3 ACCESSING MYCHART To access your MyChart account, open your web browser of choice and navigate to Select the MyChart at RiverBend link located under Patient Resources. After selecting the MyChart at RiverBend link, you will be forwarded to the MyChart Home Page. This page provides an overview of the MyChart product as well as a link to access your account. To access your MyChart account, select MyChart Login & FAQ. MyChart Basics Page 2

4 After selecting the MyChart Login & FAQ link, you will be forwarded to the MyChart Login Page. This is the page you will use to log into your account. Enter your MyChart username and password in the designated boxes. Pay special attention to the case you are using. MyChart passwords are case-sensitive. PASSWORD AND USERNAME RESET If you experience difficulties logging in or have forgotten your password or username you may reset your log in credentials by selecting the Forgot Password or Forgot MyChart Username links located in the login box. 5 To begin the username recovery process, enter your first and last name as well as your DOB and last 4 digits of your SSN. If you are unable to remember any of this information, you may contact the MyChart support line at If you have forgotten your password and wish to reset it, you may do so by selecting the Forgot Password link. Once you select the link you will be prompted with your password reset security question. Enter your secret answer in the box. If you do not remember your answer, call the MyChart Support line at Live support is available Monday- Friday 8:30am-5:00pm. MyChart Basics Page 4

6 Once you successfully answer the question, you will be able to create a new password for your MyChart account. Pay special attention to the format and case of your new password. Passwords are casesensitive and must be entered in the exact case they were created in. If you do not have a MyChart activation code or the code issued to you has expired, you can request a new code via the web. Be sure to complete the form in its entirety. Pay special attention to your address, this is where your activation code will be sent. MyChart Basics Page 5

7 ACTIVATING YOUR MYCHART ACCOUNT Once you receive an activation code, you can begin using MyChart. Select the NEW USER button. Enter the activation code you received as well as the last four digits of your SSN and DOB. MyChart uses your SSN for authentication purposes and you must have a SSN on file in order to access MyChart. Next create a MyChart Username and password. Next you will choose a security question and enter a secret answer. This will be used for verification if you forget your password. Make sure your answer is meaningful, but not easy for others to guess. MyChart Basics Page 6

8 MYCHART BASICS When you first login to MyChart, you will come to your personal home page. Your home page will display your name in the top left corner. If new information is available in your MyChart account (i.e., a new test result has been posted, or a new message from your doctor or nurse), an alert will appear in the top box. Click on the alert to see your new information. You can get a snapshot of your health by clicking on My Medical Record, and then selecting Health Summary or you can select the View Your Health Summary shortcut. You ll be able to see, on one screen, information from your medical record such as current health issues, allergies, medications, immunizations, medical and family history, and test results. 9 CURRENT HEALTH ISSUES Health Issues are recurring, long-term health problems, such as high blood pressure or lower back pain. They are viewable in MyChart in two areas; both are listed under My Medical Record: 1. In your overall Health Summary 2. Under Current Health Your doctor has the option to not include all of your diagnoses in this section. If you believe an item is missing, or wish to make changes, you can request changes by sending a non-urgent message to your provider. Information on how to send messages through MyChart will be covered later on in this booklet. MyChart Basics Page 8

10 VIEW TEST RESULTS With MyChart, you can now view your test results online. To view your results, select Results located under the My Medical Record menu. MyChart will only display results from procedures performed at RiverBend Medical Group locations. If your lab services were performed elsewhere, they will not be available in MyChart. Please be aware that lab results will not be available immediately and require the provider sign off on the results before displaying in MyChart. If you have questions regarding the results of a lab or a delay in posting, please contact your provider by phone or by sending a non-urgent MyChart message. Instructions on sending non-urgent messages are covered later on in the manual. MyChart Basics Page 9

11 ALLERGIES Like your Current Health Issues, you can view your Allergies and Medications in MyChart. Click on My Medical Record, and then select: Allergies or Medications. Allergies Click on the allergen hyperlink (underlined) to view additional details regarding the allergen. Your reported reaction will display to the right of the allergen. MEDICATIONS Similar to Current Health Issues and Allergies, you can also view your current medications under the Medications menu. In addition to the name of the medication, you will be able to view dosage, instructions, and the prescribing physician. You will also be able to request a refill via MyChart. To request a refill in MyChart, select the Request a Refill option. MyChart Basics Page 10

12 REQUESTING A REFILL On the Request Rx Refill screen you will see a listing of all current medications prescribed to you. To refill one of these medications, select the box next to the name of the medication. You may also add comments and/or instructions in the box located below your list of medications. This is useful if you would like to pick up your prescription at your local RiverBend Medical Group office. Once you have selected the medication you want to refill, select Continue to move on to the next step. MyChart Basics Page 11

13 We now need to select the pharmacy we will be using. In addition to specifying which pharmacy we will be using, we also want to verify we have selected the correct medication. The selected medication can be found in the first field on the screen (Prescriptions). Once you have verified the correct medication has been selected, we can now select the pharmacy. To find a pharmacy select the drop down arrow in the Pickup pharmacy field. If you do not see your pharmacy on the list, choose Other (specify below) and enter your pharmacy in the box below. You may also choose Other (specify below) if you would like to pick up your prescription at your doctor s office. To do this, enter the RiverBend Medical Group location you would like to pick your prescription up at. MyChart Basics Page 12

