1 2015 PQRS File Creation/Submission Guide Prior to submitting PQRS files A. Review the set-up considerations B. Create your PQRS Files Refer to your vendor specific user guide for creation of PQRS files Be specific in how you name and where you save your files C. Test the system by running the submission engine validation tool Set-Up Considerations The minimum system requirements to effectively access the Submission Portlet are listed in the following subsections: Hardware Requirements Computer with a 1 gigahertz (GHz) processor with minimum of 150 MB free disk space Windows-7 with 512 MB Memory Software Requirements Microsoft Internet Explorer Version 11.0 Windows-7 operating system WinZip version 7.0 (or compatible zip programs using default compression settings) for Zip file creation to upload data Adobe Acrobat Reader Version 5.0. To download a free copy from Adobe Acrobat Reader Version 5.0, click the Adobe Acrobat Reader link on the Adobe website ( Internet Connections The PQRS Submission Portlet will be accessible via any Internet connection running on a minimum of 33.6k or high speed connection. Pop-Up Blocker Browser Settings Internet Explorer s Pop-up blocker feature must be disabled in order to use the Submissions Portlet. 1. Open Internet Explorer Browser. The Internet Explorer Browser contains a Tools drop-down menu where certain settings, such as the Turn off Pop-up Blocker feature, may be enabled 2. Click Tools on the main menu bar located at the top of the browser window 3. Select Pop-up Blocker from the Tools drop-down menu 4. Select Turn Off Pop-up Blocker to disable the Pop-up Blocker setting from the browser window 1

2 Testing the System Log in to the PQRS Portal After logging in to the PQRS Portal the Submission Engine Validation Tool hyperlink appears on the welcome screen Submitting a File for Validation 1. On the home page, in the Participation Year field, default selected would be In the Submission Type field, select the Submission Type from the drop-down for the file that will be validated. Submission Types for 2015 are Registry XML, QCDR XML, QRDA Category 1, and QRDA Category 3 (If you are unsure of the correct submission type, contact your vendor.) 3. In the File field, click Browse to attach the file (see Step B) The SEVT accepts individual XML files with a maximum of 10 MB file size. The tool does not accept ZIP files 4. Click Validate to process the file 2

3 File Validation Passed If the submitted file passes validation, a green-bordered box appears containing the message The file passed validation. with a link to validate another file To validate another file, click the Validate Another File link. The screen is refreshed and the validation form fields and the validation results display boxes are cleared For 2015 participation year QRDA Category 1 or QRDA Category 3 submitted file validation, a yellow-bordered box containing an warning icon, the message The file passed validation with (#) Warning(s), along with a link to validate another file appears. To view the detailed validation results, click (#) Error(s) and (#) Warning(s) Returned. A window displaying the number of Errors, the number of Warnings, and the warning messages appears. This tool shows a maximum of 100 Error and Warning messages. File Validation Failed If the submitted file fails validation, a red-bordered box appears containing an error icon, the message (#) Error(s) Returned, and a link to validate another file. The tool will display a maximum of 100 errors. To validate another file, click the Validate Another File link. The screen is refreshed and the validation form fields and the validation results display boxes are cleared. For all Submission Types, a window appears containing a three-column table listing each error message with line number, error code and message detail of the file that caused the error. For 2015 participation year QRDA Category 1 or QRDA Category 3 submitted file validation, a red-bordered box containing an error icon, the message (#) Error(s) Returned, and a warning icon, the message (#) Warning(s) Returned, along with a link to validate another file appears. 3

4 PQRS QRDA File Submission Log in to the PQRS Portal After logging in to the PQRS Portal the Submissions hyperlink appears on the welcome screen. Click the Submissions link from the Site Navigation. You will be taken to a Warning screen Check the box next to the I Accept the above Terms and Conditions language, and click the I Accept button. You will be taken to the Submission portlet. Using the PQRS Submission Portlet The Submission Portlet is used to submit files to PQRS. You must have the PQRS Submitter, Individual Practitioner, or Individual Practitioner Representative role to submit files. You are able to submit the following four Submission type files: Registry Qualified Clinical Data Registry (QCDR) Quality Reporting Data Architecture Category 1 (QRDA-1) (Patient Level) Quality Reporting Data Architecture Category 3 (QRDA-3) (Aggregate Level) 1. Within the Submission Portlet, click the Browse button to Select Document(s) 2. Navigate to the location on your computer and select the data file to be uploaded. Click the Open button. XML/ZIP format file sizes must be greater than 0 bytes and may not exceed 20 MB 3. The file(s) selected appear in the Select Document(s) field. You can perform any of the following actions: Click Browse to select another file Click Reset to clear the selected file from Select Document(s) text Verify the correct file(s) to be submitted was selected and then click Upload 5. If Upload was selected, a confirmation dialog box appears. Click OK to confirm the request or Cancel to decline 6. If OK is selected, a progression window appears showing the upload status 4
