1 A GUIDE TO YourStore 3.0 Selling contact lenses online has never been easier! This is the homepage of YourStore The header displays just your company name as its default. The rectangular space (where you now see the words Family Vision ) can be customized with either your company logo or both your logo and your company name. Please send your account number, logo(s), address, and phone number to marketing@wisvis. com, with the subject line YourStore Banner Request, to get started! To begin the ordering process, your patients will either select a brand name from the left of the page (such as Acuvue) or View All Products. There are also a few resources listed on the left, as well. 3. Once a brand is selected, a list of available contact lenses appears There is an alpha-numeric filter at the top of the page that can help narrow down all of our products this is especially helpful when View All Products has been selected When the desired contact lens is found (such as Acuvue Oasys), click on the Order button next to it. 5

2 Clicking on the Order button will lead to a page where the patient fills out the name and prescription of the person they are ordering for, as well as the quantity of boxes. NOTE: some contact lenses will auto-fill the number of boxes to equal a year-supply. This can be changed in this screen or in the Cart If there is a rebate with the product, it is shown on the right in orange, as is the total needed for free shipping If you only want to order contacts for one eye, uncheck the box next to either Right Eye or Left Eye. The box that is checked designates the eye you wish to order. 8. Once all the information is filled out, click Add To Cart. 8 9 You now see the Cart. 9. The number in the blue bar on the top right part of the page will now correspond to how many different contact lenses you have in your cart. 10. If you would like to order more contact lenses, you can click a brand on the left. Follow the above instructions to place the additional orders in your cart. 11. The rebates that correspond to the purchase, as well as the amount of money saved, are shown in the two areas below your lenses. 12. Once you have finalized your order, select your shipping method: Standard, UPS Ground, 2nd Day Air, or Next Day Air. The total that is shown at the bottom does not include any applicable taxes. 13. Click Place Order once you are done. 14. Your patients will need to sign in in order to place their order. At the top of the page, please select either Existing Account or New Account. For Existing, simply enter your address and password, and hit Continue. For New, fill out the information and hit Create Account. 15. Once your patient has signed in, they will then fill out the pertinent shipping and billing information and click on the Order Confirmation button to proceed

3 Clicking on the Order Confirmation button takes your patient to a page to review their order. This is their last chance to make sure everything is correct before placing their order. 16. Once everything has been verified, your patient needs to enter the displayed words before they are allowed to place their order. 17. Clicking the Place Order button sends their order to our system for processing The Order Confirmation page is your patient s receipt. Your patient should print it for their records and rebate redemption by clicking the Print button. 18 As an added benefit, we send out a Thanks for your order confirmation to the address that your patient entered.

4 How Your Patient Logs In 1. To log onto YourStore, click the Sign In button at the top right hand of the screen. Signing in allows your patient to see their Order History, edit their account information and password, and check out faster Enter your address and password, and click Sign In Once signed in, your patients name will be at the top of the page and the image will change to a green check mark The patients last order will be at the bottom of their login screen, allowing them to reorder them with one click. 5. Clicking on My Account at the top of the page will bring you to the shipping information your patient provided us. To change things, click on Edit Account Information Edit the necessary information, and then click Update Account Information when done Selecting the Order History option at the top of the page shows all of the products that the patient has ordered. 8. Selecting Print will allow the patient to print out their invoice for that particular purchase. 9. Selecting Reorder will put the pertinent products into the patients cart

5 What you and your staff see Once the patient has ordered you will receive a confirmation and can find the order in the Your Open Orders section of the Order from WVA page. 19. You or your staff can then access the information provided by the patient by clicking on the web number associated with the web user After you verify that the prescription your patient chose is correct, click Continue. You will be prompted to check your order one last time and finalize it. Once the order has been finalized, WVA will then run the credit card and ship the order to your patient You can print the order for your own records or click Orders on the navigation bar to go back to the Order from WVA page.

