General Information Website and Login Open a web browser and go to the web portal site https://caredx.careevolve.com. Login with your username, password, and location (UNOS center code). If you have forgotten your login credentials, follow the online prompts to reset or retrieve your password, username, or location. If you have any further questions about your login, contact Customer Care by calling the number 1-888-ALLOMAP (1-888-255-6627), or by clicking the blue link Contact Client Services or using the email address: customercare@caredx.com. If there is no email address stored in your CareDx Customer Web Portal User Account, you will be asked to enter your email address after your 5 th login. Either enter your email address or click the Continue link to ignore. You will be prompted 3 more times in the future (after the 15 th, 30 th, and 45 th login), and if you choose to ignore the message after the 45 th login, you will not be asked again. EULA If this is the first time logging in, you will be required to accept or decline an enduser license agreement (EULA) to enter CareDx Customer Web Portal. Unless it is modified and published again by an administrator, you will only see the EULA once. Logout Manually logout of the application at any time by clicking the Logout link, which will always be located at the top of the page. You will be logged out automatically if the application is idle for a time pre-determined by the laboratory. Announcements To view general announcements from the lab, click the Announcements link. If no Announcements exist, the link will not be visible. CareDx Customer Web Portal User Guide Version 3.6.3 4

Patients It is necessary to have added some patient details in order to proceed. Follow the steps below to add a patient. Overview of Patients Tab Header The Patients tab supplies patient information and maintenance functions for existing patients, as well as the ability to add new patients to the system and merge patients. List Patients The List Patients link will display the A-Z search bar. Use the A-Z bar along the top of the patient window to move directly to patients whose last names begin with that letter. The total number of patients is listed on the right side of the patient window. You may move between pages quickly by using the forward or back arrows on the right side of the window. Each patient row contains the following columns: Name, Office ID, Gender, DOB, and SSN. To view a patient s Order/Result List, click on their Name from the List Patients page. Add Patient Step Action 1 Click the Add Patient link under the Patients tab header. CareDx Customer Web Portal User Guide Version 3.6.3 6

2 The Add Patient page will display. It is divided into four main areas: Demographics, Guarantor, Insurance-1 and Insurance-2. All fields with red text are required and must be completed, or the new patient information cannot be saved. By default, Self will automatically be populated into the Guarantor field, as seen below. (Note: Visible and required fields are customizable.) 3 When all of the information has been entered, click Save. To cancel the new patient, click the Cancel button. To go directly to placing an order, click Save & Place Order. See Orders section below for details. CareDx Customer Web Portal User Guide Version 3.6.3 7

Quick Search A patient search is required prior to creating a new order. Step Action 1 Locate the Search box at the right side of the Lab Center Header. 2 Type in the Search box either the patient s ID, or last name (e.g., Smith), and click the Go button. The search will locate any matches beginning with the string of text or ID. 3 A list of matches will display, from which you may make a selection if more than one name is offered. Advanced Search Follow these steps to use the Advanced Search feature to locate a patient. Step Action 1 Click on the Advanced Search link on the Lab Center Header menu. 2 The Advanced Search window will open. Complete or partially complete as many fields as you want, and click the Search button. CareDx Customer Web Portal User Guide Version 3.6.3 8

3 A list of matches will display, from which you may make a selection. Click the Start Search Over link to return to the Advanced Search. CareDx Customer Web Portal User Guide Version 3.6.3 9

Orders Placing Orders All required patient details must be entered before placing an order for a test. New Order: Overview Workflow After a patient is selected and the new order is started, the general workflow of the ordering process is as follows. Note: Each area of the process is defined in detail with graphics in the following pages of this guide. 1. Search for patient name using search box indicated below (e.g., Wilson), then hit GO to get Result Details for the desired patient: CareDx Customer Web Portal User Guide Version 3.6.3 10

3. From New Order form, select Order Type (Standing or Draw Now) and Bill Type (Client, NonPatient, or Third Party), these may be selected from this ordering page, as described in greater detail later in Order Type Overview section. CareDx Customer Web Portal User Guide Version 3.6.3 12

