About FSFN The Florida Safe Families Network (FSFN) helps you understand the steps to complete your work in the FSFN system. This user guide does not cover every system feature built into FSFN, but describes the most commonly used functions to complete your day-to-day work in the FSFN application. This user guide, when used with Online Help and the How Do I Guides, helps you successfully use the FSFN system as a support tool in your important work of safeguarding the safety, permanence, and well-being of children, families, and adults of Florida. The Intended Audience This user guide serves a wide audience of FSFN end users who include: Adult and Child Protective Investigators Legal Workers Ongoing Case Managers Independent Living Staff Supervisors Support and Data Entry Staff Prerequisite Knowledge This user guide was developed with specific prerequisite expectations. Before reading this guide, please read the following information. If you need to refer to additional s, How Do I Guides, or other information, visit the DCF FSFN Website (http://fsfn.dcf.state.fl.us). You must have a working understanding of Florida Department of Children and Families Policies and Practices as related to the use and operation of FSFN. You must have a working understanding of the basic FSFN navigation and functionality. See the Multi-Topic Basics and How Do I Guide. If you are a supervisor, also see the Multi-Topic Supervisory and How Do I Guide. Key tasks described in this guide start at the FSFN Desktop, Case Book, or Person Book, unless otherwise noted. If you are unfamiliar with how to navigate to these three pages, see the s for Multi-Topic Basics, Case Book, and Person Book. The Search page referred to within this user guide is the Search page with four (4) tabs: Person, Case, Provider/Organization, and Worker. This Search page is commonly called the Utility Search or Navigational Search. It is accessed primarily from the Desktop by clicking Search on the Banner or Utilities menu. Other type of searches may be referred to as data retrieval searches. These searches use different search functionality than the Utility Search. For example, the Person Search page provides a method to search for a person December 15, 2013 Page 6 of 51

and retrieve person data that pre-fills into the page in which you are working. The Person Search page should not be confused with the Search page with the Person tab. About the Young Adult Case Planning The Young Adult Case Planning functionality in Florida Safe Families Network (FSFN) supports the development of the Young Adult Case Plan. As you work with the case, the Young Adult Case Plan follows the young adult formerly in Foster Care until age 23 through documenting the planning and preparation activities, as well as progress and participation of youth and young adults over the course of time. This supports improved accountability for work and efforts made toward assisting youth in achieving independence as an adult, and provide a complete history of their progress while involved in the Independent Living Program. Young Adult Case Plans are goal directed, and must clearly identify outcomes, key actions and how the progress of the plan is measured. The following should always be clearly stated in the plan: A summary of the young adult's current living arrangements; The services to be provided to the young adult; Clear expectations about desired outcomes with associated actions and tasks; and The time frames in which to complete the actions and tasks. There are two primary components in the FSFN Young Adult Case Planning functionality: Young Adult Case Plan Worksheet page and Young Adult Judicial Review Worksheet page. These components are made up of numerous tabs and pop-up pages to support documentation of services, and activities that have been put in place to support and verify the well-being and progress toward independency of the young adult for whom the plan has been designed. The Young Adult Case Plan Worksheet page captures information pertaining to all planning activities. On the Young Adult Case Plan Worksheet page, you identify services to assist the young adult and provide specified time frames for completion of noted tasks. Information from the Young Adult Case Plan Worksheet page is used to pre-fill the Young Adult Judicial Review Worksheet page. On the Young Adult Judicial Review Worksheet page, you capture additional information such as the young adult s progress toward desired outcomes and other pertinent information about the young adult to present to the Court. With the information captured on the Young Adult Case Plan Worksheet page and Young Adult Judicial Worksheet page, you can produce written documents, such as the Young Adult Court Involved Case Plan and Young Adult Judicial Review. Young Adult Case Plan Worksheet Page About the Young Adult Case Plan Worksheet Page Within the Young Adult Case Plan Worksheet page, you capture the supervised living arrangement, education, employment and medical/mental health information, outcomes and attachments. The Young Adult Case Plan Worksheet consists of some pre-filled information which determines the information that is captured on the page within the five tabs: Supervised Living Arrangement: contains information regarding the type of living arrangement and current information about how the living arrangement supports the goals of the young adult. Education: contains information regarding the young adult s education. The education information is pre-filled from the Education page within the FSFN Case that is associated to the young adult. Employment and Medical/Mental Health: contains information for the young adult. The employment information is pre-filled from the Assets/Employment page within the FSFN Case for the young adult. Similarly, the medical/mental health is retrieved from the Medical/Mental Health page within the FSFN Case. Outcomes: captures the outcomes identified for the young adult and associated actions and tasks in order to achieve the desired outcome. Attachments: provides a checklist of documentation for Education, Independent Living, and Optional attachments that needs to be attached. The Actions and Text List Box on the Independent Living page allows the user to create/open the Young Adult Case Planning Worksheet or open the Young Adult Judicial Review Worksheet. There will be only one Young Adult Case Planning Worksheet page within the Independent Living module. The Young Adult Case Planning Worksheet page is launched in edit mode if the user has appropriate security profile, the user has an open assignment to the open FSFN Case, and the page was not accessed via the Search Utility page. If it is accessed from the Search Utility page the Young Adult Case Planning page will be in view-only mode regardless of user assignment to the FSFN case. Using the Young Adult Case Plan Worksheet page, you can produce documents such as, Court Involved Case Plan legal document. December 15, 2013 Page 9 of 51

