3rd EUROPEAN MEETING OF YOUNG OPHTHALMOLOGISTS I General provisions These Terms and Conditions shall apply to the Participants and Virtual Participants of the 3 rd European Meeting of Young Ophthalmologists (hereinafter: EMYO 2018 ), which is organised by InspireCongress sp. z o.o. with its registered office in Wrocław, ul. Nowowiejska 38, entered in the Register of Entrepreneurs kept by the District Court for Wrocław-Fabryczna in Wrocław, 6 th Commercial Division of the National Court Register, under No. KRS 0000290025, REGON 020613939, NIP: 898-21-22-587, acting on behalf of Thea Polska Sp. z o.o. with its registered office in Warsaw, ul. Cicha 7, 00-353 Warsaw. 1. The terms used herein shall have the following meaning: Conference: 3 rd European Meeting of Young Ophthalmologists held in Cracow, Poland, on July 13-14, 2018, in Auditorium Maximum of the Jagiellonian University Organiser: InspireCongress Sp. z o.o., ul. Nowowiejska 38, 50-315 Wrocław, REGON: 020613939, NIP: 898-21-22-587, KRS 0000290025 Personal Data Controller: Thea Polska Sp. z o.o. with its registered office in Warsaw, ul. Cicha 7, 00-353 Warsaw Participant: any ophthalmologist in training or up to 5 years after completing specialisation in ophthalmology who is registered to take part in the Conference via an on-line registration form available on www.emyo2018.com Virtual Participant: any person with interest in ophthalmology who is registered to gain access to live streaming of the Conference via an on-line registration form available on www.emyo2018.com Confirmation of Registration: an email sent to a Participant/Virtual Participant by the Conference Organiser after the successful completion of the on-line registration 2. The official website of EMYO 2018 is www.emyo2018.com 3. The official language of the Conference is English and no translation will be provided. 4. The Conference Programme shall be available on the official website. The Organiser reserves the right to make changes to the Conference Programme and shall be obliged to publish current information on the website. The changes shall not alter the basic principles of the Conference. 5. The Terms and Conditions constitute an integral part of the registration process to EMYO 2018. By registering to the Conference each Participant/Virtual Participant confirms that he/she has read the Terms and Conditions and accepts them. 6. The Terms and Conditions are available at the Organizer s office and on the official website. 7. The Organizer can be contacted by email at: ms@inspirecongress.pl, or by phone on: +48 699 960 557. II Registration 1. Participants and Virtual Participants shall register for the Conference via an on-line registration form available on www.emyo2018.com by creating an account and providing full personal information. 2. By filling out the registration form a Participant/Virtual Participant enters into a binding agreement with the Organiser under these Terms and Conditions.

3. Participants and Virtual Participants shall be obliged to fill out the registration form correctly and to ensure that the information given is consistent with the actual status. 4. The Organiser disclaims liability for losses arising from entering incorrect or incomplete data in the registration form. 5. After the successful completion of the on-line registration a Confirmation of Registration will be sent to the email address provided in the registration form. 6. The registration ends on July 10, 2018, or earlier if the limit of participants is reached. 7. In justified random cases a replacement Participant can be nominated and such a name change is free of charge. The replacement will only be accepted after a written notification sent to ms@inspirecongress.pl, and only until 30 days before the Conference, that is not later than June 13, 2018. Information submitted after this date might not be acknowledged by the Organiser. The notification must clearly indicate the old and the new name and full personal information of the replacement Participant must be provided. 8. Upon arrival at the Conference venue each registered Participant shall receive conference materials and an ID badge. 9. Virtual Participants shall get access to live streaming of the first day of the Conference upon receipt of a personalized access code. 10. Virtual participation is free of charge. 11. Detailed information concerning virtual participation shall be available on the official website at a later date. Payment 1. By accepting these Terms and Conditions a Participant agrees to pay a registration fee of 220 PLN (~ 50 EUR). 2. Title of transfer: Registration Fee EMYO 2018 NAME and SURNAME of the Participant III 3. The payment must be done within 14 days after registration. 4. If the payment is not received within the terms specified herein the registration will be automatically cancelled. 5. The registration fee includes: accommodation in double rooms (two nights - 12 th and 13 th July 2018), admission to all scientific sessions, conference materials, lunch and coffee break refreshments, dinner and a social event on July 13,, 2018. 6. All Participants shall arrange and cover the cost of their travel to attend the Conference. 7. All participants should make their own arrangements with regard to personal insurance. IV Cancellation and reimbursement 1. Participants shall have the right to withdraw from participation in the Conference and are entitled to full reimbursement of the registration fee provided that a written notification of the withdrawal from participation in the Conference is submitted to the email address: ms@inspirecongress.pl not later than 30 days before the Conference, that is not later than June 13, 2018. 2. Cancellations made after this date or no-show at the Conference will not be reimbursed.