Order Type Overview There are two Order Types which may be created, with Standing Order as a default and Draw Now. (Draw Later, although offered, is not applicable at this stage.) Standing: Draw Now: A Standing Order spans multiple dates. A calendar tool is available in which future dates are selected. A Standing Order consists of a template order and one or more individual orders which have Incomplete status until the collection date when they are reopened and completed. The template order is not required but is useful for editing of the current standing order pattern or for repeat use of a specific standing order testing pattern. Draw Now orders are orders with today as the collection date. The order requisition and label (if applicable) are generated immediately. The order has a status of Complete. Draw Later: is not applicable to this version of the Web Portal. Bill Type Overview Available Bill Types are specific to the location. In some cases, if there is only one Bill Type available, the dropdown will be grayed out. Common Bill Types include Client, NonPatient and Third Party. Ordering Scenarios Order Type Once the Order Type and Bill Type have been determined, the page will refresh and ordering tabs will appear with the Demographics tab open. Use the grid below to review how the Order Type and Bill Type affect the remainder of the order. Ordering Tabs Standing Draw Now For example, if Order Type selected is Standing order and Bill Type is Third Party, then tests are ordered after the Demographics box has been clicked and updates made to all categories, Update checked, and then Tests box appears clicked, and a Search Tests screen will appear as in the example below. The following additional diagnoses were obtained by adding check marks and clicking on Next Scheduling Tests Checking the boxes next to the diagnostic tests reveals additional test results, which may be check-marked, then click Next to reveal the Schedule where certain dates for testing may be entered as shown below. Submit Schedule (below a drop-down box for selection of weekly or monthly visits) is clicked after selecting the dates (e.g., close to the 22 nd of each month here) of future visits in the following depiction. A summary template of selected, scheduled monthly visits for Standing Orders is shown as follows: To complete Standing Order, click on Return to Patient Chart, and follow the Draw Now workflow. All required Patient Demographics fields will appear in this section. If all required fields are complete, there will be a green checkmark following Patient Demographics title. Any fields appearing in red font (after clicking Edit) need to be completed before proceeding with the order. CareDx Customer Web Portal User Guide Version 3.6.3 17

Action Complete any missing information indicated by a red field. For example, enter information for Date of Birth: Action Click the Edit button to change any of the fields; then hit the Save button so that these changes will be saved to the patient s demographics permanently. Guarantor All required Guarantor fields will appear in this section. If all required fields are complete, there will be a green checkmark after Guarantor title. Any fields appearing in red need to be completed before proceeding with the order. Action Complete any missing information indicated by a red field. Insurance-1 All required Primary Insurance fields will appear in this section. If all required fields are complete, there will be a green checkmark or a red exclamation mark following Primary Insurance -1 title. Action Complete any missing information indicated by a red field. For example, enter Primary Insurance-1 and Policy ID # in the appropriate boxes: Action Click the Edit button to change any of the fields, then click on the Save button so that these changes will be saved to the patient s demographics details permanently. Insurance-2 All required Secondary Insurance-2 fields will appear in this section. If all required fields are complete, there will be a green checkmark or a red exclamation mark following Primary Insurance -2 title. Any fields appearing in red need to be completed before proceeding with the order. Action Click the Edit button to change any of the fields, then click on the Save button so that these changes will be saved to the patient s demographics details permanently. Adding More, or Editing Standing Order Tests Go to the Orders/Results List within a patient s chart, and then select Template #. CareDx Customer Web Portal User Guide Version 3.6.3 21

Ordering Draw Now Tests The first tab in the ordering process for Draw Now with Bill Type of Third Party shows the Demographics tab, which has a similar format and process as the standard order. Based on the Bill Type, there can be up to 5 areas on the tab. I. Order Information II. Patient Demographics III. Guarantor IV. Insurance 1 V. Insurance 2 Having searched for a patient by entering the first few letters of the name, select Search, such as w for last name for patient, James Wilson. Once you find the patient you are seeking (e.g., James Wilson), you may select a New Order after you locate the Draw Now selection button shown in the following window: CareDx Customer Web Portal User Guide Version 3.6.3 23