About Creating a Young Adult Case Plan Worksheet Page You can create a Young Adult Case Plan Worksheet page from the Independent Living page. The young adult must be of age 18 or older before you can create a Young Adult Case Plan Worksheet page. Key Tasks To create a Young Adult Case Plan Worksheet from the Independent Living page 1. In the Actions List Box, click the Create Young Adult Case Planning Worksheet hyperlink. December 15, 2013 Page 10 of 51

The Young Adult Case Plan Worksheet page displays. See next section for information on completing the worksheet. About Accessing a Young Adult Case Plan Worksheet Page Once a Young Adult Case Plan Worksheet page is created, you can access it from the Independent Living page from which it was created. You do not need to be assigned to the case to view a Young Adult Case Plan Worksheet page from the Search page. You may not edit it when accessed from Search, even with an assignment to the case. Supervisors can access Young Adult Case Plan Worksheet pages, in view-only mode, associated to workers they supervise from the Workers expando on their FSFN Desktop. Key Tasks To access a Young Adult Case Plan Worksheet from the Independent Living page 1. In the Actions List Box, click the Open Young Adult Case Planning Worksheet hyperlink. The Young Adult Case Plan Worksheet page displays. See next section for information on completing the worksheet. To access a Young Adult Case Plan Worksheet from the Search page 1. On the FSFN Desktop, click Search on the banner. OR 1. On the Desktop, from the Utilities menu, click Search. 2. From either the Case or Person tab, conduct a search to locate the appropriate case or person. For more information on searching for a case or person, see the Search User Guide on the DCF FSFN Website (http://fsfn.dcf.state.fl.us). 3. After the search is conducted and the appropriate case or person is found, expand the case or person in the search results. 4. Click the Independent Living icon. 5. Click the appropriate Independent Living hyperlink. 6. In the Actions List Box, click the Open Young Adult Case Plan Worksheet hyperlink. The Young Adult Case Plan Worksheet page displays in view-only mode. The Young Adult Case Plan Worksheet page is only accessible in view-only mode from the Search page, even if you have an assignment to the case. December 15, 2013 Page 12 of 51

About the Case Information When you create or access a Young Adult Case Plan Worksheet page, the Case Information group box displays at the top of the page. It contains basic information on the associated FSFN Case; the selected Case information pre-filled the Young Adult Case Plan Worksheet page when you created it. It also contains the Case Plan Annual Date that you can enter. After the Young Adult Case Plan Worksheet page is initially successfully saved, FSFN generates a Case Plan Worksheet ID. It is displayed in the Case Information group box. The Last Updated By displayed in the Case Information group box is the user that created the page. Key Tasks To access the Maintain Case page 1. In the Case Information group box, click the Case Name hyperlink. 2. The Maintain Case page displays for the case associated to the Young Adult Case Plan Worksheet page. The Maintain Case page displays in edit mode if you accessed the Independent Living page from your FSFN Desktop under the Cases Outliner, or Case Book; otherwise, it is view-only. For more information on the Maintain Case page, see the Maintain Case User Guide on the DCF FSFN Website (http://fsfn.dcf.state.fl.us). 3. Modify the Maintain Case page as needed if it is in edit mode. 4. When you finish editing or viewing the Maintain Case page, click Save and Close. 5. The Young Adult Case Plan Worksheet page displays. 6. At the bottom of the Young Adult Case Plan Worksheet page, click the Refresh button. It is important to refresh the Young Adult Case Plan Worksheet page when information is updated on another page associated to the case. For example, if you updated the Case Name, the new Case Name displays after refreshing. To access the Person Management page 1. In the Case Information group box, click the Young Adult hyperlink. 2. The Person Management page displays for the young adult. The Person Management page displays in edit mode if you accessed the Independent Living page from your FSFN Desktop under the Cases Outliner, or Case Book; otherwise, it is view-only. For more information on the Person Management page, see the Person Management on the DCF FSFN Website (http://fsfn.dcf.state.fl.us). December 15, 2013 Page 13 of 51

