How to Email the Proctoring Form to Your Local Testing Center Once you complete your Proctoring Form you need to take one final step and send it to your local testing center. If you are unsure how to complete your Proctoring Form, see the videos and guides that are on the Faculty Help section of the Click for Help tab. In the accompanying video, we walk through how to send a completed proctoring form to your local testing center. Before you send the document, you will need to: 1. Complete the form. 2. Find the email address of your regional testing center. The form is designed to be as simple as possible. If you have questions about completing the form, be sure to read the Instructions for Completing Proctoring Form link, which is also located on the Click for Help tab. The list of email addresses is also on the Click for Help tab in Blackboard. Once you a completed form and the correct email address, it s time to attach and send the form. We re going to walk through this process in the two different main email systems used at Ivy Tech the Outlook Web App which all faculty and staff have access to through Campus Connect as well as Microsoft Outlook which full time faculty and staff have access to on a college owned computer. Remember! The testing centers are ONLY PERMITTED to accept Proctoring Forms from you at your Ivy Tech email address. Emails received from non-ivy Tech addresses will not be processed by the Testing Center. They will ask you to resubmit the form if you send it from a non-ivy Tech address.
Sending the Completed Form with the Outlook Web App 1. Access the Outlook Web App by logging in to Campus Connect at http://cc.ivytech.edu. Click the Outlook button on the top right hand side of the screen. Outlook should open in the browser window. 2. At the top of the screen, click the + New button and select Email message from the drop-down. 3. Enter the email address of the testing center for the region for which you are teaching. The list of addresses for the testing centers is on the Click for Help tab in Blackboard. In this example, we re teaching a class offered by Terre Haute so we typed in the address proctorformwabashvalley@ivytech.edu. If you re not sure which campus address you should use check with your faculty supervisor, program chair, online technology coordinator, or testing center staff.
4. Enter an appropriate subject line. We suggest using the filename you used for the Proctoring Form in our example that is ACCT101-35G-SMITH-EXAM1 and a brief message if you wish. Remember information about how to proctor the exam should NOT be included in the body of the email all information must be included in the form. 5. Now you re ready to attach your form. Click the Attach button. Select Attachments from the pop up menu.
6. Navigate to where you saved the completed form on your computer. Attach the form to the email by clicking the name of it and clicking the Open button. Remember: Proctoring forms, once completed, contain confidential information including student accommodations for disabilities, exam passwords, and student ID numbers. NEVER save assessment proctoring forms on a public network drive. Also, double-check the email address BEFORE you send the form
make sure you only provide the form to someone who has an educational need to know this information about the students contained on the form. 7. If you need to add any additional file attachments be sure to include those as well. Once your email is complete, click the Send button. Congratulations! You ve Successfully Submitted Your Proctoring Form. Remember to tell students they can schedule their proctoring appointment now that you have submitted your proctoring form. Also, remember that the testing center may reach out to you using the contact information you supplied on the proctoring form with questions. Please respond to those questions as quickly as possible to help the testing center serve your students as quickly as possible. Sending the Completed Form with Microsoft Outlook 1. Open Outlook on your computer. 2. Open a New Email by clicking New Email on the top left of the screen. 3. Enter the email address for the regional testing center. Addresses can be found on the Click for Help tab on Blackboard under Where to Send Your Proctoring Form.
