What is the? Launched in late December 2014, the new PEAKHealth mobile app gives Medicaid and Child Health Plan Plus (CHP+) members a simple way to keep their information up to date and access important health information right from their phone. The PEAKHealth app is for current Medicaid and CHP+ clients and users must have a PEAK account to log in. PEAKHealth is not for people who want to apply for benefits. pg. 1
Where is the located? Users can download the free PEAKHealth app from Google Play for Android devices or the App Store for IOS devices by searching for PEAKHealth. What can the do? provides a health tips section that provides users with useful information on fitness, happiness, health, nutrition and parenting. The goal is to encourage everyone in the Colorado Medicaid Community to pursue an active and healthy life, while staying on the path to better wellness. pg. 2
Login Process This mobile app uses the member s PEAK account user name and password. When the user logs into PEAKHealth an alert will display if the username and password entered is not recognized If the user has 3 failed login attempts, the user is advised that they have been locked out and that instructions for resetting their password have been sent via email. The email is sent automatically with the following language: Your account has been locked due to 3 failed login attempts. We have reset your password and sent you an email. Please check the email account associated with your username for instructions on resetting your password. Remember to look in your spam folder, where automated messages sometimes filter. If you still can't login, please dial 855-664-1169 for additional help. If the member does not have a PEAK account they can easily set up an account online at www.colorado.gov/peak pg. 3
Manage My Account When a client installs the they will have the ability to manage their Medical Assistance case within the app. They have the ability to: View Current Benefits Medical Assistance and Child Health Plan Plus(CHP+) programs only Upload paystub documents Submit payment for Medicaid and CHP+ enrollment costs View the Medical cards for the entire Household-It is a digital medical identification card that makes it easy for members to visit a health care provider. Medical Cards for the entire household can be accessed from the app. The app will display indicators below the Medical Card to indicate if there is more than one card for the user to view. The Benefits Information displays an explanation of benefits provided by the Colorado Medicaid program pg. 4
Account Information Once logged in the user has access to view their current Medical Assistance case information and a user can view and update their information if a change occurred such as: View and make payments View and update phone numbers View and update mailing address View and update job income View and remove members of the household pg. 5
Provider Location If a user needs assistance in locating a provider within a certain area they have the ability to search and view provider locations by type in map and list views. Medicaid health care provider information can be accessed by opening the Map icon and choosing the type of provider. Providers can be added to the user s Favorites list to quickly find the information later. How will this affect eligibility workers? Since users have the ability to use PEAKHealth to report changes to their case, eligibility workers will have to know what types of changes are reported and what will happen in CBMS once a change has been submitted. Clients can report changes like: Upload verification documentation (paystubs only) Change mailing address Update phone number pg. 6
Report job income changes End a job All of these changes will be routed to the PEAK Inbox in CBMS and listed as a Change. Users will be required to process and clear these from the PEAK Inbox using the same process as today. pg. 7
