INSTRUCTION MANUAL FOR ONLINE APPLICATION FOR PG-DENTAL FOR ODISHA STATE QUOTA SEATS Different Steps for Online Application for Admission in to PG-Dental Courses :- (A) Payment of Application Fee Through Bank Draft (B) Preliminary Registration (C) Online Application (D) Take print out of application (2 Copies) (E) Verify documents as per the schedule (F) Choice Filing & Locking (G) Download Allotment and Joining Documents Required before applying Online :- 1) Scan copy of Pass Port Size Photograph within 100 KB and in.jpg /.jpeg formats 2) Scan Copy of Full Signature within 20 KB and in.jpg /.jpeg formats 3) NEET Score Card 4) Service Certificate (In case Employed under State Govt.) 5) Sponsorship Certificate (In case getting sponsorship) Eligible Candidates fulfilling minimum eligibility criteria as per NEET eligibility (NEET Score for General Candidates- 340, SC/ST/OBC Candidates- 295 and PH Candidates-317) are eligible to apply. Besides the above, if any candidates securing less scores, applies, his / her application will get rejected until and unless. NEET revises the minimum eligible criteria to further lower percentile. If any non eligible candidate is applying, then he is applying at his / her own risks. Preliminary Registration :- 1) First Enter your NEET Roll No Number 2) Enter your Date of Birth (Select from calendar) and Pls wait a while 3) Your NEET Score and Name as per NEET data will be shown below (If not showing, pls chk your browser Java script should be enabled, else change the browser to Chrome latest version) 4) Then enter your email ID which will be your Log In ID in future. (Pls don t give wrong email ID) *Only one e-mail ID can be registered and a candidate can apply only once 5) Enter your Password (Password should be minimum 6 characters long having one upper case, one lower case, one special character and one integer like Purna@12) *While entering the password, please ensure that the key boards are working perfectly, as in some cases, if key bord will not function perfectly, the password may be wrongly assumed and may create problem while login) 6) Re-type your password again 7) Select a secret question and enter the answer to your question, so that it will be helpful in case you lost your password 8) Then click here for Online Application
Online Application :- (A) Personal Information Section :- 1) Please fill up your personal information (Your Name as per NEET record, and registered e-mail ID will not be entered here, as it will be shown automatically which will not be changed and should not be entered again) 2) Enter your father s Name and Name of Husband (name of husband for Female Married applicants, they desire to fill up, as it is not mandatory) 3) Give your Mobile Number and Alternate Mobile Number 4) Date of Birth should not be entered, as it will be shown from NEET data (as given during NEET exam) 5) Select your gender 6) Select your category from drop down. If you belong to more than one category like SC with GCH with PH and Ex-Serviceman, then Please Press Ctrl Key from the Key Board and Select the Categories one by One (will be highlighted in blue like colour)and by double clicking it will be deselected 7) Please select your resident state 8) Select the BDS study State from the drop down menu
(B) NEET Particulars Section :- 1) In this section, NEET data like NEET Roll No and NEET Score, Name and DOB as per NEET data will be shown to you for confirmation with your NEET Score Card. If found discrepancies, please bring this to the kind notice of the verification team during document verification, otherwise, you can mail to the technical help line indicating the problem. (C) BDS Particulars Section :- 1) Fill up your BDS particulars like your Dental Registration No, name of the Dental Council and the validity of the Registration 2) If you will get selected and will join in odisha under State Quota seats, your BDS registration should be updated and if your registration is outside of odisha, then you have to transfer it to odisha State (D) Service Location Particulars Section :- 1) If you are employed in Govt. of Odisha (OPSC cleared or Regular) / or any other PSU present in Odisha or ESI Hospitals, then please mention your Service Location details like Present Place of Posting, District in which you are posted in respective sections. 2) You have to fill Service Location details, irrespective of your category like Direct or In-service. As this information will be required to make you relived to join in PG courses. If this section is not applicable to you, then leave the fields blank / unfilled. (E) Counseling Fee Particulars Section :- Please enter your bank Draft Number (made for application fees) and details of Bank NAME AND BANK Draft date in respective places. Details of Bank Drafts can be found out in the Prospectus. (F) Address for Correspondence :- Please fill up your Permanent Address and if your Present Address is same with your Permanent address, then Please Tick (select) the Chk Box so that, the details filled up under Permanent address will be copies to the Present Address section, else, fill both the address separately.
