1 Purpse and Objective The purpse f this dcument is t prvide a descriptin f the functinal apprach t be used fr building Direct Line Wrker s. These reprts will be generated by the users f the rganizatins wh cnduct the OCAN with their individual Cnsumers. Mst f the reprts display infrmatin fr an individual Cnsumer that is assigned t the Staff instead f a grup f Cnsumers. Users f the system will be able t see all the infrmatin that they require depending n the parameters they chse fr a specific reprt. Mre infrmatin fr each reprt and its parameters are prvided in their respective sectins. Page 4 f 29

2 Assumptin The design and the intent f each reprt have gne thrugh a rigrus prcess and have been reviewed by the different rganizatins as well as the researchers wh develped the tlkit frm which OCAN was derived. Their feedback was essential in making sure the right infrmatin is being displayed in each reprt and the design f each reprt is user friendly. The final prduct delivered in the autmated slutin shuld resemble the lk and feel f the reprt in the mck ups. Page 5 f 29

3 Specificatin Overview This sectin defines the attribute categries f all the reprts. Once the applicatin has been further defined and detailed design has begun, the business may discver new reprting requirements. The fllwing attribute categries will be specified fr each : Name: A unique name fr each reprt as agreed by the business users. The reprt name will be displayed at the tp f every page f the reprt. Descriptin: A brief descriptin f the reprt including its purpse and use. The purpse f the reprt as defined in the mck ups shuld als be displayed n the first page f each reprt. Filter Parameters: A list f user-defined reprt parameters which includes the parameter names and its assciated values will be requested when a user selects a reprt t generate. Mre infrmatin abut the parameters is in Sectin 4 Parameters. Layut: This sectin prvides an verview f the reprt cntent and guidelines f what infrmatin is t be displayed within the reprt. Frmat: A list f recmmendatins t ptimize the lk and feel f the reprt Display Fields: A list f all the fields that will be displayed n the reprt. Page 6 f 29

4 Parameters The mck up design belw shws the ptins that the users shuld see when generating reprts fr their Cnsumers. When the users are ready t generate the reprt, the fllwing parameters shuld be available t them t chse frm. Details f each reprt parameter are defined in the reprt specificatin sectin f this dcument. 5 Specificatins 5.1 Individual Need Rating Over Time Title Mck Up Descriptin Individual Need Rating Over Time Please refer t Sectin 6.1 Appendix A I. The purpse f this reprt is t shw change ver time in a specific Need Rating fr an individual Cnsumer. It adds up the number f needs acrss all Dmains by Need Rating (e.g. Unmet Needs, Met Needs, N Needs, and Unknwn) fr all r selected OCANs that were cnducted with the Cnsumer and displays the results in an individual need rating line graph. Each line graph that is displayed cmpares the Cnsumer and the Staff's perspective. The Staff may share this reprt with their Cnsumer as well. II. Since this reprt cmpares the need rating f a cnsumer ver a perid ff time frm the cnsumer and staff perspective, the assumptin is this reprt wuld be mre effective if the cnsumer had a Staff and crrespnding Cnsumer OCAN and in additin if there was at least tw r mre OCANs cnducted with the cnsumer ver a perid f time. Parameter I. The User selects t generate the Individual Needs ver Time reprt. II. The User selects frm a list f OCANs that were cnducted with the Cnsumer. A. The first OCAN displayed shuld be the first OCAN that was cnducted with the Cnsumer at the Organizatin that is generating the reprt. This culd either be an Initial OCAN if the Cnsumer was new t the Mental Health System when he/she entered the Organizatin r it culd be Reassessment if the Cnsumer has been part f the Mental Health System prir t cming t the Organizatin. B. The latest OCAN shuld be listed as the Current OCAN. C. The last OCAN (OCAN cnducted prir t the mst current OCAN) shuld be listed as the Previus OCAN. D. All OCANs cnducted between the previus and first OCAN (if any) shuld just be named accrding t the OCAN that was cnducted with the Organizatin and be numbered chrnlgically. E. All OCANs listed in this parameter shuld have a date next t it. Page 8 f 29

