Connections: Add Surrogate Connections: Add Surrogate The following walk-thru will explain the process of how an Individual Provider will initiate a Surrogate request to grant access to an organization to work on their behalf. Surrogacy allows anyone with a role for an organization to work on behalf of an Individual Provider or Healthcare Organization in NPPES, PECOS, and/or EHR Incentive Program. Once the surrogacy connection has been approved, anyone with a role in the organization will be able to work on behalf of the Individual Provider or Healthcare Organization EUS is only able to assist the account owner in registering for a role or in modifying the account. 1. The user will go to the Identity & Access Management System web site at https://nppes.cms.hhs.gov/iaweb and select "Accept."
2. The user will enter their User ID and Password and select "Sign In."
3. The user will select the "My Connections" tab and then the plus sign next to their name. If the provider has multiple roles to different organizations or individuals, they will need to make sure they select the plus sign surrogate request. next to the correct name for whom they want to send the
4. The user will select "Add Surrogate" under where it says "...has authorized the following surrogates."
5. The user will enter the Type 2 NPI of the Organization they want to be their surrogate and select "Search". If the organization the provider wants to be their surrogate does not have a Type 2 NPI, the user can search by the Organization Name, an Individual Providers First and Last Name, City, State, and/or Zip Code or can search by multiple fields to narrow the search.
6. The user will select the radio button next to the organization's name and then select the Business Functions: PECOS, EHR Incentive Program, and NPPES the surrogate should have access to and then select "Continue".
7. The user will review the business functions for which they are requesting access and then select "Submit" to complete the request.
The user does not need to enter an Alternate Email Address. The system will send the notification email to the email address on file in the providers account. However, the provider does not need the email to approve the request. If an alternate email address is entered, all notifications will be sent to that email for the request and the email address cannot be removed from the system.
8. The user will receive a message indicating that the surrogate request has been successfully submitted. This page will display the tracking numbers (which will begin with an S). Since each request is sent separately, the provider will receive a tracking number for each system. The user can select "Print this page" for their records and select "Done". The Optional Surrogacy Confirmation form will not be generated when an Individual Provider is initiating the surrogacy connection. The form does not generate because the Individual Provider is already giving their authorization for the organization to work on their behalf.
An approved Authorized or Delegated Official for the organization log will into I&A to approve the request. Refer to Surrogate Approval Process for the instructions the AO or DO will follow to approve the request.
If the Individual Provider is given the option to upload documents, the Agent should advise the Individual Provider not to upload documents because EUS cannot approve these requests. The request can only be approved by having the AO or DO for the organization log into I&A to approve the request. Article I005
