I/we am/are aware that the CityFit Club is a self-service fitness club open to customers 24 hours a day, 7 days a week, and that exercises performed on its premises may be dangerous. I/we am/are responsible for any accidents and injuries resulting from my/our child s non-compliance with CityFit Club safety rules and policies. Being aware of the possibility of injury while exercising at the CityFit Club, I/we acknowledge that accident insurance should be purchased on my/our own terms. CLIENT REGISTRATION PROCESS 1. Download the legal guardian declaration form for giving consent to the use of CityFit Club services by a minor. The form is available at www.cityfit.pl or in the CityFit Club lobby. 2. Give the declaration form to your legal guardian for his/her signature. 3. Return the signed form with the personal information of the person that will be using the Club, which is required for creating a CityFit account. Please submit the form to customer service at the Club, or put it in the document box which can be found at the Club s entrance. 4. Wait for an e-mail with your password to the customer zone. 5. Sign in to your customer zone account to select and subscribe to the membership of your choice. 4/5
CLIENT REGISTRATION PROCESS 6. Pay for your membership using your chosen method of payment: card payment or direct debit. Remember, you can only place a direct debit payment on your own bank account. SMS 7. Wait for an e-mail and a text message with your PIN to register a fingerprint in the kiosk located in the Club s lobby. 8. Come for your first visit to the Club and scan your fingerprint. 9.
