1 KENYATAAN TAWARAN TENDER HOSPITAL PUSRAWI SDN BHD 1. Tender adalah dipelawa kepada Kontraktor yang mempunyai Sijil Perolehan Kerja Kerajaan berdaftar dengan Lembaga Pembangunan Industri Pembinaan Malaysia (LPIPM / CIDB) dalam Gred, Kategori dan Pengkhususan yang berkaitan bagi kerja-kerja berikut :- Tender Kelayakan Di paparkan di Laman Web Pusrawi Tarikh Tutup Serahan Tender PERKHIDMATAN MEMBEKAL, MEMASANG, MENGUJI, MELATIH DAN MENYELENGGARA SISTEM PICTURE ARCHIVE COMMUNICATION SYSTEM (PACS) DAN RADIOLOGY INFORMATION SYSTEM (RIS) Perakuan Pendaftaran Peniagaan. Hospital PUSRAWI berhak menolak penyertaan jika pada tanggapan Hospital PUSRAWI terdapat mana-mana butiran kelayakan yang tidak layak. 3. Semua tawaran tender hendaklah dimasukkan ke dalam peti tender yang telah ditentukan sebelum atau pada pukul 3.00petang pada tarikh yang ditetapkan). Tawaran tender yang lewat diterima dari waktu dan tarikh yang ditetapkan tidak akan dipertimbangkan. 4. 3 HOSPITAL PUSRAWI SDN BHD SYARAT-SYARAT TENDER (AM) Kontraktor-Kontraktor hendaklah mematuhi sepenuhnya syarat-syarat ini apabila menyertai tender. Kegagalan mematuhi mana-mana syarat di bawah ini, membolehkan tender tersebut tidak dipertimbangkan dan terus ditolak. KELAYAKAN UNTUK MEMASUKI TENDER Mengemukakan profile syarikat terkini termasuk senarai kerja yang telah dilakukan. CARA MENGISI BORANG TENDER Hanya borang yang dikeluarkan olehhospital PUSRAWI sahaja diterima bagi maksud tender ini. Lain-lain borang tidak diterima kecuali sebagai tambahan kepada borang yang asal. Borang tambahan yang hendak digunakan mestilah dengan berukuran yang sama dan mengandungi ruangan-ruangan yang sama dan serupa dari segi ukurannya. Tiap-tiap helaian borang tender yang digunakan (termasuk borang tambahan) mestilah ditandatangani dan dicop nama Kontraktor (Syarikat) di sudut kanan sebelah atas dan mestilah diberi nombor muka surat yang bersiri mengikut susunannya. Dua salinan borang yang diisi dengan lengkap adalah diperlukan bagi maksud tender ini iaitu satu salinan asal dan satu salinan fotostat yang terang. Borang-borang tersebut mestilah ditaip dan borang-borang yang ditulis dengan tangan akan ditolak. Kontraktor-Kontraktor mestilah mengisi dengan lengkap ruangan-ruangan yang disediakan di dalam borang tender. 2

