3 3 Your Health Care and Dependent Care Flexible Spending Account (FSA), Health Reimbursement Account (HRA) and/or Health Savings Account (HSA) are administered through Optum s Health Advantage Website, which brings you an integrated web and mobile experience to manage your Healthy Rewards and Health Accounts. Visit optumhealthfinancial.com to manage your account, 24 hours a day, 7 days a week. Health Accounts The secure website enables you to: File a claim Set up direct deposit to receive your reimbursements faster View up-to-the-minute account balances Upload receipts and track expenses View or download plan information, forms and notifications Request additional cards Report a lost or stolen card Set up alerts and text notification alerts And more! Simply follow the instructions on pages Note: Screens in this guide may vary from what you see when you re online, as your actual view depends on the plans you re enrolled in.

4 4 GETTING STARTED: HEALTH ACCOUNTS Logging in to the Health Advantage website for the first time 1. Visit optumhealthfinancial.com and click Log in. 2. Click Register now to set up your account profile. Since this is a secure website with financial information, you ll be asked to enter your Social Security number or your 5-digit Crewmember ID, date of birth and last name. 3. Create a username and password. Set up security questions and enter or confirm your address. 4. Accept the website terms and conditions. Activating your Optum Payment Card optumhealthfinancial.com Once you ve registered on the site, you ll be able to manage your account 24 hours a day, 7 days a week. When you receive your card(s) in the mail: Follow the instructions on the card sticker and call You will need to have the last four digits of your Social Security number to activate your card. Sign the back of your card. Keep your card even if you use all of your funds. The same card will be used for your next plan year.

5 5 GETTING STARTED: HEALTH ACCOUNTS Navigating the home page Accounts: View account information, claim status/ payment history and transactions, including those from the Healthy Rewards program. Tools & Support: View and download forms, review plan information, and access education materials, including instructional videos. Statements & Notifications: View and download account statements and manage electronic communication preferences, including text and notifications. Manage My Expenses: Get a centralized view of health care activity across all of your accounts. You ll find information on payment card transactions, claims, contributions and distributions. This is also where you can store and add expenses for future reimbursement or reference. Profile: Update your personal information, add your address to receive communications more quickly, report a lost/stolen card or request additional cards, manage your banking preferences such as setting up direct deposits, and add beneficiary information (if you have an HSA). Top Navigation Bar: Get quick access to account information and other tools. Welcome Banner: Read important messages and updates. I Want To: This sidebar contains the most frequently used functions on the website, so you can easily link to file a claim or manage your expenses. optumhealthfinancial.com Available Balance: This links to the Account Summary page, where you can see and manage your accounts. View up-to-the-minute account balances and account activity details. Message Center: See alerts and access relevant links to keep current on your accounts.

6 6 GETTING STARTED: HEALTH ACCOUNTS Managing and adding expenses Click Add Expense from the Manage My Expenses tab and, on the next screen, enter the expense description, date of service, amount, provider and recipient/patient. Then upload your receipt and add notes. Click Add when complete, and the expense will be added to the list of transactions. Viewing your accounts summary The Accounts tab lets you view your account summary information in one place. You can see realtime balances or follow links to get more detailed information such as transaction history, claim status and earned Healthy Rewards. If enrolled in an HSA, you can use the drop-down menu to select HSA, and the screen will refresh with pending and processed transactions. Under the description you will see all transactions in the accounts, including Wellness Contributions. Click on any of the transactions, and the screen will expand to provide additional information. Viewing your Healthy Rewards To view your earned Healthy Rewards, click Account Activity on the Accounts tab. Use the drop-down menu to select your Healthy Rewards account. You can also view your other financial accounts, view seed dollars and, if enrolled in an FSA plan, see your election for the year. From here you can also submit a claim or update your notes. optumhealthfinancial.com

7 7 GETTING STARTED: HEALTH ACCOUNTS Finding forms View and download forms, documents and additional resources on the Tools & Support tab. Viewing and downloading account statements Use the Statements & Notifications tab to access account statements. Setting up alerts To receive information about your claims (Advice of Deposits, denial and appeals information, receipt reminders) and accounts statements electronically rather than by mail, simply add or confirm your address to your Profile (see the Updating Your Profile section on the next page). Optum will send you an with a link to view your document online. It s that easy! Setting up text notification alerts If you wish to set up text notifications regarding your account, click Update Notification Preferences under the Statements & Notifications tab. Enter your phone number and select a time zone. Helpful Hint: On this page you can find the Quick Links section with additional instructions and tutorials. optumhealthfinancial.com Helpful Hint: Be sure to keep your address updated under your Profile (see page 8). You can choose to receive paper mailings again at any time without charge by removing your address from your Profile.

