How It Works: A Healthier You Biometric Screenings Overview 1. Eligibility: Blue KC group medical insured subscriber and spouses are eligible to participate in the onsite event. 2. Scheduling Policy: A minimum notice of six (6) weeks is required prior to a booked screening event. Booked Screenings with less than 6 weeks advanced notice will be considered on a case by case basis and will depend on staff availability. 3. Participation Policy / Adjustments to Estimated Participation: A minimum participation of twenty (20) participants per event is required. Client will be responsible for at least this number. Supplies and staffing are provided for the minimum number established plus an additional 20%. Client can adjust projected minimums prior to 10 calendar days before the scheduled screening event. 4. To begin planning for your onsite event, the following information is required: Group Name Event Location Address Contact information per location Number of total employees onsite per location Number of group medical Blue KC subscribers & spouses Event Start Time 5. Cancellation policy: If Client cancels a Booked Screening inside 10 calendar days of the scheduled event, Client will be responsible for reimbursing Blue KC 100% of the charges and expenses payable to Blue KC for such Booked Screening. 6. Event Duration Policy: Events considered as a standard event require a minimum of five (5) hours of screening time excluding setup and breakdown times. If less than five (5) hours is requested by the client, additional fees may apply. If client requests event hours or staffing additions outside of the recommended structure, additional fees may apply. 7. Split Shifts: Two events on the same day and location separated by an amount of time greater than one (1) hour is treated as detached events with each holding to the respective event minimum of 20 Participants. 8. Premium Hours: Booked screening events on weekends, federal holidays, and between the hours of 7pm and 7am, will be subject to an additional fee billed to Client. 9. Non-Blue KC Group Medical Plan Participants: Client will be invoiced $45 per participant for individuals not active on the Blue KC group medical plan on the date of the event attended. If client allows walk-ins on the day of the event, we recommend participants bring their most recent Blue KC member ID card to show proof of insurance; otherwise, they may be counted as a non-member and a direct invoice will be issued. *Standard non-fasting finger stick package - Total cholesterol, HDL, glucose, blood pressure, height, weight, waist circumference, and BMI. 2017 Blue Cross and Blue Shield of Kansas City is an independent licensee of the Blue Cross and Blue Shield Association.

MANUAL SUBMISSION FORM (PHYSICIAN FORM) Members can access the Manual Submission Form by following the same registration steps listed below. To submit a completed form: Members must complete all fields Members must submit lab values Members must obtain their physician s signature on the form Members must submit signed consent (either in person at the event or via electronic signature when downloading the MSF online) Automatic confirmation emails will be issued to the member within 48 hours to confirm receipt of a completed form and any required necessary documentation. If the form is missing information that requires action from the member an email will be sent to the member highlighting the missing information and requesting the member resubmit. Monthly reminder emails will be sent until the program deadline to the members to obtain the missing information. Once the wellness program year ends, any individuals who have not submitted missing information will be closed out as incomplete. Members who opt for the Manual Submission Form will be required to do fasting bloodwork via venipuncture from their physician s office. This is a quality control measure designed to validate individuals results. Blank forms cannot be distributed. By registering for their form online, members electronically sign a consent that allows their results to be posted to their AHY portal. REGISTRATION OPTIONS: Once the screening event is booked, AHY will provide a marketing flier to promote event details. Once you receive the flier, you may start advertising your screening event to your participants. Participants have multiple options to register for an appointment. In order to help ensure proper delivery of emails and easy access to the registration site, please have your IT System Administrator whitelist the following email address and URLs: Whitelist the following URLs: bluekc.healthmine.com https://sso.healthinsight.com https://scheduler.ehealthscreenings.com Whitelist the following email domains: healthmine.com service@ehealthscreenings.com