6 Your account tools located in the YourStore tab. Add/Edit Patient Accounts is an optional patient management database. You are not required to utilize this tool for YourStore. 22. To search for a patient, make sure you are on the Manage Patients tab at the top of the page. Then start typing in either the patient s last name, first name, or address, select the Search button, and select their name from the list populated below the field By selecting the Manage Prices tab, you can both update or delete a free shipping threshold for your patients. Simply hit the Update button, type in your price, and reselect the Update button Under the same tab, you can also set your own prices by selecting a Vendor from the drop down list. You can now set tiered pricing with the option to set singlebox, six-month, and annual supplies! To define a price, enter in an amount and click the Set button to finish. Update and Delete are also available. 7 YourStore 3.0 EXTRAS Enhanced 1 YourStore 3.0 Enhanced allows you to customize to better mimic your practices branding and color schemes. If you are not currently running YourStore 3.0 Enhanced, please contact your WVA Sales Representative for more information. YourStore 1. Your customizable banner is one of the advantages to having YourStore 3.0 Enhanced. Not only do we put your logo on your page, but we also design the entire space to adhere to your practices branding and to make a bigger statement. This feature is free with the Enhanced version. Talk to your WVA Sales Representative for more information Banner Image Size Dimensions: 850 pixels by 140 pixels 2-5. These are just four of the multiple color schemes your practice would have at their disposal. You can change it as frequently as you would like it to. Don t see the exact theme you want? Contact your WVA Representative for more information on how to get your own custom-colored theme! 6-8. There are three different configurations that you can utilize in YourStore 3.0 Enhanced. You can easily change the layout as often as you want. Each account has a few standard images to choose from. If you would like to send in your own images, you may do so for free. Please refer to the Managing Your YourStore Image Layout form for sizing and layout questions. If you would like WVA to design a custom ad, that is also possible. Talk to your WVA Sales Representative for more information on these two options

9 YourStore 3.0 FAQ How do I get YourStore 3.0? Log onto and click on the Resources tab. Under the heading Web Info, click on YourStore 3.0 Agreement Fill out the agreement and fax back to A WVA Representative will contact you and assist in the YourStore 3.0 activation. Do I need a website to utilize YourStore 3.0? Yes, it is necessary to have a website in order to take advantage of YourStore 3.0. WVA is prepared to build you a quality custom website. Please go to for more information. How do I link YourStore 3.0 to my website? WVA will provide you a link that your web designer places into your current website. This link could be displayed as a button that your patients click to order contacts. They will automatically be taken to YourStore 3.0 and may begin placing orders. How are orders and prescriptions verified? Prescriptions are verified on the website. After logging in you will see an order pending, created by Web User, in the Open Orders dashboard. Click on the order number to see the detail and billing information for your patient. Once you have verified the order is correct, click Submit and the order will be sent to WVA. You can then print the confirmation page for your records. What costs are associated with YourStore 3.0? YourStore 3.0 is a complimentary service provided by WVA. The customizable version, YourStore 3.0 Enhanced, has a quarterly fee. Will my Patient s YourStore 3.0 orders pull from my lens banks? Yes. Are patients charged sales tax? YourStore 3.0 orders do not collect sales tax. Are YourStore 3.0 orders subject to sales tax? The orders may be subject to a sales tax. Your WVA Sales Representative is available for further consultation. Is my account charged sales tax? The account may be subject to a sales tax. Your WVA Sales Representative is available for further consultation. Do YourStore 3.0 orders qualify for manufacturer promotions? Yes, rebates are available for qualifying purchases through YourStore 3.0. Patients have the ability to see the most current mail-in rebates when selecting their prescribed brand of lenses. Who is responsible for charging the patient s credit card? YourStore 3.0 transactions are auto-processed via WVA s secure website.