8 8 GETTING STARTED: HEALTH ACCOUNTS Updating your Profile Visit the Profile tab to update your personal and login information, add or confirm your address and phone number to receive communications quickly, add dependents, and manage your banking preferences, including setting up direct deposit. You can also report a lost or stolen Optum Payment Card or request additional cards. If you have a Health Savings Account, this is also where you add beneficiary information. Adding a bank account for direct deposit Click Profile on the home page, then Banking/ Cards and then Add Bank Account. Provide the required information for your personal bank account. Requesting additional cards From the Profile tab on the home page, click Banking/Cards and then Order Replacement. Verify your address. (If the address is incorrect, update your information through the Update Profile link first.) A new card with the same card number will be issued and mailed to the primary cardholder s address within 5 7 business days. Reporting a lost or stolen card From the Profile tab on the home page, click Banking/ Cards and then Report Lost/Stolen. Verify the primary cardholder s address. (If the address is incorrect, update it via the Update Profile link first, and then return to report the card lost or stolen.) A new card with a new card number will automatically be issued and mailed to replace the lost/stolen card within 5 7 business days. optumhealthfinancial.com

9 9 HRA (GREEN PLAN) SUBMITTING A CLAIM Filing a claim Filing a claim is one of the most frequently used features. To file a claim, click File a Claim from the home page. Click Next, and you will be prompted to upload supporting documentation. You will then enter your claim information including start/end date of service, amount, provider information, category of expense, type of expense, description of service, name of person incurring the expense, and mileage. When finished, click Next. Use the drop-down menus on the next screen to select the account and the method to receive reimbursement. Upload your receipt, and click Next. (Uploading a receipt is required to submit a claim. For more information, see Why Do We Ask for Receipts? under the Tools & Support tab.) Provide final confirmation of the claim by agreeing to the terms and conditions, and click Submit. You will also have the option to enter another claim or save the claim for later. optumhealthfinancial.com

10 10 HRA (GREEN PLAN) SUBMITTING A CLAIM Remember to keep all receipts, even the ones you ve uploaded, so that you have them for future reference to verify that your service or item purchased was an IRS-qualified expense. When submitting a receipt, be sure it includes: The date of service The name of the provider The service or item received The amount paid Add a bank account and/or use your payment card for faster reimbursement from your Health Account. Acceptable Receipt This receipt has all the information needed for a claim: 1 Provider (pharmacy) name 2 Date of purchase 3 Item or service 4 Amount paid Unacceptable Receipt This receipt has only the amount paid. 11 11 HSA (BLUE PLAN) CONTRIBUTIONS AND DISTRIBUTIONS Making a contribution to your account To make a contribution to your account, click Make HSA Transaction from the home page. A distribution can be issued as a payment to a health care provider. It can also be deposited online into your personal bank account or sent to you as a check. To request payment to a provider, enter in new provider details for the transaction. These provider details will be saved and can be quickly selected the next time you need to make a payment. Enter the contribution details and click Submit. It s that easy. Making a distribution from your account To request a distribution from your account, click Make HSA Transaction from the home page. Choose My HSA as the account from which to create the transaction. From here you can select Add Bank Account, or choose your bank account from the drop-down menu if you have entered it previously. Select My HSA for the contribution, and set up the frequency. You can either make a one-time contribution or set up recurring contributions to your HSA. optumhealthfinancial.com

12 12 HSA (BLUE PLAN) DISTRIBUTIONS You can set up a one-time payment to a provider or set up recurring payments for qualified medical expenses such as orthodontia. Next, enter the transaction details and click Next to continue to the transaction summary. You will be prompted to answer a security question for your HSA to complete the transaction. Agree to the terms and click Submit. optumhealthfinancial.com