If there is nly ne OCAN cmpleted t date fr a cnsumer, the reprt can still be generated albeit it wnt have much value t the end user since the graph wuld nly display 2 crdinates (ne fr Staff and ne fr Cnsumer). The reprt will display ne r up t 4 line graphs in the reprt depending n the hw many need ratings the user wants t see. C. Each need rating will be displayed in an individual line graph and will nt be cnslidated int ne line graph. D. Each line graph n the reprt will shw the number f need ratings fr each Assessment given by bth the Cnsumer and Staff. E. The OCANs will be listed n the x-axis and will list the OCAN that was cnducted first by the Organizatin ging frm left t the mst Current OCAN n the right. F. 5.2 Needs ver Time (Cnsumer and Staff) Title Mck Up Descriptin Needs ver Time (Cnsumer and Staff) Please refer t Sectin 6.2 Appendix B I. This reprt can be used by the Staff t shw the changes in Needs ver Time frm the Cnsumer's and Staff's perspective by cmparing Need Ratings frm previus OCAN Assessments that have been selected. This reprt will shw the prgress that has been made ver time and identify the strengths as well as areas that need further fcus. Any changes that have been made in the Need Rating fr each Dmain frm the previus OCANs will be marked by a checkbx. The Staff may share this reprt with their Cnsumer as well. II. Since this reprt cmpares the need rating f a cnsumer ver a perid f time frm the cnsumer and staff perspective, the assumptin is that this reprt wuld be mre effective if the cnsumer had a Staff and crrespnding Cnsumer OCAN and als if there was at least tw r mre OCANs cnducted with the cnsumer ver a perid f time. Summary f Needs : The reprt will display a summary f the ttal number f the selected Dmains in each Need Rating categry fr each OCAN included in the reprt fr bth the Staff and Cnsumer, descending in the fllwing rder: Unmet Needs, then Met Needs, then N Needs and finally Unknwn Fr each OCAN selected, prvide the apprpriate OCAN title (fr e.g. Current OCAN, Previus OCAN) A date will be displayed under each OCAN t shw when it was cnducted. D. Breakdwn f Summary f Needs: This sectin f the reprt will display all the need ratings fr the dmains fr the selected OCANs. The dmains listed in the reprt will be based n the dmains selected in the reprt parameter step by Page 12 f 29

the user. Fr each OCAN selected, the relevant OCAN title (fr e.g. Current OCAN, Previus OCAN) shall be displayed. A date will be displayed under each OCAN t shw when it was cnducted. The rder f the dmains will be based n the gruping fr the need rating f the mst Current OCAN in the fllwing descending rder: Unmet Need, then Met Need, then N Need and finally Unknwn. If the need rating f a particular dmain fr Cnsumer and Staff are the same in the Current OCAN, that is cnsidered as an agreement. Agreement in Dmains shuld be listed abve ther dmains within the particular need rating grup. Fr e.g. if the Cnsumer and Staff agree that Physical Health is an Unmet Need, then it will be displayed first since there is agreement between the Cnsumer and Staff. All the ther agreed unmet need dmains shall be listed fllwed by met need agreed dmains and then met needs disagreement s n and s frth. If there is disagreement -- fr e.g. the Cnsumer identifies that the Self Care Dmain is an Unmet Need and the Staff identifies Self Care as a Met Need - -- then the higher rating will be taken int accunt, which is Unmet Need, and will be displayed after all dmains where there was an agreement fr Unmet need. The gruping will then cntinue listing agreements between Cnsumer and Staff in Met Needs and s n until all Dmains and their Need Ratings are listed fr the Current OCAN. Therefre, as mentined abve, the gruping will descend in the fllwing rder by agreement between the Cnsumer and Staff nly fr the mst current OCAN included in the reprt: Unmet Need, then Met Need, then N Need and finally Unknwn. The ther selected OCANs need values f the dmains shuld fllw the same rder as the mst Current OCAN. Every time Unmet Need is listed fr a particular Dmain, it shuld be displayed in red and bld text. If there is a change in need rating frm the previus OCAN when displaying the dmain details table, a check mark will be displayed t dente the change in the Change clumn. Page 13 f 29