The following page indicates in red where information needs to be added. Update required information and hit Save. Click Tests button that will highlight Tests in orange, then select tests such as 100 AlloMap HTx test and possibly a repeated test name will appear below the window requesting you to also select it, using a check-mark. The next window that appears requires you to click on the Diagnosis tab highlighted in orange, then click on diagnoses, such as V42.1 Transplant Heart and other diagnoses that are relevant to the patient. Hit Next button. Note that there is a button called Save Draft that allows draft of diagnoses to be saved. CareDx Customer Web Portal User Guide Version 3.6.3 25

Hit the Next button to set up a review that summarizes details, hit Complete to end. Outstanding Orders Inbox Outstanding orders are orders which have been manifested but have not yet resulted. Once resulted, orders with a status of Final or Corrected Final will automatically drop off this list. Orders with a cancelled status are unaffected and will remain on the list. Orders may also be removed manually, in the case that order codes don t match result codes, or an order has a cancelled status. CareDx Customer Web Portal User Guide Version 3.6.3 26

Step Action 1 Select the Outstanding Orders Inbox link under the Orders tab header. 2 A list of all outstanding orders for your location will be displayed on the screen. The date range is based on the user s result history threshold. 3 You may Sort the outstanding orders by any column. Click on the column name to sort in ascending order. Click the column name a second time to sort in descending order. 4 View and Print any of the orders in the list by clicking the print icon to the far right of the order. 5 Remove one or more orders from the list by checking the box to the left of the order, and clicking the Remove Selected button at the top of the list. Print the entire list using the Print icon. Manifesting Orders New Manifest This link shows all pending orders in the location for the past 14 days. If there are no orders for the past 14 days, it will default only to a range in which there are orders (but not more than 14 days). You may select any or all orders to place in a manifest list, which may then be viewed or printed from the Manifest List page. Step Action 1 Click the New Manifest link under the Orders header. 2 A list of all pending orders for your location will appear on the screen. To expand or narrow the list, drop down the arrows for the Lab, Requisition Type, or Location. 3 Initially, all pending orders will be selected, as indicated by the check in the box to the left of the order. You may de-select all orders by unchecking the box at the top of the check box column. CareDx Customer Web Portal User Guide Version 3.6.3 29

4 Once you have all of the pending orders selected that you want to appear in your manifest list, complete the process by clicking the Print Manifest button at the bottom of the page. 5 A new window will open displaying the manifest details. The Print dialog box will open for immediate printing. Note: the physical printing of the manifest is can be printed to match your workflow but it is not required to complete the ordering process. 6 If you cancel out of the Print dialog box, and then want to print the page, right-click on the page and select Print. Order/Result List Once a patient has been selected, the Order/Result List for the patient will be displayed. Place the cursor over the Order Numbers to quickly review tests included in that order. This patient has one complete AlloMap report. Click on Chart Tools in screenshot above and select the drop-down choice of Order/Result List to get the following screen. Similarly, place the cursor over the Report Date or Requisition Number, shown in red circles above, and click once to quickly view brief order details (see above) or click on the same Requisition Number for results of the test completed from the order as shown in red circles (see below). Next, you may double click Requisition Number in the screenshot above, indicted by the red arrow, to obtain a detailed report to save, print or email, as shown in the following screenshot. From this screen and other screens for the same patient, you may also view the patient s Demographics, or a Cumulative Report may be generated. CareDx Customer Web Portal User Guide Version 3.6.3 32

Results Overview The Results Inbox link under the Results tab displays recent lab results. The Results Inbox displays a predetermined number of day s worth of results, referred to as the Results History Threshold. Results are organized by result report date in descending order. If there are any alerts, they will be grouped together at the top of the list of results. Alerts are customized ranges set up in your system. You may sort by column headings. Click the heading once to sort in ascending order, and click again to sort in descending order. CareDx Customer Web Portal User Guide Version 3.6.3 33

Column Name Description Name Sorted by patient last names. Date Resulted date. Requisition Order identification number. Lab Laboratory where test analysis is performed. Flag Always will be set to N. Alert Y or N to indicate a center-defined alert value. Alert results will automatically appear at the top of the Inbox results list. Status Defines the status of the results. F=Final, C=Corrected Final Provider Ordering provider. Viewed Y or N to indicate that the result has been viewed. Reviewed Y or N to indicate that the result was marked as reviewed. Printed Y or N to indicate that PDF result format has been displayed. (May or may not have been actually printed.) CareDx Customer Web Portal User Guide Version 3.6.3 34

