Meaningful Use Modified Stage 2 Related Objective: Patient Electronic Access (VDT): Provide patients the ability to view online, download and transmit their health information within 4 business days of the information being available to the EP. (Measure 2) Patient Electronic Access Measure 2 for an EHR reporting period in 2015 and 2016: At least one patient seen by the EP during the EHR reporting period (or patient-authorized representative) views, downloads or transmits to a third party his or her health information during the EHR reporting period. Patient Electronic Access Measure 2 for an EHR reporting period in 2017: More than 5 percent of unique patients seen by the EP during the EHR reporting period (or his or her authorized representative) view, download or transmit to a third party their health information during the EHR reporting period. When email information is collected and entered on the profile during registration for the patient and/or an authorized representative, they will receive an email with a link to register for Patient Portal access. If the patient or authorized representative states they did not receive an email, then confirm the correct email and perform a portal reset (see the Patient Portal Invitation via Email tip sheet). Select Register. Enter Date of Birth, Email Address and Phone Number. The Profile Number will pull from the link. Select three security questions and provide answers. These are needed in case the password is forgotten. Select this link to access Portal Main Menu. NOTE: If Exclude from Portal is checked on the patient account, that visit will not display on the portal and that account will only count in the denominator for Measure 1.