3. Modify the Person Management page as needed if it is in edit mode. 4. When you finish editing or viewing the Person Management page, click Save and Close. 5. The Young Adult Case Plan Worksheet page displays. 6. At the bottom of the Young Adult Case Plan Worksheet page, click the Refresh button. It is important to refresh the Young Adult Case Plan Worksheet page when information is updated on another page associated to the case. For example, if you updated the Date of Birth for the young adult, the new Date of Birth displays after refreshing. December 15, 2013 Page 14 of 51

Case Plan - Supervised Living Arrangement Tab About the Supervised Living Arrangement Tab The Supervised Living Arrangement tab contains information regarding the type of living arrangement and current information about how the living arrangement supports the goals of the young adult. The living arrangement information is pre-filled from the Living Arrangement pages within the FSFN Case that is associated to the young adult after they turned 18. The Supervised Living Arrangement group box displays the Begin Date/Time; End Date/Time; Type; Primary Caregiver; and Secondary Caregiver. Key Tasks To complete the Living Arrangement questions 1. On the Supervised Living Arrangement tab, complete the four narrative fields. How does the current supervised living arrangement provide the young adult with a level of supervision consistent with his or her individual education, health care needs, permanency plan, and independent living goals? How does the current supervised living arrangement offer, at a minimum, life skills instruction, counseling, education support, employment preparation and placement and development of support networks? Describe the strengths of the current living arrangement. Describe any problems with the current living arrangement, if any. Case Plan - Education Tab About the Education Tab The Education tab contains information regarding the young adult s education. The education information is pre-filled from the Education page within the FSFN Case that is associated to the young adult. These fields are not editable directly on the Young Adult Case Plan Worksheet, but can be edited through the Education hyperlink. Key Tasks To access the Education page 1. In the Education group box, click the Education hyperlink. 2. The Education page displays for the young adult in the FSFN Case. The Education page displays in edit mode if you accessed the Independent Living page from your FSFN Desktop under the Cases Outliner, or Case Book; otherwise, it is view-only. For more information on the Education page, see the Education on the DCF FSFN Website (http://fsfn.dcf.state.fl.us). 3. Modify the Education page as needed if it is in edit mode. 4. When you finish editing or viewing the Education page, click Save and Close. 5. The Young Adult Case Plan Worksheet page displays. 6. At the bottom of the Young Adult Case Plan Worksheet page, click the Refresh button. It is important to refresh the Young Adult Case Plan Worksheet page when information is updated on another page associated to the case. For example, if you updated the School Name, the new School Name displays after refreshing. To complete the Education questions: 1. On the Education tab, answer the two questions and complete the narrative fields. Is the young adult performing on education level? Is the young adult also in Post-Secondary Educational Services and Support (PESS)?. The text box fields are not pre-filled and can hold up to 3,200 characters. December 15, 2013 Page 17 of 51