Online Registration AHY Portal: Blue KC members can register for their onsite screening appointment or access the Manual Submission Form (physician form) by visiting our A Healthier You portal, accessible through the member portal on MyBlueKC.com. Use Google Chrome when accessing the AHY portal. Available to Blue KC subscribers and spouse. Members will be required to answer initial onboarding registration questions when accessing the AHY platform for the first time. Responses to these questions will personalize the site to the individual. These questions are not associated with the Health Risk Assessment. Following completion of the onboarding questionnaire members can access the Biometric Screening link to schedule their appointment or access the Manual Submission Form (Physician Form) Blue KC A Healthier You App Registration: Download the Blue KC A Healthier You app from the Google Play store or Apple store. Available to Blue KC subscribers and spouse. Individuals will need to register as a new user through the app since it is not affiliated with MyBlueKC.com website login information. Blue KC Member ID card information is required to register on the app.members can email AHY@BlueKC.com for assistance with online or portal registration. Once registration is completed, members will receive a confirmation email containing pre-populated and consent forms. Participants will be prompted to print these forms and bring with them to their scheduled screening appointment. Password to access the screening paperwork is contained in the email.additionally, members will receive a reminder email the day before their appointment including the forms with instructions to print all forms. Members also have access to print their forms through the system any time prior to their scheduled appointment. MONITOR REGISTRATION: Track appointment signups! As an administrator, you can check the registration progress of your event by logging into the admin section of the online administrative tool. You will be provided your group specific log in information by your Blue KC Health Event Screening Specialist. Contact AHY@BlueKC.com to learn more about monitoring registration!

DAY OF EVENT: 1. Watch for additional information coming from your Blue KC team! Four to five days before your screening event(s) you will receive a pre-event email from your A Healthier You Event Specialist. It will contain information on the on-site screening staff, confirm set-up times, number of table and chairs needed, etc. If supplies are being shipped to you, your pre-event email will also include shipping tracking numbers so you can monitor their status and confirm delivery. 2. The day before the event you will receive the sign-up sheet with the daily appointments and blank paperwork. The morning of the event, print a copy of the daily appointment schedule for the Lead Screener. Screening staff will bring extra blank registration forms with them; however if more people than expected forget their pre-populated form or there are more walk-ins than expected you may be asked to print additional copies. 3. The Lead Screener will arrive 45-60 minutes prior to each event. A representative from your location must meet the technicians and show them to the appropriate screening room(s). 4. Once the screening event is finished, technicians will pack up everything they brought with them along with the completed participant surveys, clean their areas and remove all waste. The Lead Screener will alert you when they are leaving the premises. 5. If you have chosen to allow walk-ins at your event, please communicate to employees that they will need to bring their most recent insurance card with them to the event. Doing so will ensure the individual completes the process and their data is correctly uploaded to the AHY portal. 6. If a member should miss their appointment, they will be required to call in to ehealthscreenings Customer Service (888-708-8807, opt. 1) to reschedule. They may be instructed to walk in to a different event or access the Manual Submission Form if there are no remaining events. Please note that participants will see a message on their portal indicating that their results are currently being evaluated, even if they have not yet submitted any results. GETTING THE BEST PARTICIPANT EXPERIENCE: 1. Please have participants arrive no more than 5-10 minutes prior to their scheduled screening time. This shortens wait times, time away from work and assists with event flow. 2. Upon participants arrival, they will check in If the participant is on the schedule technician will check to ensure they have their pre-populated form or provide them a blank registration form to fill in manually. If the participant is not on the schedule registration staff will determine their ability to work them into the schedule at that time or assign them a time to come back. 3.Following check-in, participants will be directed to a screening station as soon as one is available. Please note that the Health Risk Assessment (HRA) is no longer required to be completed at the onsite screening event. Members are encouraged to complete on their own via the portal or app.

PARTICIPATION REPORTING: Question Answer Can I receive completion data from Blue KC? Yes, A Healthier You can provide single lists of biometric screening including onsite attendance and the Manual Submission Form (physician form) completion for groups who tie incentives to the biometric screening How frequently will I receive Biometric Screening lists to support my communication campaign? Once you have confirmed your incentive requirements and timelines with AHY we will begin providing you with monthly lists that reflect individuals that have not yet completed the biometric screening. You can use this information to send targeted messaging to these specific individuals to ensure they complete the biometric screening by your specified deadline. What member identifying data is contained on the reoccurring biometric screening non-completion list? The biometric screening non-completion list will include the following: First Name Last Name Date of Birth Member ID What if a member has completed the HRA but they are reflected on the non-completion list? Our data is not real time data but does represent a snapshot in time. Typically an individual may still be reflected on the list due to timing of completing the biometric screening including submitting the Manual Submission Form (physician form). Members can confirm their activity completion by visiting the A Healthier You portal to view their biometric screening health action, message center and/or points awarding them for the activity. When will I receive a final list? AHY will provide you with a final list of who has completed the biometric screening one week following your deadline.