(G) Photo and Signature Upload :- 1) Please upload your scanned Passport size photograph and signature in respective fields and then click Save and Continue button. (H) Service Particulars Section :- 1) In this step, please select, whether you are a Fresher / direct Candidate or you are an In-service Candidate. 2) In-service Candidates are those, who have completed minimum 3 years of continuous service as 31/03/2019 in Odisha (either as contractual / deployment/ temporary/ adhock / regular) and he / she should have presently continuing in his Job as on 31/03/2019. 3) Then click Save and Continue and you will be directed to the Print application Page
4) If you are an In-service Candidate, please select I am an In-Service Candidate and the following sections to fill your Service details will be shown. 5) Here, as per the order of the Hon ble Supreme Court, the Service details are divided I to 2 parts 6) Candidates those whose initial posting was before 21/03/2016, they will be eligible to get additional weight age (10-20-30) as per the places under Appendix-1, and the candidates whose posting was after 21/03/2016, will be eligible to get weightage as per the V1-V2 under Annexure-A 7) Please select whether your first initial posting is before 21/03/2016 or after 21/03/2016 8) Then select your Type of Service from the drop down list 9) Then fill your service particulars as mentioned in your service certificate 10) Fill your Name of Dental Institution like PHC, Begunia/ CHC Pathara / SDH Balangir / DHH Koraput like 11) Select from the drop down list under which your institution is coming. If the name is not under the list provided, then please select Others. 12) Then enter / select from calendar the date of Joining 13) Then select the status of your Job means whether you are still continuing in the same head quarter or already relieved to a new head quarter 14) If you have already relieved, then enter your date of relieve
15) After entering date of joining and status with date of relieve, please wait a while, the system will automatically calculate the duration / number of days you were present in the same head quarter. 16) If you are continuing, automatically the date of relieve will be calculated till 31/03/2019 17) If you were posted in more than 1 head quarter, you can go on adding one by one by clicking Add New button. Once the data are added, it will be reflected in just upper section of the row. Don t click Add New after the last data entered. Leave as it is. 18) Just like that, if you have joined after 21/03/2016, the same manner all data will be filled. 19) In similar manner, please enter all your information under Leave particulars. Leave details will be deducted from the total duration of the service while calculating the additional weight age as per the prospectus.
20) The list shown under the Block / NAC / Municipality are different to that of if selected prior to 21/03/2016 or after 21/03/2016 and so additional weight age will be calculated. 21) Manipulating in the selection of the Joining will be punishable offence and the application will get rejected. 22) So please enter the information as it is mentioned in the Service Certificate being issued by the CDMO concerned 23) After the Service particulars and leave particulars are entered successfully, automatically the total no of duration (days) that you have worked, will be calculated automatically and the additional weight age will be mentioned also. If any deviation found, pls bring this to the notice of the convener during document verification. 24) Since all service particulars are system calculated, please ensure the java script and browser are having compatibility to function smoothly, else please do changes in settings to activate java.
25) After all Service particular data are entered, please click save and Continue and you will be redirected to the Print page with generation of the Application Number. Please write down the Application Number along with your User ID / email Id and Password for future reference. (I) Edit Application (in case you need some data / information to be changed) :- 1) After taking the print out of the application (2 copies), please cross chk the information / data that you have entered. At any case, if you found any data wrongly entered, then you can log in to the system by using your email ID and Password and you can view the application print page. On clicking Home right to the option of Print application, the following page will be displayed. You can now click the Edit Option and your entered data / information will be shown again which you can edit now and again repeat the procedure as mentioned before.
