PART 20 - Grandfathered Services Original Sheet 1 DIGITAL TRUNKING SERVICE A. Description Digital Trunking Service provides for Digital Trunk access from a central office via AT&T DS1 Service. This arrangement furnishes the equivalent of 24 Network Access Lines for use as Wide Area Telecommunications Service (WATS), or as P.B.X. trunks, which may or may not include Direct Inward Dialing (DID) Service. In addition, Digital Trunking Service as described below is also available via a cross-connection point within the Company s central office. This arrangement furnishes the equivalent of 24 ports for use as P.B.X. Ground Start ports as described in Part 19, Section 1, which may or may not include DID Service. Digital Trunking Service is classified as a local exchange, competitive telecommunications service for Business customers with twelve (12) or more lines in Access Area A, Access Area B, and the following districts: Arlington Heights, Bensenville, Champaign Main, Champaign University, Decatur Main, Decatur North, Deerfield, Downers Grove, Elk Grove, Elmhurst, Geneva, Glenview, Hinsdale, Lombard, Naperville, Northbrook, Springfield Lake, Springfield Main, Springfield West, Wheaton, Wheeling. In addition to the rates and charges following, the rates and charges for DID Service, as specified in Part 6 Section 1, and intrastate WATS access lines as specified in Part 10 are also applicable, as appropriate. For service provided via a cross-connection point within the Company s central office, in addition to the rates and charges following, the rates and charges for DID Service, as specified in Part 6, Section 1 are applicable as appropriate. Line Connection Charges for P.B.X. trunks are not applicable. The terms, conditions and rates specified herein are in addition to the applicable terms, conditions and rates specified in this and other Parts of this guidebook. B. Terms and conditions 1. Availability of Service Digital Trunking Service is provided subject to the availability of facilities. A service inquiry must, therefore, be made to determine availability of service. 2. Provision of Service a. Digital Trunking Service provided to the customer s premises is only provided in base capacities of 24 channels. These channels are available for access to any mix of P.B.X. trunks or WATS. Access to channel services is not available. b. Where Digital Trunking Service is provided via a cross-connection point within the Company s central office, the group of 24 ports are available with any mix of P.B.X. ports, which may or may not include Direct Inward Dialing (DID) Service. c. Service is furnished on a full-time basis (24 hours a day, seven days a week). ATT TN IL-14-0021 Effective: May 1, 2014

Digital Central Offices where no Line Side Capabilities or Services are required /D7W/ /1/ b. All Analog Central Offices and those Digital Central Offices where Line Side Capabilities such as Dial Tone, Ground Start and Multi-Line Hunt Group and/or Services such as Custom Calling are required /D8W/ Nonrecurring Charge $315.00 575.00 Month Rate $3,500.00(I) 4,220.00(I) 2. When Digital Trunking Service is provided to a customer premises: a. Rates and charges for Ameritech DS1 Service as specified in Part 15, Section 3 are applicable in addition to the rates in paragraphs 1 and 2. b. Service Charges as specified for 1.544 Mbps Digital Service Un-channelized Customer Premises to Central Office Lines are applicable for the establishment of the DS1 channels as specified in Part 15. c. In addition to the above rates, End User Common Line (EUCL) Charges as filed for the State of Illinois with the F.C.C. in Ameritech Operating Companies Tariff F.C.C. No. 2 are applicable. 3. When Digital Trunking Service is provided via a cross-connection point within the Company s central office: a. Service Charges are applicable to the establishment of service. b. Channel charges apply per PBX Port as specified in Part 7, Section 5, whether Cross Connection takes place at the same time as the Digital Trunking Service is ordered or at a later point in time. /1/ Digital Trunking does not include line side capabilities as described above. Therefore, only wink start, immediate start or delay start are available for these trunk side connections. ATT TN IL-17-0012 Effective: January 2, 2017

PART 20 - Grandfathered Services 7th Revised Sheet 3 DIRECT INWARD DIALING (DID) TO PBX SYSTEMS A. Reserved for Future Use B. Terms and Conditions Additional DID numbers can be reserved for future use by ordering numbers at rates specified in paragraph B. The Company does not guarantee to provide reserved numbers in a consecutive order. The Company will be responsible for interception and administration of these numbers. 