1 Frm Filing Instructins Subscribers creating nline frms fr the first time must cnfirm/set parameters n their cmputer. Refer t the Technical Setup Instructins at the end f this dcument befre cntinuing with frm creatin. Frm Creatin Instructins The fllwing 5 steps must be cmpleted t create and submit nline frms t ISS: 1. Dwnlad & Save Frm 2. Cmplete Frm 3. Save Frm 4. Clear Frm fr Reuse (Optinal) 5. Uplad Frm Dwnlad & Save Frm Navigate t the apprpriate page based upn the specific frm needed. Select the frm t be cmpleted, and dwnlad the frm by right-clicking n the link and saving the assciated file t yur cmputer. Nte: the Adbe Web Viewer brwser plugin can be disabled in rder t dwnlad PDFs by default, rather than pening them in yur brwser. 2 Cmplete Frm Enter all required fields indicated by a * Enter nly nn-zer amunts - fields with n amunts t reprt shuld be left blank. Fr acknwledged states withut data, a separate NTR frm must be cmpleted Cmplete the preparer infrmatin at the bttm f the frm - Cmpany Name, Preparer s Name, Preparer s Title, Preparer s Phne Number (with extensin if applicable), and Date f Entry Fr refiles, cmplete the entire frm with all apprpriate amunts, nt just amunts that have changed. Save Frm Frms that are nt cmpleted in ne sessin can be saved and repened at a later time. Simply chse the File Save As ptin, name the frm and select the netwrk lcatin. Wrk in prgress shuld be saved withut clicking Secure. Once the frm is cmplete and ready fr submissin t ISS, the fllwing steps shuld be taken: Secure the frm frm future editing by clicking the Secure buttn. The status assciated with Secure will change and the Secure buttn will n lnger be displayed. Once secured, save the file as described abve. Clear Frm fr Reuse In the event the same frm is needed fr anther cmpany, such as an affiliate, it is nt necessary t pen a new frm. The cmpleted frms can be easily reused. Click Reset, This results in the: Clearing f the frm fields, except preparer infrmatin Frm status being reset t Nt Secured fr Uplad! Secure buttn being reactivated Repeat the abve steps t Create, Secure and Save the frm until all cmpanies are cmplete fr a line. 2

3 Uplad Frm Frms are submitted individually thrugh the ISS website. T submit a frm click the Uplad Frms buttn n the right side f the page. The user name, cmpany name, and date will be pre-defined based upn yur lgin infrmatin. Select the frm name t uplad frm the list prvided. Brwse yur netwrk and select the pdf file saved earlier. After all fields are cmplete, click the Submit buttn. Cntinue t brwse, select, and submit frms until all have been upladed. 3

4 Technical Setup Instructins Perfrm the fllwing setup instructins ne time t any PC used t pen, cmplete, and submit frms t ISS. Fr cnfirmatin f the release level and settings in yur installatin, cntact yur netwrk administratr. Technical Requirements JavaScript shuld be enabled in the brwser and nt blcked by any firewall r anti-virus sftware. The frm shuld be dwnladed frm the website then saved, pened, and cmpleted in Adbe rather than in yur web brwser. T d s, right-click n the frms VIEW link and chse save link as r save target as. If cmpleted in yur brwser, the interactive frm may be saved as a blank cpy. The Adbe Web Viewer plugin can be manually disabled. Fllwing these steps will result in all PDF frms t be dwnladed and/r pened in Adbe instead f in-brwser, including thse utside f the ISS website. Refer t the brwser-specific instructins belw t disable Adbe Web Viewer. Disable Adbe Web Viewer in Internet Explrer: Click n the Tls menu and select Manage add-ns. Lcate, select, and disable Adbe PDF Reader. If Adbe PDF Reader is nt in the list f add-ns, chse Shw: All Add-ns. With the plugin disabled, clicking n a frm link shuld dwnlad and pen the file in Adbe. 4

5 Disable PDF Viewer in Chrme: In the navigatin bar, type chrme://settings/cntent/pdfdcuments and hit enter. Only ne ptin shuld be displayed, Open PDFs using a different applicatin. By enabling this ptin, Chrme will dwnlad PDFs and pen in Adbe. Nw, clicking n a frm link will dwnlad the file. These shuld appear at the bttm f yur brwser and in yur Chrme dwnlads flder. 5

6 Disable in-brwser viewing in FireFx: In the navigatin bar, type: abut:preferences#applicatins and hit enter. Lcate the cntent type Prtable Dcument Frmat (PDF) and chse Always Ask in the Actin menu.