13 13 HSA (BLUE PLAN) INVESTMENTS Investing your account With an HSA, you can also invest once your account balance meets the $1,000 threshold. Click Set Up Investment Transfers and a pop-up window will appear. Review the information presented including the two agreements, set your investment threshold to equal to or greater than the default, based on your personal financial situation, and click Save. Next click Manage Investments and answer the security question. The question will be set up upon first login and is required as an additional security measure to view and manage investments. This will bring you to the investments page, where you can view fund options and performance, set investment allocations based on your personal financial situation, realign your portfolio and view additional details about your HSA investment account. From the Manage My Accounts tab, click the Investment Elections menu to set investment elections. You can even make a proactive investment allocation, thus planning ahead for that very first sweep! Once you have made your investment elections, the automated sweep process will allocate funds that exceed the minimum required cash balance into your investments account. It will replenish your cash account if the balance falls below the threshold. If you would like additional investment assistance beyond the resources available online, you may work with an independent certified financial advisor. From the Fund Performance link located under the Account Information tab, you can view investment options, historical performance and prospectus information online, even if you haven t transferred money into your investment account yet. 14 14 HEALTH ADVANTAGE BY OPTUM MOBILE APP Getting started with the app To manage your Health Account anytime, anywhere, download the Health Advantage by Optum Mobile App from the Apple or Google Play Store. First-time login instructions: 1. Enter your temporary username. This is your last name in lowercase followed by the last 4 digits of your Social Security number (SSN) (example: smith4321). 2. Enter your temporary password. This is the word optum followed by the last 5 digits of your SSN (example: optum54321). 3. You will be prompted to create a unique username and password. iphone users: Download the Health Advantage Mobile App Android users: The most commonly used tasks are right up front, including: Requesting reimbursements Uploading expenses Viewing balances Checking messages and more Viewing balances, transactions and more You can view your HSA cash and investment balances, as well as transaction details, for all of your HSA contributions and distributions. You can also make a contribution to your HSA or generate a payment while on the go. For any of your other accounts such as FSA or HRA, it will show contributions and all claims activity, including payments and denials. Filing a claim Messages will be displayed for payment card claims requiring receipts. If a receipt is needed, such as for an FSA or HRA purchase that is not auto-substantiated, you will be guided to easily snap a picture of the receipt and upload it to validate your claim. Viewing or entering expenses To view or enter expenses (not to file a claim but to store and track for future payment), go to Expenses. Past expenses are available to view and update, and you may add a new expense as well. Anything that is entered on the mobile device is immediately updated to your account online. 15 15 Available via smartphone, tablet or computer, Rally powered by BlueCarpet can help you make simple changes to your daily routine, set goals and stay on target. After completing a quick and fun survey, you ll get personalized recommendations to help you reach your health goals. Rally is also where you can track your Healthy Rewards, which you earn for making smart choices such as learning CPR, working out, completing your Health Survey or signing your child up for extracurricular activities. Healthy Rewards To get started, complete the steps on the following pages.

16 16 LOGGING IN TO RALLY FOR THE FIRST TIME 1. Visit jetblue.werally.com and click Get Started. 2. Enter your JetBlue address and create a password. Your password must include: Minimum of 8 characters At least one number At least one uppercase and one lowercase letter At least one special character 3. Enter your participant info: employee ID (your 5-digit Crewmember ID), first name, last name and date of birth. Your covered spouse/domestic partner should use your employee ID to set up his or her account. 4. Set your username and a profile image. Your username should be fun and memorable but not your real name. jetblue.werally.com

17 17 COMPLETING YOUR HEALTH SURVEY Get your Rally Age and earn $75 in Healthy Rewards! Once you have registered, click Start the Survey. After completing the Health Survey, you will receive your Rally Age, which is calculated from the answers you provide in the survey. Your Health Survey responses will also personalize your Rally experience by recommending Missions customized digital action plans that are simple, actionfocused and attainable. Your responses will be kept confidential in accordance with the law and will only be used to provide personalized health and wellness recommendations. jetblue.werally.com

18 18 GETTING YOUR RALLY AGE After you complete your Health Survey, you will be presented with your Rally Age. Your Rally Age gives you an idea of your current overall health based on your answers. Helpful Hint: You may be able to improve or maintain your Rally Age by completing Missions and other healthy actions. jetblue.werally.com

19 19 ACCESSING YOUR HEALTHY REWARDS To learn more about Healthy Rewards and track your progress, select Dashboard within the top navigation bar. The Dashboard page will be the first page you see after you sign in to Rally (once you have completed your one-time registration). Then select View Full Program. To view the complete list of available Healthy Rewards activities, select Rewards within the top navigation bar and scroll down to Available Activities. A list of Healthy Rewards activities will be shown on this page. Next to each activity listed, there will be a green call to action such as Learn More, Verify and View Detail. For activities that have Verify, you will need to confirm completion. Based on the activity, you will then need to upload proof of completion or enter a word pin to self-attest. Helpful Hint: You can also view and track your Healthy Rewards progress by selecting Rewards within the top navigation bar. To view your list of completed Healthy Rewards activities, select Rewards within the top navigation bar and scroll down to Completed Activities. 20 20 ACCESSING YOUR HEALTHY REWARDS, CONTINUED For activities that have Learn More, you will be taken to a tutorial to complete your activity. For activities that have View Detail, you will see the activity requirements for rewards. jetblue.werally.com

