Compare the test version number on the Teleform sheet supplied with the version number in the top left corner of this page. If they do not match, ask the test supervisor for a new sheet. 2. Enter your name and Student ID (in pencil) on the Teleform sheet and shade in the corresponding bubbles underneath. Your name and Student Id should both be entered left aligned.