E. Stacked bar graph fr Needs ver time: The reprt will display a stacked up bar graph fr bth the Cnsumer and Staff fr each OCAN included in the reprt. III. Fter: <Blank> The Unmet Needs will be listed at the tp f the bar fllwed by Met Needs, N Needs and Unknwn. The Unmet Needs will be listed at the tp f the bar fllwed by Met Needs, N Needs and Unknwn. Each bar graph will be divided by the different need ratings fr their specific OCAN and the value f each bar graph shuld always equal 24 which represent the number f dmains. The value f each need rating is t be displayed within their specific bar graph fr each OCAN. The graph will separate the OCANs by thick black lines. The legend f the bar graph shuld be listed at the bttm f the graph. The y-axis n the graph shuld display a maximum value f 24 and shuld be labelled # f Dmains. The x-axis n the graph will list each OCAN included in the reprt and will be displayed under the Cnsumer and Staff clumns fr each OCAN. The x-axis will be labelled Assessments. The name f the reprt shuld be displayed at the tp f the bar graph. Frmat I. It is recmmended that the reprt layut is t be displayed in landscape rientatin II. The Cnsumer and Staff clumns where the need ratings are displayed fr each dmain shuld be different clrs and cnsistent fr the Cnsumer and Staff. It will shw which dmains are Unmet Need, Met Need, N Need and Unknwn by bth the Cnsumer and Staff III. We recmmend yu select clurs which can easily be distinguished if the reprt is printed black and white. IV. The OCANs are t be separated frm each ther by thick black lines. 5.3 Summary f Actins and Cmments Title Mck Up Descriptin Summary f Actins and Cmments Please refer t Sectin 6.3 Appendix C I. This reprt displays a summary f Actins and Cmments (bth fr the Cnsumer and Staff) that were recrded in the selected OCANS fr an individual Cnsumer. The reprt lists all the Actins and Cmments assciated t OCANs and Dmains that were chsen by the Staff t be displayed. The Actins als list wh is respnsible fr the Actin and the Review Date fr the Actin. The Dmains are categrized by need rating within the Current OCAN as well as displaying the need ratings frm previus OCANs. In the case f different need ratings by the Cnsumer and Mental Health Wrker, the higher need rating determines the categry. The need ratings frm highest t lwest are Unmet Needs, Met Needs, N Needs and Unknwn. The need rating given by the Cnsumer and the Staff are als displayed next t the Cmments fr each OCAN. This infrmatin can be passed alng t ther rganizatins if requested. II. Since this reprt encmpasses Actins and Cmments frm the Full OCAN r Cre+ Self OCAN type ver a perid ff time frm the cnsumer and staff perspective, the assumptin is this reprt wuld be mre effective if the cnsumer had a Staff and crrespnding Cnsumer OCAN r at least the Cnsumer OCAN and in additin if there was at least tw r mre OCANs cnducted with the cnsumer ver a perid f time. When gruping current OCAN by Need Rating, the higher need rating given by either the Staff r the Cnsumer will be taken int Accunt. Fr e.g., fr the Page 16 f 29

Accmmdatins the Staff rates the Dmain as an Unmet Need and the Cnsumer rates it as a Met Need, then the Accmmdatin Dmain will fall under the Unmet Need gruping since Unmet Need is a higher rating. F. In the case f different need ratings by the Cnsumer and Mental Health Wrker, the higher need rating determines the categry G. If the Cnsumer has been discharged frm the OCAN fr mre than 3 calendar mnths, the Cnsumer and their crrespnding OCANs will n lnger be part f the Staff s Wrklad. The infrmatin wuld prvide mre insight int the magnitude f the wrklad fr the staff since Re-key r Review may be less f a wrklad than Initial OCAN. Belw are the pssible values fr this field Initial OCAN,Reassessment,(Prir t) Discharge, Significant Change, Review, Re-key, Other (cnsumer request) This field reveals if the Staff member is the lead fr the assessment OCAN LEAD = Yes: If the Staff member is an OCAN lead then the Staff will be respnsible fr the Reassessment, therefre the Reassessment time frame clumn fr that rw wuld have the expected date fr the reassessment. OCAN LEAD = N: If the Staff member was nt an OCAN lead, then the Staff member is nt respnsible fr cmpleting the Reassessment fr the client; therefre, the OCAN at the 6 mnths time frame clumn fr that rw will be blank fr the reassessment. This is the date range fr when the Reassessment at 6 Mnths shuld be cmpleted fr the OCANs that are currently in Prgress. Like all OCANs, the Reassessment als has a 30 day cmpletin windw. Reassessment time frames will nly be displayed fr OCANs where the Reasn fr OCAN is either Initial OCAN r Reassessment. All ther OCANs will nt be shwn. The start date f the Reassessment will be 6 calendar mnths frm the start date f the Initial OCAN that is In Prgress and end date will be 30 calendar days frm the start date f the Reassessment Time Frame. Once the first Reassessments started by the user, this new Reassessment will becme part f the Staff Wrklad and a new Reassessment time frame will be created fr the recently In Prgress Reassessment OCAN. The timeframe will always fllw a 6 mnth heartbeat frm the start date f the Initial OCAN until discharge. Fr e.g., if the Initial OCAN that is part f the Staff wrk lad has a start date f 1/24/2009, the Reassessment Time Frame will be 07/24/2009 08/22/2009 fr the first Reassessment. Fr the 2 nd Reassessment nce the first Reassessment has been started and becmes part f the Staff Wrklad, the secnd Reassessment Time Frame will be 6 mnths ut frm the first Reassessment Time Frame start date and will be 01/24/2010 02/22/2010.