View Result Details Page Follow these steps to view Results Inbox in HTML format. Step Action 1 When the cursor is placed over the patient s name in the Results Inbox, the tests included in the result will appear in a pop-up or bubble box. Test names in red and with a red exclamation point indicate a custom alert. If the result is an image report, the pop-up bubble will show the Collected Date/Time, Received Date/Time. 2 From Results Inbox, click on the name of the patient. 3 The result details will be displayed. 4 Use the scroll bar on the right side of the page to scroll through the entire contents of the report. 5 Use the Print Result button to view and print the report in PDF format. 6 Use the Save PDF button to save the file. 7 To exit the result details page and return to the main list of results, click the Back to Results Inbox link on the left upper area. Result Details Page Header Use the screen shots and descriptions below to better understand the result details page. This is an example of a CareDx patient header. Name *OFFICE ID MRN PHONE D.O.B AGE SEX REQUISITION # *BILLING ACCOUNT# COLLECTED DATE RECEIVED DATE RESULTED DATE ORDERING MD *Note: Field is not used Patient s last name, first name. Physician office ID. Patient s laboratory Medical Record Number. Patient s phone number. Patient s date of birth. Patient s age in years. May also be listed in days or months. Patient s gender. This may also be called an order number or accession number. This field is populated with the physician office s medical record or chart number. Date the sample was collected. Date the sample was received at the lab. Date the result was released from CareDx LIMS. Displays the ordering provider s name. Order/Result List Demographics New Order Email Cumulative Reporting Notes Mark As Reviewed Displays the patient s chart. Displays the patient s demographics page. Displays the new order page. Displays email option. Displays the cumulative report page in the patient s chart. Opens another window where provider notes may be entered, and once saved, will appear on the last page of the printed report. Mark the result as reviewed; Y flag appears on the Inbox. Results Body Test Description Results *Abnormal Reference Range *Units Lab Note The first column lists the description of the tests with results and related information. Results are displayed for each test. Not applicable. Indicates the laboratory normal range of values for the test (0-40). Not applicable. Identifies the sending lab, if the data were sent in the interface message. If a note exists for the result, a blue C link will be displayed. View Printable PDF Report There are two options to view a printable PDF report from the Results Inbox. Follow the steps below to generate and view a printable PDF version of the results report. Viewing Batch Reports The Batch Reports link will display a list of reports generated from the Print Reports and Global Results Search links. Batch reports are: Identified by a batch ID number Sorted by descending date Default date range is 45 days Ready to be viewed or printed at any time May be filtered by using these dropdowns: Batch Reports Details Use the screen shot and descriptions below to better understand the Batch Reports page. Batch ID Type This is a unique ID given to each batch. They are organized in descending order. Description By Spooled Finished Print Icon This column details some of the characteristics of the report. For some reports, dates are shown, and if specific accounts are included in the report, this column will display which accounts. Defines the username of the person who generated the report, including the AutoPrint user. This is the date and time on which the report was generated. This date and time define when the generation of the report was completed. The Print icon displays when the report is complete. It will not appear if there is no data to print, or if the report is pending. Click the Refresh link until the printer button appears. CareDx Customer Web Portal User Guide Version 3.6.3 50