Case Plan - Employment/Medical/Mental Health Tab About the Employment/Medical/Mental Health Tab The Employment and Medical/Mental Health tab contains information for the young adult. The employment information is pre-filled from the Assets/Employment page within the FSFN Case for the young adult. Similarly, the medical/mental health is retrieved from the Medical/Mental Health page within the FSFN Case. These fields are not editable directly on the Young Adult Case Plan Worksheet, but can be edited through the Employment or Medical/Mental Health hyperlinks. If the user does not have the applicable security to view Medical/Mental Health records, the entire Medical/Mental Health group does not display. Key Tasks To access the Assets and Employment page 1. In the Employment group box, click the Employment hyperlink. 2. The Assets and Employment page with the Assets/Liabilities tab displays for the young adult in the FSFN Case. At the bottom of the Young Adult Case Plan Worksheet page, click the Refresh button. It is important to refresh the Young Adult Case Plan Worksheet page when information is updated on another page associated to the case. To complete the Employment question: 1. On the Employment/Medical/Mental Health tab within the Employment tab, answer the question and complete the narrative field. Is the young adult participating in a program or activity designed to promote or eliminate barriers to employment? The text box field is not pre-filled and can hold up to 3,200 characters. To access the Medical/Mental Health page If the user does not have the applicable security to view Medical/Mental Health records, the entire Medical/Mental Health group does not display. 1. In the Medical/Mental Health group box, click the Medical/Mental Health hyperlink. 2. The Medical/Mental Health page with the Medical Profile tab displays for the young adult in the FSFN Case. Case Plan - Outcomes Tab About the Outcomes Tab The Outcome tab captures the outcomes identified for the young adult and associated actions and tasks in order to achieve the desired outcome. The tab contains repeating group boxes with the outcomes details. Upon clicking the Add Outcome button, FSFN inserts a new Outcome group box. The user can document the provider of the services if needed. The user can use the Provider Search page by clicking the Search hyperlink to identify a provider within FSFN or enter the provider information. The user can insert a new action or task associated with the inserted outcome record by clicking the Add Action/Task button. Key Tasks To add an Outcome 1. On the Outcome tab, click the Add Outcome button. FSFN adds an Outcome group box. 2. In the Outcome field, enter a description of the identified outcome. The field is required. (up to 200 characters) 3. In the Outcome Achievement field, enter a description of what constitutes the achievement of the identified outcome. The field is required. (up to 200 characters) 4. In the Est. Cost to Young Adult(s) (if applicable) field, enter an amount if appropriate. The field is not required. To delete an Outcome The Est. Cost to Young Adult (if applicable) field should reflect the total estimated cost for the young adult to achieve the entire outcome. For example, if the cost is $2 per session and there are 10 sessions, the total estimated cost is $20. If there are two Action/Tasks, this is a total of the young adult contribution on both Action/Tasks. 1. On the Outcome tab, click the appropriate Delete hyperlink associated to the Outcome row you wish to delete. To add an Action/Task When you click a Delete hyperlink, FSFN generates a validation message asking if you want to continue with the deletion. Any associated Actions/Tasks are deleted too. If Yes, the row no longer displays. Upon successfully saving the Young Adult Case Plan Worksheet page, the row is deleted from the database. 1. From the Service Referral Request Needed? drop down, select Yes or No. 9. In the Frequency of Service field, enter the frequency by which the service needs to occur. If the provider is in FSFN, you can click the Search hyperlink to search the appropriate provider and the Provider Name displays in the field. The Young Adult Judicial Review Worksheet consists of some pre-filled information which determines the information that is captured on the page within the five tabs: Supervised Living Arrangement: contains information regarding the type of living arrangement and current information about how the living arrangement supports the goals of the young adult. Resources/Participation: contains information about the resources that are being provided to meet the young adult s needs and the assistance that is provided to the foster parents or legal guardian to help meet the young adult s needs. Outcomes Evaluation: includes the outcomes and actions/tasks from the Young Adult Case Planning Worksheet page. Review Summary: captures the summary of Judicial Review activities such as the significant changes since the last Case Plan or Judicial Review, progress, and recommendations for the young adult. Attachments: provides a checklist of documentation for Education, Independent Living, and Optional attachments that needs to be attached. The Actions and Text List Box on the Independent Living page allows the user to create/open the Young Adult Case Planning Worksheet or open the Young Adult Judicial Review Worksheet. The Young Adult Judicial Review Worksheet is created from the Young Adult Case Plan Worksheet. There will be only one Young Adult Judicial Review Worksheet page within the Independent Living module. If the Young Adult Judicial Review Worksheet page has not been created and saved, the Young Adult Judicial Review Worksheet hyperlink in the Actions and Text List Box on the Young Adult Case Planning Worksheet page will launch a new Young Adult Judicial Review Worksheet page with data pre-filled from the Young Adult Case Planning Worksheet page and other supporting pages such as Living Arrangement and Legal Actions. On the Supervised Living Arrangement tab, click the Update questions from Case Plan Worksheet button. OR 1. Update the text in the narrative fields. How does the current supervised living arrangement provide the young adult with a level of supervision consistent with his or her individual education, health care needs, permanency plan, and independent living goals?