V Rights and Responsibilities of Conference Participants/Virtual Participants 1. All Participants/Virtual Participants shall be obliged to read and understand these Terms and Conditions. 2. All Participants/Virtual Participants using the Conference website shall be responsible for the security of their user account and are obliged to protect their account password and not disclose it to third parties. If public computers, such as computers in internet cafes, universities, offices or libraries are used, users should log out after every session in order to prevent third parties from accessing their private information and personal data. 3. Photographing or filming during Conference events is allowed only by means of personal-use equipment and only for personal use. 4. All Participants/Virtual Participants are guaranteed the opportunity to express their opinion about the Conference and leave a written feedback. VI Protection of personal data and information clause 1. Thea Polska Sp. z o.o. with its registered office in Warsaw, ul. Cicha 7, 00-353 Warsaw, shall be the Personal Data Controller. The Controller can be contacted by email at: info.poland@theapharma.com, or by phone on: +48 22 642 87 77. 2. In cases related to personal data the Data Protection Officer shall be contacted. The Data Protection Officer can be contacted by email at: country.privacy@theapharma.com. 3. Personal data are processed in order to conclude and perform the Agreement binding under these Terms and Conditions, including the organisation of the Conference and access to the users account on the Conference website, and for the communication between the Participant/Virtual Participant and the Conference Organisers, pursuant to Art. 6 item 1 letter b) GDPR. Personal data are processed also in order to comply with the legal provisions regarding tax settlements pursuant to Art. 6 item 1 letter c) GDPR. The necessity of both data processing in order to conclude and perform the Agreement and to comply with the legal provisions are the legal basis for data processing. 4. Providing personal data is voluntary yet necessary for registration and organization purposes. 5. Processing of personal data has been entrusted to the Organiser based on the data processing outsourcing agreement made in writing between Thea Polska Sp. z o. o. (Personal Data Controller) and InspireCongress Sp. z o. o (Organizer). 6. Independent from creating an account to register for the Conference, each Participant/Virtual Participant may receive commercial information from Thea Polska Sp. z o. o. by electronic communication and/or by automated calling systems or telecommunications terminal equipment. To activate this service a voluntarily consent must be given in the registration form to receiving commercial information by electronic means within the meaning of the Act of 18 July 2002 on the provision of services by electronic means (Polish Journal of Laws 2017. 1219, as amended) and to the use of telecommu nications terminal equipment and automatic calling systems for direct marketing purposes within the meaning of Art. 172 of the Act of 16 July 2004 on telecommunication law (Polish Journal of Laws 2017.1907, as amended). 7. If consent is given to receiving commercial information by electronic communication and/or by automated calling systems or telecommunications terminal equipment for the purpose of direct marketing, the personal data shall be also processed by Thea Polska Sp. z o. o. for the legally justified purpose in the form of direct marketing of their own products and services, pursuant to Art. 6 item 1 letter f) of the GDPR. The personal data shall be kept until these consents are withdrawn, in the scope required for sending marketing information. 8. Personal data shall not be kept for the period longer than necessary for the purpose for which it was collected, including the organisation of the Conference and access to the users account on the Conference website nevertheless not longer

than for periods compliant with the provisions of law. After this period personal data shall be kept solely in the scope necessary for settlement of any services rendered and fulfilment of any other obligations under commonly applicable laws (specifically tax regulations) and asserting claims with respect to the concluded agreement. 9. If consent is given for the transfer of personal data to Laboratoires THEA S.A.S and Thea Subsidiaries (Thea Group) the personal data shall be kept until the consent is withdrawn, in the scope required for sending information on professional events. 10. The Participant/Virtual Participants has the right to: request access to and rectification or erasure of your personal data or restriction of processing and also the right to data portability, to object at any time to processing of personal data concerning Participant/Virtual Participant for direct marketing of their own products and services purposes, to withdraw consent referred to in point 6 and 9 above at any time, which does not affect the lawfulness of processing based on consent before its withdrawal. The consent may be withdrawn by sending an email to ms@inspirecongress.pl. lodge a complaint with the supervisory authority (Prezes Urzędu Ochrony Danych Osobowych). 11. During the Conference the Organiser may take photos and film the sessions (live streaming) after obtaining appropriate consents from Participants. 12. Personal data are not used for automated decision-making, including profiling. VII Amendment, modification and deletion of personal data 1. Each registered Participant/Virtual Participant shall have the right to access, modify, correct and delete his/her personal data by logging into their account. 2. Deleting a user account from the website requires contacting the Organiser directly and sending an appropriate request to the email address: ms@inspirecongress.pl VIII Disclaimer of liability 1. The Organiser reserves the right to cancel or postpone the Conference in the event of circumstances beyond their control. 2. Should the conference be disrupted or prevented from going ahead as a result of unforeseen political events, industrial action or force majeure, the Organizer shall not be liable to pay any compensation. 3. The Organizer excludes liability for any personal loss by or injury to Participants during the event. Participants are recommended to take out their own personal insurance.

IX Final provisions 1. All Participants/Virtual Participants shall be notified of any changes to the Terms and Conditions by email sent to the address provided during the registration. If a Participant/Virtual Participant does not accept the changes to the Terms and Conditions, he/she should inform the Organiser of this decision in writing within 7 days from the date the altered Terms and Conditions enter force, by sending an email to ms@inspirecongress.pl. If the Organiser does not receive a written declaration of not accepting the changes to the Terms and Conditions within the aforementioned time, it shall be understood that the Participant/Virtual Participant has accepted the altered Terms and Conditions. 2. In all matters not regulated herein the provisions of the Polish Civil Code shall apply. 3. These Terms and Conditions shall enter force on 15.09.2017