Print Reports To create a results batch report, click the Print Reports link from the Results tab. The batch reports screen will appear, as seen here. Follow the steps below to generate a report. Step Action 1 Select the type of report to generate: Unprinted Reports: report includes only result reports which have not yet been printed (meaning, previously displayed in PDF format). 2 All Reports will be selected by default. If your lab also generates reports, the option to print those reports will also appear. 3 Type a date range, or use the calendar icon to select the date. The date range will default to 60 days, but can be changed (contact CareDx). 4 Select the sort order, either by patient or physician. 5 Finally, in the Accounts section of the form, place checkmarks next to the locations to include in the results report. 6 A new window will appear with the batch report ID. Look for this number in the Batch Reports area. 7 On the Batch Reports page, click the Refresh button to refresh the report generation. 8 After opening the report from the batch reports link, it will be in printable PDF format. Global Results Search On this screen, you may search for specifically defined results from your entire patient population. Search criteria include: location, date range, patient age, patient gender, patient ZIP code, analytes (up to 10 may be defined) and their specific values (Any of the conditions or All of the conditions met), keyword search (for text-based results). Select the output for the report: Browser, spreadsheet, or PDF. Analytes in the browser or a spreadsheet may be displayed horizontally or vertically. Enter the date range for the search. Note: Only analytes that were resulted during that date range will appear in the analyte search. You may also generate the report to contain no patient names or IDs by adding a check mark to the Hide Patient Identifiers option. Anonymous will appear in the place of the patient name. Save the search for future use by clicking the Save Search button and naming the search. To clear all fields, click the Reset button. To save the details of the search, click the Save Search button. Generate a Global Results Search Follow the steps below to create a Global Results Search. Step Action 1 Click the Global Results Search link under the Results header. 2 Select the Lab (if only one lab is available, there will be no Lab dropdown), Location, and output type. 3 Select either Collected Date or Resulted Date. A default of the past one month will appear in the Date Range field. You may change the dates by typing new ones or by using the calendar button to select new ones. 4 Adjust the Age details: 5 Select patient gender (Sex). 6 Narrow the search further by typing patient ZIP codes in the designated field. 7 To hide patient names and list them as Anonymous, check mark the Hide Patient Identifiers box (see Global Results Search figure above). 8 If the report output is the browser or a spreadsheet, then vertical or horizontal orientation may be selected. 9 Select ANY or ALL for the conditions to be met. 10 Designate up to 24 analytes for the search. After the analyte has been selected, choose greater than, less than, equal to, not equal to, or ALL, and then enter a value. Note: 101 is the Analyte code for AlloMap HTx Results. By default, only 2 analyte areas will appear. To add another line, use the Tab key after entering the numeric value for the previous analyte. Up to 24 analytes may be added. 11 For a text-based result search, select the radio button and type the text in the Keyword field. One year is the maximum date range for a text result search. 12 Click the Reset button to clear all fields and begin again. 13 To save the details of the search for future use, click the Save Search button. 14 Click Search to generate the report. Duplicate Patients for Customer Care Use Follow these steps to check a practice for duplicate patients. If duplicate patients are identified, please contact CareDx Customer Care (customercare@caredx.com) to merge patients. Step Action 1 Select the Duplicate Patients link on the left side of the screen. 2 Drop down the arrow to select the criteria by which you want to check for duplicates. The choices are: 3 Once the criteria is selected, you will either receive this message, or a chart (example seen below) will appear with potential duplicates. Customs Overview Customs is an area of administration in which you may set up certain preferences for each practice. a. Alerts: set up customized ranges for alerts to appear at the top of the Inbox and on the Alert Report. b. Graphs: set up a list of analytes, which when resulted, are displayed in graph format on the PDF result report. c. Users Overview The Users area of Practice Administration allows you to add, modify and delete users (non-providers) from a practice. Note: Only a Lab Administrator may add or modify user accounts for a provider. Patient ID Only: User may place orders only and may only search for the patient using a patient ID. Username: 6 or more characters with the following requirements: Acceptable: letters, numbers, period, at sign, underscore, dash (hyphen) Unacceptable: special characters such as < > & % \ ` $ :! ^ [ ], spaces; only numbers. Usernames are not case-sensitive. Password Hint: this will appear after an unsuccessful login using the incorrect password; it cannot equal the password. E-mail Address: enter the email address of the user E-mail Notification: if a valid e-mail address is entered, and this box is checked, the user will receive email notifications whenever a new result is delivered to the Web Portal. Patient Search: identifies which type of patient search is available on the main page of the application. Results History Threshold: the number of day s worth of results visible in the Results Inbox. Results Page Size: the number of results to appear on one page on the Results Inbox. Ordering Location: select one of three ordering choices. Use Default Location: order will be placed in logged-in location. Allow Location Selection: in the Order Information section of the Demographics tab of the order, user may select a location from a dropdown of all locations in the practice to place the order in. The dropdown will default to the logged-in location. Force Location Selection: in the Order Information section of the Demographics tab of the order, user is forced to select the location from a dropdown of all locations in the dropdown. No location appears in the dropdown by default. QuickPrint: check this box to have all requisitions automatically sent to the default printer when the order completed and the Complete/Print button is clicked.
