Renewing Junior Member Online Registration Instructions Parent/Guardian must renew their Junior s Membership, NOT CLUB DIRECTORS or COACHES Step #1 Go to www.chrva.org, and click on Register for CHRVA from the menu of items on the left side of the screen. It will bring up the screen to the right. If your child was registered with a club team in CHRVA last season, go to the paragraph, labeled THIRD, click on the Renewing Registrations link. This will take you directly to the USAV Webpoint web site login page pictured in Step #2.

Step #2 If your child already has a login, proceed to Step #4. For almost everyone this year, you do not have a login, so click Request a Login and continue to Step #3. Step #3 Enter your child s first & last names, email address, and birth date, and click Request Login (Parents: this should NOT be your email address. You will have an opportunity later to enter your email address). Your child s User Name & Password will be emailed to them.

Step #4 Type in your child s User Name & Password, and click Log In. Step #5 Click the Renew Online button on the left side of your screen, or click the HERE area in the blue header area. This will bring up the next screen with your child s information already in the fields.

Step #6 Verify your child s information is still correct. Make sure their email address is still current. All correspondence will be sent to that email address along with confirmation information. Update their Current Grade. Click Continue. Step #7 Check your child s information for accuracy. Click the edit button if you need to correct your child s membership information. At this time, choose UNDECIDED in the Club drop down box. As long as you select UNDECIDED, you may login in again later and change to the club that your child will be affiliated with. (This only is available one time when UNDECIDED is selected). After that you will have to contact the region office to make changes. Fill out the Parent/Guardian information. Click Submit.

Step #8 Check this screen one last time for accuracy of information and make any changes to the Membership Information for the Junior Member by clicking edit. You may choose to make a donation to the National High Performance Program by checking the box and your total due will be adjusted accordingly. DO NOT check the box for Extended Official s Insurance unless your child is a certified Official (not just a Junior who passed basic class). It will delay membership if this box is checked and they are NOT a certified Official. Step #9 The next piece of the page allows you to select your form of payment. You may select to pay your fees directly by credit card which will allow you to complete your membership today. Or you may select to submit your payment with your application and mail a check to the address given at the end of this document, which will delay your membership until the payment is received.

Step #10 The last part of the page is a very important part of the registration process. A parent or guardian will be providing information, as well as, accepting the terms of membership on behalf of their child or legal ward. Check the box under USAV Waiver & Release of Liability to acknowledge agreement to the waiver. Click on the blue USAV Waiver & Release of Liability. A box will open so that it can be read. Checking the box is accepted as your electronic signature for the document. Check the box under USAV Participant Code of Conduct to acknowledge agreement to the Code of Conduct. Click on the blue USAV Participant Code of Conduct. A box will open so that it can be read. Checking the box is accepted as your electronic signature for the document.

Check the box in front of the red text which is your agreement to the terms of membership and that you are the legally authorized person to sign this electronic document. The last part is to fill in your Driver s License number of the Parent or Guardian for verification of identity (only use last 4 digits if your social security number is used for your driver s license number). Click Confirm at the bottom of the page. Click OK when this box comes up if you are ready to submit your application. Another message will come up informing you that your application has been submitted. Click OK.

Step #11 There will be a long confirmation page next with all your information included. It gives you the option of printing (look at either the top right or bottom right corner of the page) where it says Click Here for a printer friendly version. Step #12 You will receive a confirmation email from the USAV Webpoint system which will contain all information pertaining to your membership, including monies due, login user name, password, and options you have chose. Step #13 Your membership is not complete until payment is received by the region. If you chose to pay by credit card, your membership is complete, and you should be receiving a confirmation email. Or Send a check with Junior member name printed legibly in the memo field, payable to CHRVA to CHRVA Registrar 6738 Ford Road Frederick, MD 21702 You will receive a confirmation email when your membership is paid.
