Healthcare professionals can track important changes in a client s condition through intuitive exception reporting capabilities and automated FYI (For Your Information) alert notification. These changes can be quickly identified, documented and shared with other team members through CareNet, including family members or other non-traditional care team members when appropriate. 3 CareNet Web-based Patient Monitoring Alerts and Notifications Healthcom s FYI (For Your Information) automatic alert notifications can keep you in the loop even when you re not logged into the system. Quickly identify and address alerts. Include comments for other team members. CareNet FYI (For Your Information) This application permits the user to identify the client with an alias if desired (e.g., chart #), choose a method of notification (e.g., fax, , text message, page), and a frequency of notification (immediate, daily, weekly, etc.). This allows team members to receive important alert notifications even when they are not logged into the system. Client Documentation Add unlimited journal entries to client records. Restrict or allow viewing by staff or family to increase lines of communications. Address Alerts and Add Notes Additional details can be viewed by clicking on a client name. Alerts can be addressed, annotated and cleared directly from this page. Team members can add valuable journal entries or notes that are important for the clinical team to see and review.

4 Access Point Secure transmission via standard phone lines or cell. The Wireless Access Point is the device responsible for collecting data from telehealth devices and transmitting that data to the Healthcom servers. Secure transmission is guaranteed whether transmission is over a standard phone line, or through the cellular network. Healthcom s CareVital program uses Bluetooth enabled devices to collect patient vital signs and securely transfer them to our web servers. The collected data may be viewed and managed directly from Healthcom s CareNet website. Effective Design Wireless capabilities eliminate complicated installations. 5 Blood Pressure The wireless blood pressure device is an upper arm automatic blood pressure monitor which uses the oscillometric method of blood pressure measurement. At the end of the measurement, blood pressure and heart rate readings are sent automatically via Bluetooth to the wireless Access Point. If there is an interruption in the communication, readings will be stored in memory (up to 40 sets). 6 Scale The Bluetooth wireless digital scale provides highly accurate and precise measurements for telemedicine applications. This scale is one of the thinnest and lightest on the market. Measuring less than 1 thick, it is easy to step on and store. This new design offers more precise readings and functionality compared to traditional scales. After a weight reading is taken, the device will automatically send the readings via Bluetooth to the wireless Access Point. If the Access Point is not within range of the scale, the readings will be stored in memory (up to 40 sets). 7 Pulse Oximetry Healthcom lets you choose from two different pulse oximeter devices. Both eliminate the cable between the patient and the monitor. 8 Blood Glucose Monitoring The wireless blood glucose device (One touch Ultra) uses the latest blood glucose monitoring technology. It measures the Glucose content of a blood sample by means of an electrical current produced in the test strip and sent to the meter for measurement. Test results are Plasma-calibrated. This makes it easier for you and your diabetes-care team to compare results with laboratory tests. The One Touch Ultra monitoring system consists of a blood glucose meter, control solution, sampler, lancets, and Bluetooth transmitting device. (Test strips sold separately). Important: In order to transmit readings to the Access Point the blood glucose collection device must be plugged into the Bluetooth transmitting device.