1. Reserved for future use 2. Two-Way Direct Inward Dialing (DID) With Call Transfer a. Reserved for future use b. Additional numbers may be reserved for future use, in groups of 10, at the Rates and Charges specified herein. The assignment of numbers and the sequence of numbers assigned to a customer are made at the discretion of the Company. The Company will be responsible for providing intercept and for administration of reserved numbers. The customer is responsible for providing intercept on assigned but unused numbers associated with 2-Way DID With Call Transfer service. C. PART 20 - Grandfathered Services Original Sheet 4 AMERITECH CUSTOM BUSINESS (ACB) SERVICE Ameritech Custom Business Service (ACBS) will not be provided to new customers on or after July 15, 1994. Customers having this service or who have placed orders which were accepted by the company prior to this date, may continue such service from their present location. In the event the service at any location is terminated for the customer, such service will not be re-established. A. General 1. Ameritech Custom Business (ACB) Service is an optional telecommunications system arrangement which combines a maximum of six exchange access lines into an ACB group. ACB offers two types of services: a. Business Combines up to six exchange access lines into an ACB group. b. Combination Residence/Business Combines a maximum of one Residential and up to five Business exchange access lines into an ACB group. 2. ACB is available only from properly equipped electronic central offices. 3. Exchange access lines in an ACB group must all be terminated on the same premises. 4. Touch-Tone service is required with ACB. The rates and charges for Touch-Tone service are in addition to ACB rates and charges. 5. Although optional features are available through ACB, Custom Calling and Star Line Services may overlap; these services are distinct and are governed by their own respective rates and regulations. ACB optional features are offered to ACB customers. B. Feature Descriptions 1. Standard Features a. ACB Intercom Provides intercom calling between any two lines of the ACB group. b. ACB Call Hold Allows any call to be put on hold, freeing that line to pick up or make another call. c. ACB Call Pickup Enables any ACB line to answer any call made to any other ACB line. d. ACB Call Transfer Allows any call to be transferred between lines of an ACB group. e. ACB Conferencing Enables an ACB line to originate a three-way conference call. ACB Call Waiting Signals an ACB line when a second call is coming in on a busy line. This feature is not compatible with ACB automatic transfer. b. ACB Call Forwarding A customer activated feature that automatically directs incoming calls to another line either inside or outside the ACB group. 1. Where a charge is applicable between the customer s telephone and the telephone to which the calls are to be forwarded, such charge (dial station-to-station rate) is billed to the customer on every call forwarded and answered at that telephone. 2. ACB Multiple Calls Option Call Forwarding is available to ACB Call Forwarding customers with hunting arrangements who wish to have more than one call forwarded on an inter-central office basis. This feature is available where facilities permit. The charge for this feature is in addition to Call Forwarding rates. c. ACB Automatic Transfer Directs incoming calls to an ACB line to another predetermined ACB line if that ACB line is busy or not answered with a customer specified number of rings. This feature is not compatible with hunting or ACB Call Waiting. d. ACB Convenience Dialing Enables an ACB customer to individually access up to 30 frequently called or emergency numbers by dialing an access code followed by two digits. All lines in the ACB group must share the same convenience dialing list. e. ACB Distinctive Ringing Distinguishes between intercom and external calls. When a line is also equipped with ACB Call Waiting, the call waiting tone will also distinguish between intercom and external calls. ACB Distinctive Ringing must be placed on all lines of an ACB group if any such ringing is desired. /1/ Ameritech Custom Business Service (ACBS) will not be provided to new customers on or after July 15, 1994. Customers having this service or who have placed orders which were accepted by the Company prior to this date, may continue such service from their present location. Customers having this service or who have placed orders which were accepted by the Company prior to this date, may continue such service from their present location. In the event the service at any location is terminated for the customer, such service will not be re-established. /2/ A maximum of one Residential line is permitted. /3/ Where a charge is applicable between the customer s telephone and the telephone to which the calls are directed, such charge (dial station-to-station rate) is billed to the customer on every call directed and answered. /4/ Distinctive Ringing is only available in central offices equipped to provide this service. PART 20 - Grandfathered Services Original Sheet 7 PUBLIC SWITCHED DIGITAL SERVICE (PSDS) PSDS will not be provided to new customers on and after August 22, 1992. Customers having this service or who have placed an order and had it accepted by the Company as of that date, may continue such service from their present central office. In the event the service at any location is terminated for the customer, such service will not be reestablished. A. General 1. Public Switched Digital Service (PSDS) is a digital, switched telecommunications service that provides full duplex, synchronous, 56 kilobit per second information transport via a specially equipped Network Access Line. The service may be resold or shared. 