4 Introduction DeJarnette Research Systems, Inc. supplies a unique, high value, low cost, solution to a long-standing PACS problem. High speed, multi-slice CT scanners easily produce multiple procedure studies in a single acquisition. In a PACS environment, these multiple procedure studies are matched to a single procedure. This causes several problems: 1) The radiology department must work outside the normal PACS workflow to bill for the additional procedures. 2) The modality worklist presented to the CT scanner contains multiple entries one for each procedure and is difficult to clear out. 3) When it s time to pre-fetch a multiple procedure study as a prior examination, there is no information regarding the other procedures to help an intelligent pre-fetch algorithm. dysect identifies multiple procedure studies, analyzes the images and separates them into the correct anatomical regions, and automatically associates those images with the appropriate procedures. dysect performs this identification, analysis, and association automatically, without manual intervention. How dysect Works dysect connects to any DICOM network via TCP/IP. It periodically requests a DICOM modality worklist from the PACS, identifies any multiple procedure studies, replaces the individual procedures with a grouped procedure, and presents the modified DICOM modality worklist to a connected CT scanner. dysect acts as a DICOM CT Storage Service Class Provider for the connected CT scanner. It receives images from the CT scanner and matches the images to an entry in the DICOM modality worklist. If an acquisition does not match any entry in the worklist, then dysect moves the images to a preconfigured QC destination. If an acquisition matches an entry in the worklist and the entry is a single procedure study, the images are moved to a configurable destination in the PACS. If dysect is configured to add or change the Gray Scale Presentation State with the addition of a VOI LUT, then the configured window center and width for the single procedure are added to the DICOM headers of each image before the images are routed to the PACS destination. If an acquisition matches an entry in the worklist and the entry is a multiple procedure study, dysect sorts the images in slice position order and uses the modality worklist information to analyze each image. dysect separates each image into a specific anatomical region and associates each image with a particular procedure. dysect can assign an image to multiple procedures depending on the configurable overlap entered by the site. The images associated with each procedure are given the accession number assigned to the procedure and placed in a new study. dysect replaces the study UID inserted by the CT scanner with the study UID from the RIS. If dysect fails to separate a multiple procedure study, then it moves the images to a preconfigured QC destination. If dysect is configured to add or change the Gray Scale Presentation State with the addition of a VOI LUT, then the configured window center and width for each procedure are added to the DICOM headers of each image before the new study is routed to the PACS destination. dysect stores its configuration on a USB memory key that can be moved from one dysect to another. In this way, a single configuration can be replicated easily on multiple dysect devices by loading a configuration from the USB memory key. Also, if a dysect device is down, it can be replaced quickly by swapping the hardware and loading the configuration. 4

5 Interacting with dysect The graphical user interface presented by dysect is a web interface that can be accessed from any PC on the network running Microsoft Internet Explorer. Once a user logs in, the dysect web server presents four main pages to the user. Clinical Setup From the clinical setup page, the user can configure the following: Whether or not dysect will add Gray Scale Presentation State in the form of a VOI LUT. A specific VOI LUT can be configured for each CT procedure. A study overlap in mm at the top and bottom of each procedure. A list of grouped procedure descriptions and the individual procedures that belong to each group. DICOM Setup From the DICOM setup page, the user can configure the following: The AE titles and ports for each DICOM service that dysect provides: CT Storage SCP and DICOM Modality Worklist. The AE titles, ports and IP addresses for each DICOM service provided by the PACS: CT Storage SCP for the main destination and CT Storage SCP for the QC destination and DICOM Modality Worklist SCP. The user can also display dysect s DICOM Conformance Statement and User Guide. User Accounts From the user accounts page, users can be added, modified or deleted. Permissions can be specified depending on the type of user group. Service From the service page, the user can configure the following: Modality worklist polling interval. If the interval is too long, the CT scanner may not receive timely updates of the worklist. If the interval is too short, dysect may slow the PACS with constant requests for a worklist. Minimum allowed disk space. A CT study remains on the hard drive until it is successfully delivered to the PACS destination. So, if the PACS is down, studies will begin to accumulate on the hard drive until the minimum allowed disk space is reached. At this time, dysect will refuse to store images from the CT scanner. Study time-out interval that dysect uses to determine whether a study has completed. Whether or not to automatically group procedures by parsing the procedure description for anatomical regions. The user can save to or load configurations from the USB memory key. The user can also display the current DICOM modality worklist presented to the CT scanner and inspect event logs. 5
