Verify patient identification Inform patient of MyUCLAHealth rollout Open patient chart Access Code prints out in AVS Launch Communication Management Activity Click on MyChart letter button (Letter opens and activation code is automatically created) Print Letter USER hands patient AVS, myucla Health Brochure, Activation instructions, and proxy info USER hands patient printout of access code, myucla Health Brochure, Activation instructions, and proxy info Patient uses his access code at home, chooses a name and password, and logs in to MyChart.

Submits a medical advice request from Receives message in InBasket folder and reviews it Message is routed to a POOL Should the provider be involved? Forwards message to Provider Replies to patient using MyChart Encounter and marks message as Done Marks original message as done Receives message and reviews it Views response in Replies to patient using MyChart Encounter and marks message as Done

5 Medication Renewal Request MD/NP RN/MA/LVN Patient Needs an Rx Renewal. Submits an Rx renewal request from myucla Health. Receives Rx Request InBasket message Receives message. Initiates encounter documentation but does not sign med orders. Does provider need to change the Rx or document? Routes encounter to Provider. Enters the encounter via InBasket. Is request approved? Marks the initial message as Done. Completes encounter documentation. Chooses Approve Rx from IB. Receives Rx Response message in InBasket Approves or refuses medication request Does patient need to be contacted by staff? Replies to patient with instructions and dones message. Routes encounter back to RN/MA/LVN. Views response in Chooses Refuse Rx from IB.

6 Deactivation Clinic Patient Desire is present to deactivate a account Is clinician revoking access? Patient calls the clinic. USER goes to MyChart Admin activity Click Deactivate Patient Uses letter template to notify patient of deactivation if needed. Patient is deactivated.

7 Demographic and Preference Updates Demographics Submits a demographics update request from Address and phone number is automatically updated in the patient s record. FPG Patient Needs to update information. What information is being updated? Preferences Submits a patient preferences update from Automatically updated in the system. Password address Updates password. Updates address. Receives InBasket message in a POOL and reviews it. Can also update the password security question/answer. is automatically updated in and CareConnect. Password information is automatically updated.

8 Customer Service Request Needs customer service. Selects a subject from the drop down menu. Clinic Manager/Director Patient Submits a customer service request of Compliments & Concerns Message routed to central POOL User forwards message to appropriate Clinic Manager and/or Director/CAO Can this user resolve the issue? Routes the message to another user. Resolves issue. New user receives the message in their In Basket. Replies to the patient via a secure patient message. Views the response in their MyUCLAHealth Inbox. Dones the message, removing it from their In Basket.
