7 What if? I have a TWIST card (T-Mobile SIM without a contract) or I don t have any SIM card from T-Mobile yet? It is necessary to conclude contract first: - personally at the T-Mobile Shop at Flora shopping mall - online via e-shop - via phone, by calling the Customer Centre T-Mobile When signing the contract I ask for inclusion in the program, based on the program number Then I will send an to the Discount program administrator and ask for inclusion into the discount program BSC Social Club Prague /SM We will need your: name, phone number and tariff you have chosen The same applies when you wish to bring your family members - 7 -

8 What if? I have a contract with another provider? I need code for transfer (ČVOP), this code you get from your actual provider Whith ČVOP I will conclude contract - personally at T-Mobile Shop - online e-shop - calling the Customer Centre T-Mobile When signing the contract I ask for inclusion in the program, based on the program number and password (same as previous page)

9 What if? I have a contract with T-Mobile? I choose a tariff and I ask to be included in the employment program, based on the program number and password. 10 Conditions of this offer Other discounts cannot be combined with this offer Any promotions offered by T Mobile Czech Republic for a limited time can t be used with this offer. In the case an employee wants to convert a current Subscriber Contract into an ExxonMobil Employee Contract, the current offer will be deactivated in the process of conversion All contracts are between T Mobile and the individual employee Employee (owner of the contract, participant) is responsible for payment of invoices for all numbers belong to her/his contract ExxonMobil nor the Social Club is responsible for unpaid accounts Other calling charges are according to the actual valid price list of T Mobile Czech Republic General terms and conditions of T-Mobile apply

11 Additional Conditions An employment contract with ExxonMobil Business Support Center Czechia and valid SC membership In the case that an employee ends his/her employment contract with EMBSCC his contract will continue according to General Terms and Conditions of TMCZ but all discounts will be deactivated. Same if the participant s SC membership expires. In the case that the subscriber s contract is signed for two years, customer can change the tariff to one from the actually offered tariffs however the defined period remains. In the case that the subscriber s contract has got expired contract period or has originally been signed without two years contract it can be terminated with 30 days' notice