4 (i) TARAF SYARIKAT : Nyatakan samaada bumiputera atau bukan bumiputera. (i) NAMA KONTRAKTOR/NAMA SYARIKAT : Borang tawaran harga yang dinyatakan perlu diisikan seperti dikehendaki. Jika tawaran harga berlainan daripada format, sila berikan penjelasan. (iii) HARGA Semua harga yang disebutkan mestilah di dalam RM (Ringgit Malaysia). Harga yang ditawarkan mestilah tidak melebihi dua angka perpuluhan. Sebagaicontoh: RM 0.12/unit. Mana-mana kontraktor yang menawarkan harga mengikut perubahan kadar pertukaran wangasing tidak akan dipertimbangkan. (iv) TEMPOH SAH LAKU TAWARAN : Tender-tender hendaklah terus sah selama tempoh enambulan (6) dari tarikh akhir bagi penyerahan tender sebagaimana yang ditetapkan dalam Notis Tender (dalam Syarat-Syarat Membuat Tender ini disebut Tempoh Sah Laku Tawaran dan tempoh itu boleh dengan persetujuan bersama dilanjutkan jika dan apabila perlu. (v) LAIN-LAIN MAKLUMAT : Catitkan apa-apa maklumat yang berkaitan dan difikirkan perlu diadakan di perjanjian kelak. Bagi ruangan yang kosong, misalnya tidak ada apa-apa maklumat yang hendak dicatatkan. Maka ruangan itu mestilah ditulis dengan perkataan Tidak Berkenaan. 5. DUTI SETEM Mana-mana duti setem yang kena bayar dan perbelanjaan yang berbangkit mengenai perjanjian ini akan ditanggung oleh Pihak Kontraktor. 6. KUANTITI TAWARAN/KUANTITI TAMBAHAN Hospital PUSRAWI berhak menambah atau mengurangkan kuantiti yang ditetapkan dalam dokumen tender, pada masa surat setuju terima tawaran dikeluarkan kepada kontraktorkontraktor yang berjaya, selain daripada kuantiti yang telah ditetapkan dalam suratlantikan. Hospital PUSRAWI boleh meminta perkhidmatan tambahan dalam tempoh kontrak ditandatangani dengan mengemukakan sebutharga baru. 3

Syarikat pembekal hendaklah menentukan bahawa tawaran ini tidak dijalankan oleh orang lain setelah tawaran diberikan. 5

7 LAMPIRAN B TAWARAN TENDER PERKHIDMATAN KESELAMATAN PENENTUAN-PENENTUAN PERKHIDMATAN Kontraktor yang terpilih dikehendaki mengambil Public Liability Insurance Policy untuk melindungi Hospital PUSRAWI, sekiranya timbul sebarang tindakan undang-undang, tuntutan gantirugi atau pembayaran yang mungkin dibuat keatas Hospital PUSRAWI atauhospital PUSRAWI dipaksa membayar tuntutan yang timbul dari perkara-perkara kemalangan atau kecederaan yang ditanggung oleh kontraktor atau pekerja atau mana-mana pihak yang disebabkan oleh sikap tidak bertanggungjawab pembekal atau pekerj atau sesiapa sahaja yang bekerja di bawah pengawasannya. Kontraktor adalah terikat dengan peruntukan-peruntukan Akta Rahsia Rasmi 1972 dan Pindaan

8 BAHAGIAN / UNIT : PENGIMEJAN DIAGNOSTIK (X-RAY) PERKHIDMATAN / BEKALAN : SPESIFIKASI TEKNIKAL SEBUTHARGA PERKHIDMATAN MEMBEKAL, MEMASANG, MENGUJI, MELATIH DAN MENYELENGGARA SISTEM PICTURE ARCHIVE COMMUNICATION SYSTEM (PACS) DAN RADIOLOGY INFORMATION SYSTEM (RIS) 1.0 TAKRIF DAN TERJEMAHAN 1.1 Hospital PUSRAWI Sdn Bhd (PUSRAWI) adalah Hospital swasta anak syarikatkepada Majlis Agama Wilayah Persekutuan Kuala Lumpur. 1.2 JABATAN PENGIMEJAN DIAGNOSTIK(JPD) bermaksud satu jabatan di PUSRAWI yang menyediakan perkhidmatan pengimejan perubatan seperti X-ray biasa, Ultrasound, CT Scan dan MRI. 1.3 Penyebutharga bermaksud pembekal yang membekalkan perkhidmatan/bekalan yang di tawarkan oleh PUSRAWI. 2.0 PENGENALAN 2.1 PACS dan RIS adalah pembelian baru sistem IT yang akan digunakan di Jabatan Radiologi dan dihubungkan dengan semua unit di dalam hospital seperti wad, klinik dan dewan bedah. 2.2 Penyebutharga yang dijemput adalah tertakluk kepada syarat-syarat dan arahanarahan yang terdapat dalam dokumen ini. Arahan-arahan dan syarat-syarat yang dinyatakan hendaklah disifatkan sebagai syarat kontrak. Sebutharga yang tidak mengikut syarat-syarat yang dinyatakan tidak akan dipertimbangkan. 2.3 Spesifikasi yang terdapat dalam dokumen ini adalah spesifikasi minimum yang perlu dilengkapkan oleh Penyebut Harga. Spesifikasi teknikal yang lebih detail disertakan dalam format excel dan perlu dijawab oleh Penyebutharga. 3.0 SKOP PERKHIDMATAN 3.1 Skop perkhidmatan ini adalah melibatkan pembekalan, memasang, menguji,mentauliah, melatih pengguna dan menyelenggara sistem PACS dan RIS. 4.0 SPESIFIKASI PERKAKASAN / JADUAL HARGA