21 21 MISSIONS After getting your Rally Age, you ll receive personalized activity recommendations to help reach your health goals. Rally will recommend Missions for you: simple activities designed to help immediately improve your diet, fitness and mood. Start easy, and level up when you re ready. There are a lot of Missions available, so there s something for everyone. Viewing and joining a Mission Rally uses your responses to the Health Survey to determine which Missions can be most helpful to you. For example, if you indicate in the survey that you don t exercise regularly, Rally might recommend easier Missions in the Move category that could benefit your health and are within your reach. To view recommended Missions, click Missions in the top navigation bar. To view all available Missions, click either Browse All or My Missions. Select the Mission(s) you re interested in, and click Join. Helpful Hint: Each Mission is a bit different, so be sure to read the description. Some Missions may ask you to self-report data or integrate with a device. (See Connecting to a Device on the next page.) Set up any dates/times at which you would like to be reminded about your Mission. Simply populate the hour and minute squares accordingly. Then click Save Reminders at the bottom. To encourage healthier habits, Missions are categorized into four categories: Eat, Feel, Move and Care. To see other recommendations, look in the Browse All section. You can browse or use the filters to find Missions you d like to join. jetblue.werally.com

22 22 CONNECTING TO A DEVICE To use a device such as multiple online, mobile apps and wearable devices (Jawbone, Fitbit, BodyMedia ), to track your activity on Rally, you will need to have an active account for that device. If you have already set up your device, chances are you already have such an account. If you haven t set up your device yet or don t have an existing account, follow the instructions that came with your device. Your Rally account will automatically update your activity. You can always click the gear icon in the top-right corner of the screen and select Device Settings to see which device you have in use, if applicable. You can also disconnect or change your connected device from the same settings location. When you want to check in to the Mission in Rally, simply select the Mission and click Check In. When you join a Mission that is compatible with devices, you ll be asked whether you want to use a device to check in. If you select Device or App, you will be shown a list of compatible devices. Select the device you would like to use, and you will be taken to the manufacturer s website in order to sync the device account to your Rally account. Helpful Hint: You can link your device to multiple Missions; however, you will need to indicate which device for each Mission you select. (The setup process is abbreviated after your initial setup.) jetblue.werally.com

23 23 CHECKING IN TO A MISSION Once you have joined a Mission, checking in and tracking are a breeze. Some Missions require a minimum number of check-ins, so daily check-ins are recommended. If you miss a day or two, no problem. You can check in from either your Dashboard, the Mission Detail page or the Mobile App. Checking in from the Dashboard After logging in, you will automatically be directed to your Dashboard view of Rally. At the top of the page, the Reward Preview will be present; below the Reward Preview you ll see the Missions you have joined. While only three Mission cards will be presented at one time, you can scroll over to the right to view the rest. If you hover over the bottom half of a Mission card with your mouse, the checkin option for that Mission will appear (see image above). Simply enter the option requested and click Check In. Checking in from the Mission Detail page If you d prefer, or if you ve missed a day, you can check in from the Mission Detail page. Clicking the Mission tab or the Mission card from your Dashboard will allow you to access each Detail page. From there you will be given the opportunity to check in for today or any previous day, back to when you originally joined the Mission. The image above shows how you can check in for any particular day by scrolling to the left to get to previous days. This page also shows Mission progress and much more. jetblue.werally.com

24 24 JOINING CHALLENGES Challenges consist of walking and running Challenges where you can compete against others and are awarded Rally Coins as you achieve milestones through the virtual course. A fitness-tracking device is required to join a Challenge. As you walk or run in real life using your fitness device, your distance is applied to the virtual course. To view Challenges, click Challenges within the top navigation bar and view available public step Challenges. To join a Challenge, select Join within the Challenge. If you have already linked a device to your account, you ll be automatically added to the Challenge using that device. If you have not already linked a device to your account, you will be prompted to select from multiple online options, mobile apps and wearable devices. After selecting the device, you ll be prompted to enter your login and password for your device. You will then be prompted to authorize the connection between the Optum Rally site and your device. Authorizing the connection will link the device to your Optum Rally site account. After authorizing the connection, you will be entered into the Challenge. Helpful Hint: You do not need to be in the city of the Challenge to participate. Challenges are virtual. If you have a tracking device, you can walk/run anywhere (even on a treadmill) or sync your distance with the virtual course. As you make your way around the course, you can climb the leaderboard and earn coins that can be used to enter the Rally sweepstakes. The coin breakdown is as follows: Checkpoint every 0.5 miles: 5 coins Achievement every 2.5 miles: 10 coins Trophy Milestone placed at 2.5 miles multiplied by the number of days in a Challenge. So, a threeday Challenge would have the Trophy Milestone placed at 7.5 miles (3 days x 2.5 miles). The Trophy Milestone is worth 50 coins. jetblue.werally.com

