1 West Valley High School Athletics ATHLETIC ELIGIBILITY CIF RULES AND District policy require that any student who intends to participate in an athletic contest must comply with special regulations. These rules are not negotiable and will result in game forfeiture if a school/student fails to comply. Therefore, if you plan to participate in high school athletics, please be prepared to complete the following documentation: ALL DOCUMENTS ARE NOW DONE ONLINE (see attached instructions) Please note: If you do not upload your physical and copy of insurance card online you MUST turn in a hard copy of your physical and proof of insurance (photocopy of insurance card) to the Athletic office (along with the top signature page of the athletic packet) in order to be cleared. All other signed documents need to be done online. If you have any questions please feel free to call the Athletic office at If you want to participate in multiple sports please note the instructions attached. You will need to register for each additional sport. It is also required that the following be satisfied: o Enroll in, attend & pass at least twenty (20) units; o Maintain a 2.0 GPA o It is highly encouraged that each student obtain an ASB (Associated Student Body) card (through the Student Store) By signing and completing the online process, you acknowledge and verify receipt of online athletic paperwork and agree to all of the policies and procedures set forth for participation. 2 Online Athletic Clearance Instructions 1. Visit 2. Watch quick tutorial video 3. Register. Parents register with valid username and password. You will be asked to type in a code to verify you are human. If this step is skipped your account will not activate. 4. Login 5. Select New Clearance to start the process. 6. Choose the School Year in which the student plans to participate. Example: Football in Sept 2016 would be the School Year. Choose the School at which the student attends and will compete for. Choose Sport 7. Complete all required fields for Student Information, Educational History, Medical History, and Signature Forms. Upload copies of physical using the upload physical form button and upload copy of insurance card using the upload additional form button. 8. Donate to your athletic program or pay participation fees (private schools only). 9. Once you reach the Confirmation Message you have completed the process. 10. If you would like to register for additional sports/activities you may check off those sports below the Confirmation Message. Electronic signatures will be applied to the additional sports/activities. 11. All of this data will be electronically filed with your school s athletic department for review. 3 Online Athletic Clearance FAQ Multiple Sports Once you complete a clearance for one sport and arrive at the Confirmation Message, you will have the option to check off additional sports/activities for the current school year. If you decide to participate in an additional sport/activity later on, you can access the multiple sport check boxes by clicking on the Confirmation Message of your original Clearance for that specific year. Physicals The physical form your school uses can be downloaded on Step #1 (and attached to this packet). You may upload a copy of up-to-date physical, signed and dated by doctor, during the online process. If you are unable to upload a copy, please turn in a hard copy to the athletic department, along with the signed top page of this packet. Proof of Insurance Proof of insurance is required. You may upload a copy of insurance card (with athlete s name on it) during the online process. If you are unable to upload a copy, please turn in a hard copy to the athletic department, along with the signed top page of this packet.
