomes. The chronic diseases that ImagineCare helps to manage are the following: hypertension, congestive heart failure, diabetes and chronic obstructive pulmonary disease. If you have one of these chronic diseases, you may choose to enroll in the ImagineCare Plus Chronic Disease Management Program. In this program, you will work with ImagineCare health coaches and the Dartmouth-Hitchcock Wellness Plus ( DHWP ) Care Managers, working on behalf of the Dartmouth-Hitchcock employer-sponsored health plan. When you enroll, you will be asked to agree that, if you have a Primary Care Provider at D-H, your ImagineCare data will be included in your D-H Electronic Medical Record ( ed-h ), where it will be available to the DHWP Care Managers (on behalf of the D-H employer-sponsored health plan), as well as to your D-H health care team, to provide more effective chronic disease management all with the goal of helping you achieve better health outcomes. If you do not have a Primary Care Provider at D-H, you may still enroll in the ImagineCare Plus Chronic Disease Management Program, and you will be asked to agree that your ImagineCare information will be shared with the DHWP Care Managers. If you do not have one of the chronic diseases specified above, you may enroll in the ImagineCare Wellness Program to help you with your personal wellness goals. You will be asked to agree that ImagineCare may share your information with Dartmouth Hitchcock Live Well/ Work Well Health coaches for the purpose of identifying you as a candidate for health coaching (for example, nutritional coaching) through Dartmouth- Hitchcock s Live Well/ Work Well program. If you are contacted by D-H s Live Well /Work Well program regarding a health coaching opportunity, you may elect not to participate.
3 For more information on how information may be used or disclosed, see the ImagineCare privacy policy, and the additional terms for participation in ImagineCare as an employee of Dartmouth-Hitchcock. What kinds of devices support the ImagineCare app? Currently, the ImagineCare app works only on ios devices, such as the iphone, ipad, or ipod. An Android-compatible app is under development and will be available later this year. Can I still get the activity tracker if I don t have a smartphone device? You can still get the activity tracker and enjoy some of the fitness features such as measuring steps, calories burned, heart rate measurement, basic running, exercise, and sleep summaries. Other functionality also works independent of a smartphone, including the stopwatch, timers and alarms. To get the full activity band experience and the full ImagineCare experience you must pair the device with a smartphone. Without a smartphone you will not be able to use the text, or ImagineCare application functions such as feedback and alerts. What happens after I get the Activity Tracker? If you have an ios device, once you receive the activity tracker (and other electronic devices for chronic disease management, if applicable), you can access the ImagineCare app through your ios device and follow the instructions to pair the devices with the app. Who is eligible to sign-up for ImagineCare? ImagineCare Plus Chronic Disease Management Program is open to all benefits-eligible Dartmouth-Hitchcock employees (meaning employees of Dartmouth-Hitchcock Clinic and Mary Hitchcock Memorial Hospital) and their adult dependents who participate in the D-H insurance plan and who have one of the four chronic conditions supported by ImagineCare. At this time, the ImagineCare Wellness Program is open only to benefits-eligible employees, and will not be extended to dependents. What is happening with the previous Wellness Rewards reimbursement program? D-H s goal is to offer the best possible support to our employees to help them on their health and wellbeing journey. While currently we are not offering a cash reimbursement program, as in previous years, we continue to evaluate creative wellness rewards programs, with the goal of shifting our focus to be more aligned with our organization s mission of improving the lives of the patients and communities (including our employees) we serve. We believe ImagineCare is an effective and exciting tool to help our employees achieve their wellness goals.
4 What if I ve already purchased a FitBit or other health tracking device? Will I still be able to use ImagineCare? ImagineCare does not interface with FitBit, and works best if you connect using the activity tracker provided to you when you sign up for ImagineCare. You may be able to use your FitBit or other device through additional health and wellness programs that are offered through the Live Well/Work Well employee wellness program. Do I have to give the activity tracker and other remote medical sensing devices back to D-H if I decide not to continue with ImagineCare or if I leave the organization? No, all devices that you receive through ImagineCare are yours to keep, however when you leave D-H, you will no longer be able to participate in the ImagineCare App, and nursing support. How Do I Register for ImagineCare? Upon becoming eligible for benefits through Dartmouth Hitchcock you can expect to receive an inviting you to participate in ImagineCare along with simple instruction on how to sign up. Existing employees who are interested in enrolling in ImagineCare should call ImagineCare at Can I get just the activity tracker, and not participate in ImagineCare? If so, how do I enroll in only the activity device? Yes, you can enroll in only the band, and you may decide not to participate in ImagineCare but you will not have access to Nursing or Technical support. To obtain ONLY the band, contact the benefits office at There is something wrong with devices who do I contact? Contact ImagineCare directly at for any technical questions, or issues you have with your band as long as you have also enrolled in ImagineCare. If you are a device only user please contact Benefits at ImagineCare also makes visits to your location please read D-H today or call Imagine Care for scheduled dates and times. I signed up for ImagineCare but I don t want my band, can I return it for a refund? You have up to three-weeks after signing up for ImagineCare to return the band for a refund provided the band is returned in near new condition. Damaged or destroyed bands may not be returned for a refund. If I am a band only user and I want to engage and enroll in Live Well Work Well Health Coaching, or Nutrition counseling who should I contact? Live Well Work Well Health Coaches are always available for one on one consultations and counseling. To request an appointment please call or livewellworkwell@hitchcock.org.
5 What services does Live Well Work Well Health Improvement Program offer? The employee Health Improvement Program offers a variety of wellness programs designed to help improve and promote well-being through on-line webinars, in person seminars or face to face coaching, fitness classes and many other options available on the intranet.