25 25 CONNECTING IN COMMUNITIES Communities are places where you can connect with, inspire and support others in their health and wellness journey. There are many available Communities that you can join: topics include food, fitness, allergies, parenting, diabetes and more. Within each Community, you can start discussions, ask questions and connect with other participants having the same health and wellness concerns and interests. All information you choose to share in a Community will be visible to all Rally site users, not just Crewmember users. Select either Browse All or My Communities to view Communities and join any Community of interest. To join a Community, click Join. Once you join, you can immediately start a discussion or share something within the Community. To view and join available Communities, click Communities in the top navigation bar. jetblue.werally.com

26 26 EARNING RALLY COINS Rally rewards you with coins when you complete a task. You can then redeem your coins in a wide variety of sweepstakes for chances to win health- and technology-related prizes such as fitness trackers, gift cards and more. There are many ways to earn Rally Coins. For example, you earn coins for logging in every day, completing the Health Survey, joining Communities, and making progress in Missions and Challenges. For more details, select the Rewards tab at the top of any page on Rally, and then choose View Details. Navigate to the Rewards tab within the top navigation bar to learn how to earn coins. Select Earn More Coins to learn how you can earn coins within the Rally experience. jetblue.werally.com

27 27 ENTERING SWEEPSTAKES To enter a sweepstakes, select the Rewards tab within the top navigation bar. Scroll down to Available Rewards. You ll see a series of images/text that let you know which drawings are happening and how many days you have left to enter. This section also contains the number of coins needed to enter each drawing. Using the gray arrows, scroll to see all available drawings and rewards. Assuming you have enough coins to enter the prize drawing you re interested in, click the applicable drawing square. When you re ready to continue, click Go. You ll receive an immediately confirming your entries. This confirmation will be sent to the address entered in your Rally account. Winners are selected at random and notified by . If you are the winner, we ll ask for a mailing address in the event your reward needs to be mailed. In some cases, the rewards are e-gift cards that can simply be ed. Please allow 8 12 weeks from the time you claim your reward for delivery. If you have not won, don t give up. There are new sweepstake events every two weeks. Keep trying! The entry confirmation will appear. Click Confirm. For a chance to win, simply click Enter. If you want to enter more than once and you have enough coins you ll see a chance to change your number of entries. jetblue.werally.com

28 28 MANAGING YOUR SETTINGS To manage the following: preferences Username and password address Deactivation of account To navigate back to your Rally Health profile, select the gear icon from the top-right corner of any page. Select Profile. To manage your fitness device settings, select the gear icon from the top-right corner of any page and select Settings. Then select Device Settings. Select the gear icon from the top-right corner of any page and select Settings. To change your settings, select the gear icon from the top-right corner of any page. Select Settings from the drop-down menu. At the top of the page, you can change your settings. To change your password, select the gear icon from the top-right corner of any page. 29 29 ACCESSING YOUR HEALTH ACCOUNT For quick access to your Health Account Health Care and Dependent Care Flexible Spending Account (FSA), Health Reimbursement Account (HRA) and/or Health Savings Account (HSA) from the Rally website, click View Healthcare Finances. Helpful Hint: If you have already registered your Health Account, you will not need a separate sign-in process. If it is your first time, you will need to register/create your account. jetblue.werally.com

30 30 USING THE RALLY MOBILE APP To find the Rally app for ios, launch the App Store on your iphone and search for Rally Health. Tap Get to install the app on your iphone. To find the Rally app for Android, launch the Play Store on your phone and search for Rally Health. Tap Install to install the app on your phone. Helpful Hint: You might need to upgrade your phone or operating system to the latest version in order to download the app. If you have an iphone 4s or newer running ios v8.0 or above, you should be able to download the app from the App Store. If you have an Android phone running v4.1 or above, you should be able to download the app from the Play Store. To turn reminders/notifications on or off, launch the Rally app and go to the Settings menu. From Settings, go to Notifications and toggle the control to turn on/off notifications. jetblue.werally.com