2. PSDS offers two calling arrangements. Customers must specify which arrangement they require when ordering a PSDS equipped line. a. Arrangement 1 PSDS equipped Network Access Line which allows 56 Kbps digital calls only. b. Arrangement 2 PSDS equipped Network Access Line which allows both 56 Kbps digital calls and Public Switched Network calls. 3. A PSDS Access Code, either a PSDS Network Access Code or a PSDS/Centrex Intercom Call Access Code, must be dialed to indicate a PSDS call (providing 56 Kbps information transport) is being placed. If the PSDS Access Code is not dialed, a call placed on an Arrangement 1 Type line will not be completed and a call placed on an Arrangement 2 Type line will be completed as normal Telephone Exchange Service or Centrex Service depending on what kind of Network Access Line is being utilized to provide PSDS. B. Availability of Service 1. Public Switched Digital Service is furnished by the Company, over facilities solely within the Illinois portion only of the Market Service Areas listed below. PSDS is provided subject to the availability of appropriate network facilities and is normally provided from the closest designated PSDS serving central office. Customers outside the central office areas from which PSDS is offered may obtain the capability by subscribing to the appropriate Remote Network Access Line options in paragraph D.6 or D.7. The Remote Network Access Line charges are in addition to the other PSDS charges in paragraph D, and supersede normal foreign office charges such as Foreign Central Office and Foreign Exchange mileage charges. The Remote Network Access Line charges provide for mileage charges based on the airline distance mileage from a customer's normal serving central office to a PSDS serving central office. Terms and Conditions 1. Digital Transport Service - Enhanced is offered from central offices where the Company has arranged for facilities for such service. At the request of the customer and at the discretion of the company, DTS-E may be extended to central offices within the same Local Access Transport Area (LATA) through the application of DS1, DS3 Service, OC-n Point-to-Point Service, OC-n Dedicated Ring Service or other suitable Company provided services. Clear Channel capability is an optional DS1 feature that may be used in conjunction with DTS-E. Clear Channel allows the customer to transport 1.536 Mbps on a 1.544 Mbps line with no constraint on the quantity or bit sequence. Clear Channel nonrecurring charges, as noted elsewhere in this Guidebook, are applicable only when Clear Channel is ordered subsequent to initial installation of this service. 5. When a DTS-E subscriber is also the customer of record for the Company's high speed transport services, the DTS-E customer may request provisioning of DTS-E over their high speed transport service s vacant bandwidth. 6. The same customer of record is required for both DTS-E and the associated Company facility. Sharing of common transport or DTS-E arrangements between multiple customers is not permitted. 7. Payment Plans Month to Month The minimum period is one month, unless specified otherwise. The customer must agree to a TPP /1/ /2/ that equals or exceeds the number of months remaining on the existing DTS-E Module 1, 2, or 3 TPP. b. The number of circuits on the new TPP must be equivalent to or exceed the number of circuits under the existing TPP. c. The new TPP shall begin immediately upon conversion from the existing TPP. d. The new service must be provided between the same customer locations and with the same customer of record as the disconnected service. e. Nonrecurring charges for the new service will apply. All termination charges will be based on the TPP prices in effect at the time of termination. Termination charges are applicable for DS1 Service used in conjunction with DTS-E Service Modules 1, 2, and 3 as specified in Part 15, Section 3. /1/ Effective August 31, 2011, 3 Year and 5 Year Term Payment Plans are no longer available for new installations or renewals of DTS-E Modules 1, 2 and 3. DTS-E Module 1, 2 or 3 customers currently on a Term Payment Plan may continue service at their existing TPP rate until the contract term expires. Upon completion of the current contract term, customers may continue receiving the service by entering into a new 1 Year TPP agreement at the then current applicable 1 Year TPP rates. If the customer does not enter into a new 1 Year TPP agreement, or request discontinuance of the service, the month-to-month rates in effect at such time will automatically apply. /2/ Effective December 1, 2015, customers may not establish new Term Payment Plans (TPP) of any length for DTS-E Modules 1, 2 or 3, and existing TPPs may not be renewed.