10 What happens if the credit card is declined? Your patient will be notified when they enter their credit card number that their transaction has been declined. They will have the opportunity to utilize a different card or reenter their card s numbers again. What credit cards are accepted on YourStore 3.0? YourStore 3.0 accepts the following credit cards: MasterCard, Visa, and Discover. Will my patient s credit card still be charged if I canceled their order? Your patient s credit card will only be charged if you validate and submit the order on If you choose to cancel the order, the card will not be charged. Is credit card information secure? All credit card information is encrypted and submitted through a secure server. WVA does not keep any credit card information, per PCI DSS compliance. Is YourStore 3.0 HIPPA compliant? YourStore 3.0 takes HIPPA very seriously. All patient information is transmitted in an encrypted format and is never released to third parties. WVA is subject to and remains compliant with all HIPPA federally mandated healthcare regulations. Can I set my own pricing? Yes, you are able to customize pricing for all lenses offered by WVA. How do I set my own pricing? You can set your own prices by logging into and clicking on the YourStore tab. Select a Vendor from the drop down list. To define a price for a single-box, six-month supply, or annual supply, enter in an amount and click the Set button on the product line to finish. Update and Delete are also available. How do I know if my pricing is competitive? WVA provides a quarterly Pricing Guide (found on the Resources tab at to help keep you competitive with major online retailers. In addition, your WVA Sales Representative is available for further consultation. Is there any reason not to fill a non-patient s prescription? It is up to your practice to fill a patient s order through proper verification. Will my patient s YourStore 3.0 orders be calculated toward my monthly sales volume? Yes, In addition those increases are communicated to manufacturers as well for rebates and other credit allowances. What does YourStore 3.0 communicate to the patient? YourStore 3.0 provides an order confirmation to the patient. Also, you will be notified via when a patient has placed an order. Are patients charged for shipping? Your patient has the ability to choose a specified shipping method. Depending on the method chosen, the patient will incur the charge for shipping. Incentives for free shipping are initiated with a specific dollar amount. Is my account charged shipping? Yes, unless the order meets specific vendor free shipping initiatives. What happens when the product that was ordered for my patient is damaged in transit? Your practice returns it to WVA after it is received from your patient.

11 What if I need further assistance? Help can be obtained by either your WVA representative, WVA customer service at or by at support@wisvis.com. When are prescriptions verified? After your patient s credit card has been cleared, you will be notified by that an order is pending your approval. How will I receive the profit for the sale since WVA collects payment? WVA will reimburse your practice in the form of a credit on your monthly statement. You will be reimbursed for the profit margin minus a 3% transaction fee. How do charges appear on my statement? Look for the charges to be labeled as YourStore. What if my patient wants to return their lenses? Your patient must return their purchases to your office. Please send all YourStore 3.0 returns back individually (not with standard patient returns) as soon as possible so that your patient s credit card may be refunded. 13 YourStore 3.0 THE AGREEMENT Effective / /, Wisconsin Vision Associates (WVA) holds the YourStore 3.0 agreement as a legal binding contract between WVA and, hereafter known as the account. WVA has developed the YourStore 3.0 website suite as a means to provide the account with the ability to engage in e-commerce with their patients. As a subscriber of the YourStore 3.0 website suite, the account agrees to utilize WVA as their primary source for obtainable soft contact lenses. Accounts must be in compliance with the terms and conditions in the WVA membership application, be aware of purchasing options, and uphold all communication with their patients support and prescription verification. WVA agrees to process and fulfill orders for the account. Moreover, WVA agrees not to distribute, duplicate, or provide for sale any information of a patient other than as directed by the account. The account realizes contractual obligations to vendor partners (e.g. manufacturers) will require minimal patient information. The account gives WVA the authorization to enter into e-commerce with their patients in compliance with both state laws, HIPAA, and PCI DSS. The account understands that WVA will collect payment on their behalf. All communication with said patients will be conducted by the account and will not be the responsibility of WVA. WVA will reimburse the account in the form of a credit on their monthly statement for the profit margin minus a 3% transaction fee per completed YourStore 3.0 order. This content will always remain appropriate and patient driven. WVA reserves the right to change the content without notice.