Able to support Instalment plan by leasing company (60months) JUMLAH KESELURUHAN (RM) Lump sum 5.0 PENGURUSAN PERSONEL 5.1 Pembekal hendaklah mempunyai tenaga kerja yang mencukupi dan berkelayakan. 5.2 Pembekal dikehendaki menguruskan latihan berterusan untuk memastikan semua pekerja mempunyai pengetahuan dan kemahiran terkini dalam mengendalikan tugas. 5.3 Kriteria penerimaan tertakluk pada syarat-syarat kontrak dan peraturan PUSRAWI. 6.0 KAWALAN KUALITI DAN PERKHIDMATAN 6.1 Semua peralatan hendaklah dibekalkan oleh pembekal di lokasi yang telah ditetapkan oleh PUSRAWI tanpa sebarang kos tambahan kepada PUSRAWI 6.2 Semua peralatan yang dipasang dan diuji hendaklah dilakukan oleh individu bertauliahyang dilantik oleh pembekal dan sebarang kos untuk melantik individu berkenaan adalah tanggungjawab pembekal. 6.3 PUSRAWI berhak untuk menolak sekiranya kerja-kerja yang dilaksanakan tidak mengikuti spesifikasi yang ditetapkan. PUSRAWI berhak untuk melantik mana-mana individu bertauliah untuk melengkapkan kerja-kerja tersebut dan kos berkenaan adalah tangungjawab pembekal. 6.4 Pengujian akhir software dan hardwareperlulah mendapat pengesahan daripada pegawai IT Hospital Pusrawi. 6.5 Spesifikasi yang terdapat dalam dokumen ini adalah spesifikasi minimum yang perludilengkapkan oleh Penyebut Harga. Spesifikasi teknikal yang lebih detail disertakan dalam format excel dan perlu dijawab oleh pihak pembekal.

13 7.0 TEMPOH JAMINAN 7.1 Tempoh jaminan mestilah mengikut spesifikasi yang telah di tetapkan. 7.2 Sebarang peralatan atau perisian yang mengalami masalah atau kerosakan disepanjang tempoh jaminan perlu dibaiki atau diganti segera dan sebarang kosadalah tanggungjawab pembekal sepenuhnya. 7.3 Semua kos atau upah sepanjang tempoh jaminan adalah tanggungjawab pembekalsepenuhnya. 7.4 Sebarang perkara yang tidak termasuk di dalam jaminan pembekal perlu dinyatakan di dalam Sebut Harga. 8 LOKASI PEMASANGAN JABATAN PENGIMEJAN DIAGNOSTIK(JPD) ARAS 1, HOSPITAL PUSRAWI SDN BHD, LOT 149 JALAN TUN RAZAK KUALA LUMPUR 9 PENALTI Penyebutharga akan dikenakan penalti sekiranya lewat daripada tempoh yang telah di persetujui di dalam surat tawaran setuju terima sebutharga. Sekiranya terdapat sebarang kelewatan, penyebutharga perlulah memaklumkan kepada pihak PUSRAWI secara bertulis untuk di pertimbangkan. 10 TEMPOH PERLAKSANAAN Jangka masa pemasangan, pengujian dan latihan sehingga boleh digunakan (go live) perlulah dinyatakan dengan jelas di dalam tender sebut Harga.

20 5.4 PACS/ DATABASE SERVER STORAGE General Requirement Database server must use Oracle Database for its reliability and security purpose Server software support minimum 25,000 study (exam)/year and able to upgrade Storage architecture should support Primary Storage and On-Line Backup Storage.