14 REFILLING UNLISTED MEDICATIONS If you need to refill a prescription and you are unable to locate it in the medication listing, you may request a refill via MyChart. To begin, select Request Refill Not On Medication List and complete the necessary fields. Once you have completed the necessary boxes, select send. Please be aware that in order to use this feature you must have a RiverBend Medical Group Primary Care Physician. If you do not have a RiverBend Medical Group PCP, you will need to contact your PCP to request a prescription refill. ** Please allow 2 business days (M-F) for all renewal requests. Any requests received after 5 PM will be processed the next business day. Requests received Friday afternoon may not be processed until the following Monday ** MyChart Basics Page 13

15 SCHEDULING APPOINTMENTS IN MYCHART RiverBend Medical Group patients can use MyChart to schedule appointments with their providers. Specialties available are Primary Care, Pediatrics, and OB-GYN. To begin, select the Appointments menu on the left side of your screen. To check if you have any upcoming appointments, select Upcoming Appts. If you do not see an upcoming appointment for your visit type, select Schedule an Appt to schedule a visit. To schedule a visit, select the reason for your visit by clicking the box and selecting the visit type from the drop down menu. Please keep in mind that these visits are limited to Primary Care, Pediatrics, and OB-GYN appointments. Once you select the appointment type, click Continue. MyChart Basics Page 14

16 Verify your address, if the information is correct, check the box indicating your information is correct and select Continue. If your address has recently changed, select Edit Address and enter your updated information before moving on to the next step. Next, select the location at which you would like to be seen. Once you have selected the location, click Continue. MyChart Basics Page 15

17 Choose the provider you would like to schedule a visit with from the drop down menu and select Continue. If you do not see your doctor listed, verify you have scheduled the appointment at the correct location. After selecting the provider and location, we now need to specify a date/time range for the appointment. Enter the date range by populating the From/To date boxes (you may also click the calendar on the right of each box to enter the date range). Once you have entered your date range, select your availability (Morning or Afternoon) and select Continue. ** If you need to schedule a visit and your provider is unavailable, select the Include Other Providers in PCP s Team option by checking the box. This will include all other available providers in that office who share a common specialty** MyChart Basics Page 16

18 After you have entered your availability, MyChart will display a list of available appointments. If you selected the Include Other Providers in PCP s Team option, you will see every provider in that department with open time slots that fall within your specified range. Select a time that works for you and click Continue MyChart Basics Page 17

19 Before your appointment is scheduled, you must verify your appointment information is correct. Verify the Date, Time, Location, and Provider before selecting Make Appointment. Once you have verified everything is correct, enter a comment for this visit (example left) and select Make Appointment. MyChart Basics Page 18

20 VIEWING PAST APPOINTMENTS You may also view past appointments in MyChart. To view past appointments, select the Past Appointments menu. A listing of your previous appointments will display. This listing includes the date and time of visit, provider seen, and department. CANCELING APPOINTMENTS In addition to scheduling appointments, you may also cancel upcoming appointments via MyChart. To cancel an upcoming appointment, select Upcoming Appts and locate the appointment you would like to cancel. Once you have located the appointment you want to cancel, select the row for more detail. 21 You now are viewing the appointment detail screen. Verify all information is correct and that this is the appointment you wish to cancel. If it is, select Cancel This Appointment. Once you select Cancel This Appointment, you will be prompted once more. Confirm that all information is correct and enter the reason for the cancellation. Once complete, select Confirm Cancellation to remove the appointment from your schedule. **Please be aware that this will permanently delete your appointment** MyChart Basics Page 20

22 MYCHART MESSAGING MyChart can be used to send non-urgent medical questions to your provider. This can be done by selecting Inbox located under the Message Center menu. More secure than , this can be used to communicate with your provider when you have a question that doesn t require an immediate response. As previously mentioned, this is for non-urgent medical questions. If this is a medical emergency, call 911. To send a non-urgent message to your provider, select Ask a Non-Urgent Medical Question. Any RiverBend provider you have seen in the past will display in the healthcare team drop down box. Once you have selected the provider, fill in the subject box and type your message in the blank box below. When you have finished typing your message, select send. All responses will then display in your MyChart Inbox. MyChart Basics Page 21

23 ACCOUNT MANAGEMENT You may also update your demographics using MyChart. Address, , and Password changes can all be performed while in your account. Please be aware that changes made to your account can take up to 24 hours before they update. Any changes made in your MyChart account will be reflected in your RiverBend Medical Group medical record. CHANGING YOUR MYCHART PASSWORD To change your MyChart password and security questions while logged into your account, select Preferences from the main menu and then Change Password. MyChart passwords are case sensitive and must meet the following criteria: Must be at least 8 characters in length Contain 1 number, upper case and lower case letter Must not have been used in past 365 days MyChart Basics Page 22

24 PROXY ACCOUNT ACCESS MyChart provides users with access to members of their families medical records as long as they have active proxy access to the account. Any accounts you have proxy access to will be listed under the My Family Records menu which can be found by clicking on the wrench icon at the top of the page. For patients under the age of 18, proxy access will only be granted to parents and legal guardians. Patients over the age of 18 may complete an Adult-to-Adult proxy request form to grant someone of their choosing access to their records. Unlike a minor proxy request, Adult proxies do not expire and remain in effect until the patient removes proxy access from the individual. Anyone with an active proxy to your account will be listed under the Who Can View My Account box. Select the row of the account in which you have proxy access to view their records. Please be aware that in accordance with Protected Health Information guidelines, certain information for minors may be unavailable to users accessing the account through proxy access. Proxy access forms are available at your local RiverBend Medical Group office or under the Family (Proxy) Access section online at Once completed, forms must be returned in person to your local RiverBend Medical Group location with a valid photo ID. Please be sure to complete all required fields on the forms. MyChart Basics Page 23
